In some cases, drug interactions with other drugs, both licit and illicit, have been suspected. However, in other cases, deaths appear to have occurred due to the respiratory or cardiac effects of methadone and too-rapid titration without appreciation for the accumulation of methadone over time. It is critical to understand the pharmacokinetics of methadone and to exercise vigilance during treatment initiation and dose titration (see DOSAGE AND ADMINISTRATION). Patients must also be strongly cautioned against self- medicating with CNS depressants during initiation of methadone treatment. Respiratory depression is the chief hazard associated with methadone hydrochloride administration. Methadone's peak respiratory depressant effects typically occur later, and persist longer than its peak analgesic effects, particularly in the early dosing period. These characteristics can contribute to cases of iatrogenic overdose, particularly during treatment initiation and dose titration. Cases of QT interval prolongation and serious arrhythmia (torsades de pointes) have been observed during treatment with methadone. Current Fluid Therapy Topics and Recommendations During Anesthetic Procedures Andrew Claude, DVM, DACVAA Mississippi State University Mississippi State, MS • Intravenous fluid administration is recommended during general anesthesia, even during short procedures. • The traditional IV fluid rate of 10 mls/kg/hr during general anesthesia is under review. • Knowledge of a variety of IV fluids, and their applications, is essential when choosing anesthetic protocols for different medical procedures. Anesthetic drug effects on the cardiovascular system • Almost all anesthetic drugs have the potential to adversely affect the cardiovascular system. • General anesthetic vapors (isoflurane, sevoflurane) cause a dose-dependent, peripheral vasodilation. • Alpha-2 agonists initially cause peripheral hypertension with reflex bradycardia leading to a dose-dependent decreased patient cardiac index. As the drug effects wane, centrally mediated bradycardia and hypotension are common side effects. • Phenothiazine (acepromazine) tranquilizers are central dopamine and peripheral alpha receptor antagonists. This family of drugs produces dose-dependent sedation and peripheral vasodilation (hypotension). • Dissociative NMDA antagonists (ketamine, tiletamine) increase sympathetic tone soon after administration. When dissociative NMDA antagonists are used as induction agents in patients with sympathetic exhaustion or decreased cardiac reserve (morbidly ill patients), these drugs could further depress myocardial contractility. • Propofol can depress both myocardial contractility and vascular tone resulting in marked hypotension. Propofol’s negative effects on the cardiovascular system can be especially problematic in ill patients. • Potent mu agonist opioids can enhance vagally induced bradycardia. Pharmacology- Inhalant Anesthetics Lyon Lee DVM PhD DACVA Introduction • Maintenance of general anesthesia is primarily carried out using inhalation anesthetics, although intravenous anesthetics may be used for short procedures. • Inhalation anesthetics provide quicker changes of anesthetic depth than injectable anesthetics, and reversal of central nervous depression is more readily achieved, explaining for its popularity in prolonged anesthesia (less risk of overdosing, less accumulation and quicker recovery) (see table 1) Table 1. Injectable Anesthesia in South American Camelids
INJECTABLE ANESTHESIA IN SOUTH AMERICAN CAMELIDS Thomas Riebold DVM, Diplomate ACVAA Veterinary Teaching Hospital College of Veterinary Medicine Oregon State University Corvallis, Oregon 97331 INTRODUCTION Interest in llamas, and more recently in alpacas, as pets and as breeding and pack animals has led to increased demand for veterinary services for them. While they have some unique species characteristics regarding anesthesia, many of the principles and techniques used in food animal and equine anesthesia also apply to South American camelids. Except for differences in size, anesthetic management of alpacas and llamas is similar. Much like there are species differences between cattle, sheep, and goats in their response to xylazine, it does appear that alpacas require higher doses of sedatives, approximately 10-20%, to obtain the same response that lower doses of sedatives would obtain in llamas. PREANESTHETIC CONSIDERATIONS Consideration for preanesthetic preparation includes fasting, assessment of hematologic and blood chemistry values, venous catheterization, and estimation of bodyweight. The camelid has a stomach divided into three compartments. Therefore, potential complications similar to those of domestic ruminants, regurgitation and aspiration pneumonia, exist during anesthesia. Abdominal tympany as it occurs in anesthetized domestic ruminants does not appear to occur in anesthetized camelids. It is recommended that the animals be fasted 12-18 hours and deprived of water for 8-12 hours. In nonelective cases, this is often not possible and precautions should be taken to avoid aspiration of gastric fluid and ingesta. Fasting neonatal camelids is not advisable because hypoglycemia may result. As in other species, hematologic and blood chemistry values are determined before anesthesia. -

