DISCRIMINATION POLICIES

Saluki Athletics is committed to providing an atmosphere free from bullying, harassment, intimidation or discrimination of any kind. All definitions and policies can be found in the SIU Student Conduct Code: https://srr.siu.edu/_common/documents/scc.pdf

What is prejudice? Inflexible and irrational attitudes held by members of one group about another. This can lead to behavior, called discrimination.

What is discrimination? Unequal or different treatment that causes harm.

Bullying Any intentional, repeated and aggressive act, whether physical, verbal or otherwise communicated with the intent to cause another person physical, mental, or emotional harm or which is intended to coerce an individual to act or refrain from acting.

Harassment Intentional conduct, without a legitimate purpose, which both does cause a person distress or fear and would cause a reasonable person distress or fear. No threat or overt act of violence is required.

Harassment via technology is the use of electronic or other technology, without a valid purpose, to intentionally intimidate, embarrass, ridicule, or humiliate another person. This includes the creation or distribution of video, audio, or photographic files without the consent of all parties recorded or photographed which could foreseeably intimidate, embarrass, ridicule or humiliate and does cause another person to be intimidated, embarrassed, ridiculed or humiliated. *See social media policy p.7

Hazing Any action required of or imposed on current or potential members of a group which, regardless of the consent of the participants, produces or is reasonably likely to produce bodily harm, humiliation or ridicule, substantial interference with academic efforts, or significant impairment or endangerment of physical well-being.

Examples of hazing include, but are not limited to:  Forced consumption of alcohol or other substances  Sleep deprivation  Use of alcohol in drinking games or contests  Paddling  Forced tattooing or branding  Creation of excessive fatigue  Severe shocks or humiliation  Theft or misuse of property belonging to others

Consent  Consent is a clear, affirmative, unambiguous and freely given agreement to engage in a specific sexual activity.  Consent is demonstrated verbally or through actions that clearly indicate a willingness to engage in the specific sexual activity.  Lack of verbal or physical resistance does not constitute consent.  Consent to engage in sexual activity with one person does not constitute consent to engage in sexual activity with another person, and consent for a specific activity does not imply consent for any other activity.  Use of alcohol, drugs, or other intoxicants does not diminish one’s responsibility to obtain consent.  Consent must be knowing and voluntary.  To give consent, a person must be awake, of legal age, and have the capacity to reasonably understand the nature of his/her actions.  Consent cannot be given by an individual who is mentally or physically incapacitated through the effect of drugs, alcohol or other intoxicants or for any other reason.  Consent cannot be given when it is coerced, forced, or obtained by use of duress, fear, threats, or violence.  Consent is not implied by the existence of a prior or current relationship or participation in prior sexual activity.  A person’s manner of dress does not constitute consent.  Consent to engage in sexual activity may be withdrawn at any time and is automatically withdrawn by a person who is no longer capable of giving consent.

Sexual Harassment Any unwelcome sexual advance, request for sexual favors, or any conduct of a sexual nature which has the purpose or effect of:  Substantially interfering with a student’s academic performance, ability to participate or benefit from any of the academic, athletic, educational, employment, extra-curricular or other programs of the university  Creating an intimidating, hostile or offensive environment

Sexual Assault Any sexual act directed against another person forcibly and/or against a person’s will or where the person in question does not or is unable to give consent. 17 Southern Illinois University

SOCIAL MEDIA POLICY

Websites such as Facebook, Twitter and Instagram have become a widespread part of the college experience. These personal websites can be viewed by thousands of people including fellow students, faculty, administrators and future employers. As ambassadors of SIU and Saluki Athletics, you are always in the public eye and should exercise great care when displaying any personal information in a public forum.

If you participate on a social networking site or use social media, you must keep the following guidance in mind:

 Everything you post is public information – any text or photo placed online is completely out of your control the moment it is placed online – even if you limit access to your site.  Information (including pictures, videos, comments, and posters) may be accessible even after you remove it.  What you post can harm others. You are free to express yourself on social networks in ways that you feel are appropriate for you. However, it is important that you respect the privacy and rights of others. Posting things about others can place both you and your subject in a contentious situation.  What you post may affect your future. Many employers and graduate school admissions officers review social networking sites as part of their overall evaluation of an applicant. Carefully consider how you want people to perceive you before you give them a chance to misinterpret your information (including pictures, videos, comments, and posters).  Use this spotlight in a positive manner. Show support for other teams, activities, and current events at Southern Illinois University. Alert the Saluki Athletics Compliance Office of any sites that falsely appear to be yours as this constitutes identity theft, and the compliance office will assist your efforts to have the offensive site removed.

SIU PARKING POLICIES

Parking  All students must acquire parking permits prior to parking on campus.  Students may purchase a decal at the Parking Division office (Trueblood Hall), 7:30 a.m. to 4:30 p.m., Monday through Friday.  You can purchase a parking decal online at https://siuparking.t2hosted.com/per/index.aspx  The campus uses a color-coded parking system to distinguish parking lots where an individual may park. To check out the color you need: http://parking.siu.edu/  Student-athletes with cars must also complete an athletics car registration form for NCAA compliance purposes.