NDA 19-890/S-017 Page 3 ® STADOL (butorphanol tartrate) Injection, USP STADOL NS® (butorphanol tartrate) Nasal Spray DESCRIPTION Butorphanol tartrate is a synthetically derived opioid agonist-antagonist analgesic of the phenanthrene series. The chemical name is (-)-17-(cyclobutylmethyl) morphinan-3, 14-diol [S- (R*,R*)] - 2,3 - dihydroxybutanedioate (1:1) (salt). The molecular formula is C21H29NO2,C4H6O6, which corresponds to a molecular weight of 477.55 and the following structural formula: Butorphanol tartrate is a white crystalline substance. The dose is expressed as the tartrate salt. One milligram of the salt is equivalent to 0.68 mg of the free base. The n-octanol/aqueous buffer partition coefficient of butorphanol is 180:1 at pH 7.5. STADOL (butorphanol tartrate) Injection, USP, is a sterile, parenteral, aqueous solution of butorphanol tartrate for intravenous or intramuscular administration. In addition to 1 or 2 mg of butorphanol tartrate, each mL of solution contains 3.3 mg of citric acid, 6.4 mg sodium citrate, and 6.4 mg sodium chloride, and 0.1 mg benzethonium chloride (in multiple dose vial only) as a preservative. NDA 19-890/S-017 Page 4 STADOL NS (butorphanol tartrate) Nasal Spray is an aqueous solution of butorphanol tartrate for administration as a metered spray to the nasal mucosa. Each bottle of STADOL NS contains 2.5 mL of a 10 mg/mL solution of butorphanol tartrate with sodium chloride, citric acid, and benzethonium chloride in purified water with sodium hydroxide and/or hydrochloric acid added to adjust the pH to 5.0. The pump reservoir must be fully primed (see PATIENT INSTRUCTIONS) prior to initial use. -

In Tamil Nadu & Goa, since 2007, the Animal Birth Control and Anti-Rabies Vaccination Programme has been successfully implemented for the entire state. This has led to Tamil Nadu state pioneering a new concept of a Participatory Model of the ABC Programme in 50 Municipalities and 5 Municipal Corporations, with 50% cost sharing by local bodies on participatory basis. Similarly, the Union Territory of Delhi too has adopted the Participatory Model of the ABC Programme since 2008. Tamil Nadu has also been at the forefront of rabies control initiatives, having constituted the country’s first State level Coordination Committee on Rabies Control and Prevention in January, 2009, with the first meeting held on April 20th, 2009. The Animal Welfare Board of India is promoting such initiatives throughout the country. In all Metros, where the ABC Programme has been successfully implemented in India, a significant reduction in the number of human rabies cases has been noted. The Animal Birth Control Programme is the only scientifically proven method to reduce the stray dog population in a city or town. -

Drugs that produce anesthesia may or may not provide analgesia (1, 2). Analgesia: The absence of pain in response to stimulation that would normally be painful. An analgesic drug can provide analgesia by acting at the level of the central nervous system or at the site of inflammation to diminish or block pain signals (1, 2). Sedation: A state of mental calmness, decreased response to environmental stimuli, and muscle relaxation. This state is characterized by suppression of spontaneous movement with maintenance of spinal reflexes (1). Animal anesthesia and analgesia are crucial components of an animal use protocol. This document is provided to aid in the design of an anesthetic and analgesic plan to prevent animal pain whenever possible. However, this document should not be perceived to replace consultation with the university’s veterinary staff. As required by law, the veterinary staff should be consulted to assist in the planning of procedures where anesthetics and analgesics will be used to avoid or minimize discomfort, distress and pain in animals (3, 4). Prior to administration, all use of anesthetics and analgesic are to be approved by the Institutional Animal Care and Use Committee (IACUC). -

However, the use of guidelines, checklists, and training can decrease the risk of anesthesia-related adverse events. These tools should be used not only during the time the patient is unconscious but also before and after this phase. The framework for safe anesthesia delivered as a continuum of care from home to hospital and back to home is presented in these guidelines. The critical importance of client commu- nication and staff training have been highlighted. The role of perioperative analgesia, anxiolytics, and proper handling of fractious/fearful/aggressive patients as components of anesthetic safety are stressed. Anesthesia equipment selection and care is detailed. The objective of these guidelines is to make the anesthesia period as safe as possible for dogs and cats while providing a practical framework for delivering anesthesia care.