If you do not purchase a parking decal, there are two alternative parking sources available nearby:

PARKING 101  Valid parking decals must be displayed on the vehicle according to the instructions printed on the peel-off label of the decal.  All parking lot entrances display signs indicating the type of parking decal required.  Posted signs and painted curbs must be obeyed at all times.  The fact that a motor vehicle is parked illegally is not an excuse for you to do the same.  If you park illegally and don’t receive a ticket, don’t assume that it is ok to park there.  Learn the rules and know where and how to park lawfully.  If you are a student not eligible to purchase parking privileges, you may not park at metered spaces regardless of whether or not the space is signed "Visitors Only". You must purchase parking privileges off-campus.  Appeals of parking citations must be made within 5 business days of receiving the ticket. Appeal instructions are printed on the ticket.  If your vehicle becomes disabled on campus or has been towed, call the Department of Public Safety at (618) 453-2381 immediately. DPS is open 24 hours a day.  If you have a question about parking, contact Parking Division at (618) 453-5369.

Student-Athlete Parking When Away From Campus  Overnight parking is allowed in two specific locations where student-athletes may park their vehicles while attending events away from campus: 1) lot 100, located at the corner of Washington and Mill Streets, and 2) lot 56, located south of Saluki Drive.  All vehicles that park in these lots must have one of the following decals properly displayed: unexpired red commuter, yellow commuter or green overnight decals.  All student-athletes’ vehicles parked on campus in the authorized locations above, must display their valid parking decal.  Eligible student-athletes wishing to utilize the parking exception and who live off campus and have not purchased a parking decal, must stop by the Parking Division during normal business hours to register their vehicle and obtain at least a yellow commuter decal, and properly display the parking decal on the vehicle prior to leaving for the away event.  Student-athletes not eligible for or who choose not to purchase parking privileges, may not park their vehicles on campus at any time. These vehicles must be parked at an off campus location.  Vehicles not displaying a valid parking decal, regardless of eligibility, may be ticketed or towed from campus at the student-athlete’s expense.

Bicycles  All bicycles parked on campus are required to be registered, either by SIU or by the City of Carbondale. Registering your bicycle helps local law enforcement in the recovery of your bike in the event of loss or theft.  The cost to register a bicycle for students and staff on campus is FREE.  Students must bring their bicycle along with their student ID to the parking division office to complete the registration progress.  If you will be riding your bicycle to the SIU Arena, bike racks are located in the front the building (across from the Engineering building), in front of the Troutt-Wittmann Academic Center and in the back parking lot of the arena near the Troutt-Wittmann Weight Room.  You are expected to park and lock your bike in one of the bicycle racks.  Bikes are not to be locked to light posts, building stairs or left near doorways.

UNIFORMS AND EQUIPMENT CHECKOUT POLICY Each student-athlete will be provided with the appropriate uniform and/or equipment for the sport activity in which he/she will be participating.

Checkout Procedures The student-athlete’s name and the issue of numbers of each item to be borrowed shall be recorded by a member of the equipment staff.

Responsibility Each student-athlete will be responsible for the uniform and equipment items issued to her/him throughout the loan period. Normal wear and tear on items is expected and will be understood. When equipment becomes worn or torn, it will be replaced when the item is returned by the student-athlete; however, items which appear to be abnormally abused must be repaired or replaced by the student-athlete. Student-athletes will be charged their Bursar Account for items not returned upon departure from the team, lost or ruined while under her/his care.

Due Date The exact equipment and/or uniform items checked out to each student-athlete are to be returned to the equipment room within five (5) calendar days following the team’s final competitive event of the season. The 5-day return period may be altered at the discretion of the Director of Equipment Operations. Failure to return items by the due date established for the team will result in the student-athlete being charged their Bursar Account for those items.

Denial Intercollegiate Athletics may refuse to issue uniforms, awards, and/or equipment items to a student-athlete until all previous debts are paid. These previous debts are late fees, replacements fees, and repair fees.

Laundry  Laundry services are provided to student-athletes for work out, practice and competition apparel.  Student-athletes must allow 24 hours for clothes to be laundered.  Special times may be considered if the equipment room schedule allows for it.  Please see a member of the equipment room staff for special times.

Pandemic Locker Room Rules

***AT THIS TIME LOCKER ROOMS ARE OFF LIMITS UNTIL FURTHER NOTICE***  All Laundry is to be turned in & picked up at the hampers provided on the Equipment Room Loading Dock.  Arrangements will be made with each sport as to when and how laundry will be distributed once practices and games resume in the fall.  The situation is very fluid, please be respectful of our time and any adjustments that need to be made. We are all in this together.

The following laundry and locker room procedures will be in place after the lockout is lifted. Operation: 1. No one is allowed beyond the front counter unless granted permission by a member of the equipment staff. 2. Hours of operation will be based on when workouts & practices will occur. 3. Student-Athletes with a higher risk factor, under the consultation of Athletic Medicine, will be provided a laundry bucket to turn their gear separately.

Locker Rooms: 1. Limit your time to 15 minutes to occupy the locker room. 2. Do not hang out as a group in the locker room. 3. All lockers must be straightened up and tidy at all times to allow building services & the equipment staff to come in and sanitize the lockers & locker rooms. Take home what you DON’T need. 4. Locker rooms will be checked daily by the equipment staff to check for any laundry that may have been forgotten to be turned in. 5. If you notice someone else’s laundry or towel laying around please turn it in to be washed. 6. Any trash you have and throw it away! 7. DO NOT keep half full cups or bottles of Gatorade, protein shakes, milk or water in your locker areas. Empty them and throw them out if you are finished. 8. Throw away all food when you are finished. Do not let it sit in your lockers.

Laundry: 1. ALL laundry must be washed in the Boydston Equipment Room.

2. Athletes will have 15 minutes following workouts and practices to turn in laundry so we can wash it as soon as possible.

3. Sport & Unit Specific Laundry Procedures a. Volleyball – laundry will now be turned into the hamper provided in the women’s general locker room or Davies locker room depending on workout/practice location. b. Women’s Basketball, Men’s Basketball & Soccer – laundry will continue to be collected in their respective locker rooms. c. Football – Laundry will continue to be collected in the Boydston Equipment Room. d. Staff Laundry – Will be turned into the Boydston Equipment Room laundry chutes. All laundry will then be returned to their respective locker rooms. e. Athletic Training – Towels must be turned into the Boydston Equipment Room. f. Weight Room – Towels must be turned into the Boydston Equipment Room.

Plus/Minus Grading

 Effective with the Summer 2017 term, the standard letter grading system is being modified to allow the use of plus/ minus grading.  In addition to A, B, C, D, and F, the following are the allowable plus/minus grades with their grade points per hour: A- (3.667), B+ (3.333), B- (2.667), C+ (2.333), C- (1.667), and D+ (1.333).  For the complete description of grades: https://registrar.siu.edu/grades/plusminus.php

Repeated Courses  A course repeated due to an unsatisfactory initial grade can be counted only once in meeting the NCAA satisfactory progress requirements and only after it has been successfully completed.  For students receiving a letter grade of A, B, C, D, or F, the course repetition must occur at SIU Carbondale.  Only the most recent (last) grade will be calculated in the overall GPA and count toward hours earned even if that grade is an F. For example, if a student-athlete earns an F in a course, repeats it and makes a C, calculations of the GPA only reflect the C grade.

WF Grades  A student who stops attending a class without officially dropping will be subject to being awarded a WF grade for the class.  A WF counts as an F in the student’s GPA.  If a student-athlete receives a WF or some form of “Wxx” for all grades, the student will be declared ineligible for the entire semester. Thus, any athletic contests in which the student participated that semester will be forfeited.

Academic Probation  When a student’s cumulative grade point average falls below a 2.00 (C average), the student will be placed on scholastic probation.  While on probation, students may not enroll for more than 14 hours per semester unless approved to do so by the dean of their academic unit.  Other limitations may be established by the academic unit within which the student is enrolled.  A student will remain in the category of probation until the cumulative grade point average is 2.00 or higher.  For more information: http://catalog.siu.edu/grading/

Academic Suspension  Students will be scholastically suspended from the University if they (1) fail to meet the requirements of their conditional or probational status or (2) are enrolled full time their first term of enrollment, and earn a GPA of 0.00.  Students placed on Scholastic Suspension may seek reinstatement after a minimum of two semesters’ interruption (excluding Summer session) but must furnish tangible evidence that additional education can be successfully undertaken.  Continuing students (those suspended at the end of the immediate preceding term) may not change academic units, nor may those readmitted prior to the end of the normally required two semester period of separation from the University.  Some academic units have scholastic requirements in addition to the overall University requirements listed here.  Students must comply with the University requirements as well as those requirements applying to individual schools and colleges.  Appeals must be approved at the Office of the Provost and Vice Chancellor for Academic Affairs. Decisions by the Provost are final.

Plagiarism Plagiarism includes:  Submitting the work of another person or entity as your own.  Failing to cite the work or data of another person or entity.  Improperly citing the work or data of another person or entity.  Submitting work which has been previously submitted for credit or evaluation without instructor approval.  Preparing work with the knowledge or intention that it may be represented as the work of another.  Sharing or disseminating tests, notes, slides, presentations, recordings or any other material which is the intellectual property of an instructor or of the university without prior approval.  Knowingly falsifying scientific or educational data or representing falsified scientific or educational data as the result of scientific or scholarly experimentation or research.  Violating any restriction on collaboration which has been duly communicated by an instructor, via a syllabus, or otherwise in writing or orally in the course of instruction.  Attempting to obtain a grade or other academic credit through improper means, or otherwise subverting the educational process by any means whatsoever.  Soliciting, aiding, abetting, concealing, or attempting any act of academic misconduct.

NCAA ACADEMIC RULES AND REGULATIONS

6 Hours Per Team Rule  Student-athletes must pass a minimum of 6 credit hours per term in order to be eligible for the subsequent semester.  If you have already declared a major, the 6 credit hours must count towards your degree requirements.  Football student-athletes must pass 9 credit hours (and meet all other eligibility requirements) during each fall term in order to be eligible for the following season.

Declaring a Major  By the beginning of your 3rd year (5th semester) in school, you must declare a major leading toward a bachelor’s degree.  If you fail to declare a major leading toward a bachelor’s degree by the first day of class in your 5th semester, you may be ineligible for practice and competition.  Before declaring a major, student-athletes are required to meet with the student services staff in athletics.

Enrollment in 12 Credit Hours  You must be registered for at least 12 credit hours at all times during the semester.  If you drop below 12 hours, you become immediately ineligible for practice and competition.  Before adding or dropping a course, each student-athlete is required to meet with a member of the student services staff in athletics (151 Academic Center).  Failure to obtain approval may result in your athletic ineligibility for the following academic year.

GPA Requirements Student-athletes should remember the following SIU information with regard to their cumulative GPA:

 When a student’s cumulative grade point average falls below a 2.00 (C average), the student will be placed on scholastic probation. While on probation, students may not enroll for more than 14 hours per semester unless approved to do so by the Dean of their academic unit. Other limitations may be established by the academic unit within which the student is enrolled. A student will remain in the category of probation until the cumulative grade point average is 2.00 or higher.  Students will be academically suspended from the University if they fail to meet the requirements of their conditional or probation status, including a failure to achieve a term GPA of 2.0 for two consecutive terms. Students who are suspended will be dismissed from the university for a minimum of two semesters, and will be required to petition their academic unit for re-admission to the university.  To be eligible to receive federal financial aid funds (Pell Grants, Stafford Loans, etc), an undergraduate student must maintain a cumulative grade point average of 2.0 at the end of each semester (among other academic requirements). The cumulative grade point average of all earned hours at SIU is used. Students will have their eligibility to receive federal financial aid funds only after having reached satisfactory progress levels and/or exercising their right to appeal their loss of aid.  Depending on the academic unit, student-athletes with a cumulative grade point average under 2.0 may need departmental and/or University approval to change their majors.

NCAA GPA Requirements To be eligible for competition each term, a student-athlete must maintain a minimum cumulative grade point average according to the NCAA requirements below:

 A student-athlete who is entering his or her second year of collegiate enrollment shall present a cumulative minimum grade-point average (based on a maximum 4.000) equal to 1.80.  A student-athlete who is entering his or her third year of collegiate enrollment shall present a cumulative minimum grade-point average (based on a maximum of 4.000) equal to 1.90.  A student-athlete who is entering his or her fourth or later year of collegiate enrollment shall present a cumulative minimum grade-point average (based on a maximum of 4.000) 2.00.

Summer Courses at Another Institution  Any student-athlete who plans on taking summer courses at another institution should meet with a member of the academic staff prior to enrolling in the course(s).  This ensures that credits earned at an institution other than SIU can be applied to your degree progress at SIU.  Courses taken at another institution do NOT affect your SIUC GPA.

ACADEMIC MONITORING Academic performance is monitored both formally and informally by the Student Services staff.  During the 4th, 8th and 12th week of each semester, progress reports are sent to all your instructors.  Instructors provide information about attendance, participation, grades and general comments.  This information will alert the Student Services Staff if you are at risk of failing the course.  If you are at risk to fail the course, a member of the student services staff will contact you and discuss any academic problems that may exist.  If you are contacted, it is extremely important that you respond in a timely manner to correct the issue.

Study Hall  Study hall is required by Saluki Athletics if your cumulative GPA is below 3.0.  If your GPA is below 3.0, you are required to study a minimum of 4-6 hours per week in the Troutt-Wittmann Academic Center.  All incoming freshmen and transfer student-athletes are also required to study for a minimum of 6 hours per week in the academic center.  Head coaches may require additional study hours of their student-athletes.

Weekly/Bi-Weekly Academic Meetings These meetings are used to discuss academic goals, time management and organizational skills.

 All freshmen and new transfer student-athletes are required to meet with their team’s academic advisor. See p.14 for a list of the academic advisors.  Returning student-athletes who are required to complete study hours and/or have a cumulative GPA of 3.0 or below are required to meet with their team’s academic advisor.  Student-athletes will be notified whether their meetings are on a weekly or bi-weekly basis prior to the beginning of the semester.  All new student-athletes as well as returning student-athletes with a cumulative GPA of 3.0 or below must submit paper copies of their syllabi to the academic support staff by the Friday of the first week of classes.

The Troutt-Wittmann Academic Center provides the academic support and learning environment in which all student- athletes have the opportunity to achieve their academic and personal goals.

Troutt-Wittman Academic and Training Center Rules

Failure to comply with these rules and other policies set forth by the academic staff may result in your dismissal from the Academic Center.

Laptop Computers Laptop computers are available for student-athletes to check out when away from campus for competition.

When checking out a laptop:  Laptops are to complete academic work while traveling for competition  See a staff member in the Academic Center at least 24 hours in advance  Laptops are first-come, first serve  Do not download software or non-academic resources onto laptops  Loss or breakage of the laptop is the responsibility of the student-athlete  When you return, please return laptop to a member of the student services staff (Academic Center) within 24 hours

Student Resources at the Troutt-Wittman and Morris Library  Twenty computers loaded with educational software including Microsoft Word, Powerpoint, Excel, Access and Publisher are available  Student-athletes can also research through Morris Library or search the internet.  An SIU network ID (SIU+Dawgtag) and password are required to log-on to these computers and other computers located on campus.

Troutt-Wittman Sign-In and Out Procedures In order to ensure that your coaches have the correct information on on study hall attendance, please do the following:

 Present your SIU student ID to the study table monitor at the check-in desk.  The monitor will swipe your student ID, and you will be entered in the system.  Your student ID is left with the study table monitor until you leave the academic center.  When you leave, the study table monitor will swipe your student ID and check you out of the system.  You must have your student ID card swiped to sign-out.  If you do not properly sign-in and out, study hours will not be properly calculated.  Do not swipe your own ID.  If a monitor is not at the check-in desk, please wait for a staff member to arrive.

TUTORING

This program is to help student-athletes keep pace with rigorous classes while trying to maintain busy and challenging academic and athletic schedules.  Tutors are available by appointment at the Troutt-Wittmann Academic Center for a variety of courses. Academic Staff members can set up tutoring appointments through SSC Campus  All tutors must be hired by SIU as student workers.

Tutoring Reminders  Student-athletes must come to tutoring sessions prepared.  Student-athletes cannot hire tutors.  Tutoring sessions do not replace meeting with instructors or attending class.

NCAA Rules Related to Tutoring Any inadvertent or intentional violation of the following rules may result in you jeopardizing your athletic eligibility. Note: This is not an exhaustive list.  Tutors cannot write or type papers & student-athletes will not ask them to do so.  Tutors cannot complete homework or take exams.  Tutors are not allowed to lend money in any amount to student-athletes.  Student-athletes may not ask tutors for transportation.  Student-athletes and tutors will maintain a professional relationship.  The tutor cannot tutor someone they are dating or have ever dated.  The tutor cannot tutor roommates or suitemates.  Tutors may not buy a gift, food or beverage for a student-athlete.  Tutors may not co-sign a loan for a student-athlete.  Tutors may not provide housing arrangements for the student-athlete.  Tutors may not permit a student-athlete to use institutional resources, such as a copier or fax machine.  Tutors may not make credit cards available for student-athletes to use. The university requires all students attending SIU to claim an SIU email address and check it on a regular (daily) basis.  After receiving an SIU email address, you can check your email on the internet through office.siu.edu  Office 365 provides a full featured web browser interface to check your SIU E-mail account.  You can access your SIU email by signing in using either your SIU email address or siu85xxxxxxx and NetID password.

SalukiNet

 SalukiNet is a website which offers a variety of SIU student services, other University information, and more.  When you access Salukinet, you can find student records about yourself that are maintained by the offices of Records & Registration, Undergraduate Admissions, Bursar, and Financial Aid.  To access your student records, go to http://salukinet.siu.edu and log into and use SalukiNet.

If you need to print a copy of your current class schedule, you can access SalukiNet to do so. This information can be found under the “Student” tab, then “Registration” tab, then the “Concise Student Schedule” tab, choose the correct date range, then print this page.

Desire to Learn (My Courses)

 Desire to Learn mycourses.siu.edu is a Web-based course-management system designed to allow students and faculty to participate in classes delivered online or use online materials and activities to complement face-to-face teaching.  D2L enables instructors to provide students with course materials, discussion boards, virtual chat, online quizzes, an academic resource center, and more.  The degree to which D2L is used in a course varies. For example, instructors may supplement an on-campus class by putting their syllabus and handouts on their course sites. In contrast, other courses may be conducted entirely through Desire to Learn, without any on-campus sessions.  If you’re enrolled in a course that uses D2L, be sure to find out how D2L will be used in that course.  DESIRE TO LEARN ACCOUNTS SHOULD BE CHECKED ON A DAILY BASIS.

SIU Alerts

 SIU Carbondale employs the Rave Mobile Safety alert system to send emergency text messages to cell phones and to campus e-mail addresses of students, faculty and staff.  The message may alert you to an emergency situation, including if the university is closed; ask you to take action, or both.  The system provides some weather warnings, including severe thunderstorm, tornado, flash flood and winter storm.  Since the messages are short, you may need to go to another source of university emergency information for more details.  To receive emergency text messages, students must have current contact information in SalukiNet.  Click on the “My Records” tab, then click on “View My SIUC Personal Records.” Students also will receive alerts via e-mail as long as you have claimed your account and created an siu.edu e-mail address.

SUMMER ATHLETICS AID POLICY

Summer Athletics Aid

Summer athletics aid is awarded to student-athletes above and beyond the regular academic year Grant-In-Aid Agreement, and is available on a limited basis. It is meant to promote progress toward graduation, and is not meant to be used to regain eligibility. Summer athletics aid is NOT available for coursework leading to a second major, additional minor, graduate degree, or any other coursework that is additional to the hours required to obtain a single undergraduate degree. Summer athletics aid is not an automatic award. Enrolled/continuing student-athletes must earn eligibility for summer athletics aid by demonstrating commitment to Saluki Athletics and making satisfactory progress toward a degree throughout their career at Southern Illinois. To be considered for summer aid, student-athletes must apply and meet all of the following criteria:

1. Must have demonstrated consistent progress toward a degree; 2. Must have made consistent contributions to the success of the team; and 3. Must have received athletics aid during the previous academic year, or will be receiving athletics aid in the following academic year.

Guidelines of Summer Athletics Aid Awards

 Summer athletics aid is contingent upon budget availability, which varies annually. It is never guaranteed. Summer athletics aid is limited to one class. Very limited consideration for additional hours related to graduation purposes MAY be available.  The amount of summer athletics aid awarded is within the discretion of the Head Coach and SIU Athletics Administration. As a result, a student-athlete is not guaranteed to receive summer athletics aid in the same proportion of the amount of athletics aid received in the previous academic year.  A student-athlete who has not received athletics aid from SIU during a previous academic year may receive athletics aid to attend SIU’s summer session(s) as long as he/she has been awarded athletics aid for the following academic year. The student-athlete is not guaranteed to receive summer athletics aid in the same proportion of the amount he/she will receive the following academic year.  A summer athletics aid award MAY consist of tuition, fees, books and room & board. All summer athletics aid awards vary based on the student-athlete’s summer enrollment.  Student-athletes awarded summer athletics aid will receive such aid only for session(s) in which they are enrolled. For example, a student-athlete enrolled only in Summer Session II and receiving a full scholarship (tuition, fees, books and room &board) will receive room & board for Summer Session II; the student-athlete will not receive room & board for the entire summer (all three Summer Sessions).  Student-athletes receiving summer athletics aid must continue to meet regularly with academic staff and complete all study hall requirements during throughout their summer enrollment.  Student-athletes enrolled solely in on-line course(s) must fulfill a weekly study hall requirement of one hour per unit of enrollment (3 unit course = 3 hours of study hall per week).  Any changes to enrollment after the approval of summer aid must be approved by the Athletic Director or designee through submission of a supplemental Summer Athletics Aid Application.

 Tuition and Fees  Tuition and fees are charged at a variable rates based on units of enrollment and date of enrollment.  Student‐athletes earning a grade (or grades) of F, or no credit in any academic course(s) must meet with the Athletic Director and/or a designee to discuss the circumstances resulting in the failed grade(s) at the earliest opportunity after final grades have posted and prior to the start of the next academic term. Information gathered at the meeting will be used to determine whether the student‐athlete will be charged for the cost of the failed course, tuition and fees.

 Room & Board  Room & board will only be provided for the session(s) in which the student-athlete is enrolled and is subject to budget availability. Room & board in NOT guaranteed.

Procedure for Applying for Summer Athletics Aid

1. Each student-athlete interested in receiving summer athletics aid must complete the Summer Athletics Aid Application. 2. The student-athlete must complete and submit the application to his/her Athletic Academic Advisor for verification that the coursework requested is degree applicable. 3. The Athletic Academic Advisor submits the completed Application to the Director of Compliance or a designee who calculates the value of the requested award and provides the Application to the Head Coach for approval and signature. 4. The Head Coach makes approval recommendations to the Athletic Director or a designee.

Procedure for Approval and Notification of Summer Athletics Aid

1. The Assistant AD for Academics, Executive Senior Associate AD for Internal Operations and Director of Compliance review and approve summer athletics aid with consideration given to gender equity, equitable distribution of aid, APR ramifications and budget constraints. The group presents summer athletics aid recommendations to the AD for approval. 2. The Associate Athletics Director will notify the appropriate Head Coaches of the approval or denial of summer athletic aid awards. 3. The Director of Compliance or a designee will provide Head Coaches with each student-athlete’s individual summer aid award breakdown. 4. It is the responsibility of the Head Coach to notify their student-athlete(s) of the approval or denial of their summer athletics aid application and the approved award amount(s). 5. Student-athletes approved for summer athletics aid will sign a Summer Grant-In-Aid Agreement. 6. Athletic Academic Coordinators work with the student-athletes who are approved for summer athletics aid to ensure appropriate class registration.

Fifth-Year Aid

Fifth-year aid is awarded to student-athletes who have exhausted their eligibility, have not completed their undergraduate degree and still have a semester (or year) remaining on their five-year clock. Fifth-year aid is NOT available for coursework leading to a second major, additional minor, graduate degree, or any other coursework that is additional to the hours required to obtain a single undergraduate degree. Fifth-year aid is not an automatic award. To be considered for fifth-year aid, student-athletes must apply and meet all of the following criteria:

1. Must have demonstrated consistent progress toward a degree; 2. Must have made consistent contributions to the success of the team; and 3. Must have received athletics aid during the previous academic year.

Requirements of Fifth-Year Aid Award

 An award of fifth-year aid only covers tuition and will never be more than (and could be less than) the previous academic year’s athletics aid award percentage.  An award of fifth-year aid is only available for undergraduate degree credit. It will only cover courses required for graduation in the student-athlete’s 1st degree program (tuition only). It is not available for the purpose of obtaining a 2nd degree, additional minor, graduate degree, or any other coursework outside the student-athlete’s 1st degree program. Room and board costs and textbooks will not be covered in a fifth-year grant-in- aid.  All student-athletes receiving fifth-year aid are required to work 16 hours per week for the Department of Athletics, and all home football games during the fall term. Event work assignments for the spring term will be determined in January.  Awarding of fifth-year aid is contingent upon budget availability, which varies annually; therefore, fifth-year aid is awarded on a semester-by-semester basis and is not guaranteed.

Procedure for Applying for Fifth-year Aid

5. Each student-athlete eligible for fifth-year aid consideration must complete the Fifth-Year Aid Application Form, which is distributed (along with the 5th Year Aid Policy) to Head Coaches, Academic Coordinators and eligible student-athletes by the Executive Senior Associate AD for Internal Operations in March. 6. The Form is then submitted to the appropriate Athletic Academic Coordinator verification of the number of hours required for degree completion (1st degree only). 7. The student-athlete attaches a copy of his/her graduation plan to the application, and submits it to his/her Head Coach. 8. The Head Coach reviews the Fifth-Year Aid Application Form, signs and submits the completed Form to the Executive Senior Associate AD for Internal Operations by April 15.

Procedure for Approval and Notification of Fifth-Year Aid

7. The Assistant AD for Academics, Executive Senior Associate AD for Internal Operations and Director of Compliance review and approve fifth-year aid with consideration given to gender equity, equitable distribution of aid, APR ramifications and budget constraints. The group presents fifth-year aid recommendations to the AD for approval. 8. The Executive Senior Associate AD for Internal Operations will notify all applicants, the Head Coach and Academic Coordinator via email of the approval or denial of their fifth-year aid award by April 30. 9. The Executive Senior Associate AD for Internal Operations and Director of Compliance meet with all fifth-year aid recipients prior to the first day of classes to discuss the Code of Conduct, to schedule work assignments, and to sign Grant-In-Aid Agreements.

TRAVEL POLICIES

University Athletic Travel Policy All student-athletes are required to travel as a team to and from the site of competition. Only under exceptional circum- stances will a student-athlete be allowed to travel separately from the team. Permission to travel separately from the team must be granted by the coach of such student-athlete and, if the student-athlete is a minor, and the parent/guardian of the student-athlete. All such permission and approval must be obtained in writing prior to any travel to an athletic competition or event. The student-athlete should contact her/his coach well in advance of the contest in order to complete the necessary form and obtain signatures.

SIU Athletic Team Travel Policy

Hotel Rooms No more than two to a room; one per bed. If a rollaway is needed, coaches should make arrangements in advance.

Transportation Teams with over eight individuals traveling must use a bus or minibus. Van travel is not permitted between midnight and 6 a.m. Van travel is not permitted for trips that exceed 500 miles one way, unless multiple certified drivers are available per vehicle (not including student-athletes). Coaches can drive no more than eight hours at one stretch. When the distance is greater than 500 miles one way, commercial airline transportation should be used unless there are extenuating circumstances or alternative arrangements are approved in advance.

Airline travel may also be used for conference-mandated travel during the week in order to minimize missed class time.

Class Absences Due to Travel for Competition  When practice schedules conflict with class schedules, student-athletes are required to attend class.  Student-athletes who must schedule classes during practice times shall have their class schedules approved in advance by a member of the athletics academic services staff.  Under the University’s Undergraduate Student Absence Policy for University-Approved Activities, instructors will provide reasonable accommodation to students who miss exams, quizzes and the like due to participation in University-approved activities such as travel for athletics competition.  It is the student’s responsibility to use the Student Absence Form provided by Athletic Student Services to officially inform instructors of all scheduled absences as soon as possible in the term.  It is also the students’ responsibility to arrange for the completion of all missed classroom work.  Ultimately, students are responsible for the material covered in class.  Student-athletes who will be traveling to away competitions should be notified of their travel schedules at the beginning of each semester and should notify their instructors accordingly.  Coaches whose travel rosters vary should inform student-athletes of who will travel at least one week prior to the event, if possible.  Student-athletes are expected to make up missed assignments prior to their departures for athletic events, subject to the discretion of their instructors.

Student-Athlete Advisory Committee (SAAC)  SAAC is a registered student organization on campus.  The committee serves to provide you and your teammates with a substantial role in the overall policy-formation process regarding relevant and current issues in SIU athletics as well as the Missouri Valley Conference.  One of the responsibilities of SAAC is to advise the department on addressing issues of importance to student- athletes.  If you have concerns that you feel should be shared with the department’s administration, your voice can be heard through this board.  SAAC, which meets once a month, includes representatives from all 15 teams and is chaired by an executive committee chosen each year.  Please contact a member of the staff in the Troutt-Wittmann Academic Center if you are interested in representing your team or have an issue you would like the committee to discuss.  For more information about SAAC at the national level, visit: http://www.ncaa.org/student-athletes/ncaa-student- athlete-advisory-committees-saacs

Letter Awards A letter award winner is an individual who has contributed to the success of the team through exceptional performance, dedication and effort. • Junior college and transfer student-athletes with two years of eligibility who qualify for a letter will receive a letter jacket their first year and a watch their last year. Junior college and transfer student-athletes with 3 years of eligibility at SIU who qualify for a letter will receive a letter jacket their first year, watch their second year, and blanket their third year.

Dr. Don Boydston Legacy Athletic Scholarship Endowment • The Dr. Don Boydston Legacy Athletic Scholarship Endowment was established on August 12, 2009, by his wife, Dr. Jo Ann Boydston, to honor one of the most significant figures in Saluki Athletics history. The endowment funds athletic scholarships designed to recognize SIU student-athletes who demonstrate the ideals for which “Doc” Boydston stood. Throughout his storied tenure as an educator and athletics administrator at Southern Illinois University Carbondale, Donald N. "Doc" Boydston demonstrated a commitment to inclusion and diversity, opportunity and success -- in athletics and in the classroom.

• Boydston's 15 years as athletics director from 1957 to 1972 produced one of the nation's most well-rounded intercollegiate athletics programs. Between 1960 and 1972, SIUC teams won 12 national championships in gymnastics, cross-country, swimming, golf and tennis. In dual meets, matches and games, SIU from 1957 through 1972 had a .715 winning percentage and moved from cellar-dweller status in a small college conference to university status and national recognition for a balanced program in all sports. During the “Boydston Era,” the SIU Arena was built, the baseball facility developed, the track all-weather surfaced, and money was set aside for the renovation of McAndrew Stadium. 36 Southern Illinois University

• Four Boydston Fellows are named annually. Preference is given to deserving racial minority student-athletes who have consistently demonstrated personal characteristics of integrity, commitment to equality, academic excellence, and social justice. The Boydston Fellows must have sophomore status or higher, be making satisfactory progress toward their degree and may represent any varsity sport.

Conference Academic Awards

 The Missouri Valley Conference and the Missouri Valley Football Conference recognize student-athletes for academic achievements through three different awards.  The President’s Council Academic Excellence Award is presented to student-athletes who have a minimum 3.80 cumulative GPA, have participated in a Valley sport for at least two years and are within 18 hours of graduation.  The Commissioner’s Academic Excellence Award requires student-athletes to earn at least a 3.50 GPA in an academic year.  The MVC Conference Honor Roll consists of student-athletes who have earned a 3.20 (3.00 for MVFC) GPA in at least 12 credit hours during a selected term.

The MVC also recognizes student-athletes through the Elite 17 Award. The Elite 17 award is presented to the student- athlete who is among the top finishers with the highest GPA at the MVC conference championships.

University Honors SIU presents several academic awards, many of which include a cash award, to deserving students each spring during the Honors Day ceremony. All academic cash awards received by a student-athlete are subject to NCAA regulations and must be approved by the athletics department before the student-athlete can receive the cash award.

McNair Scholars Program  The SIUC McNair Scholars Program provides enriching educational experiences for low-income, first-generation college students and members of underserved groups in higher education.  Through a strong mentoring network, the program supports and sponsors educational and professional development activities that lay the foundation for stellar research and academic skills.  We promote academic excellence, success in graduate school, and achievement of a PhD.  If you are interested in more information about the program, go to https://mcnair.siu.edu/, or contact Rhetta Seymour at the McNair Scholars Program directly ([email protected]; 453-4582).

NCAA and MVC Post Graduate Scholarships  Postgraduate scholarships are available to student-athletes who have completed their eligibility, and plan on continuing their education at a postsecondary institution within a specific time frame.  Academic excellence as well as athletic contributions are major factors for these postgraduate scholarships.  MVC Postgraduate Scholarships are given out every Spring and are worth $5,000, while NCAA Postgraduate Scholarships are awarded for each competitive season (Fall, Winter, Spring), and are worth $7,500.  Contact Dr. Partridge ([email protected]; 453-3119) if you are interested in applying for these scholarships.

COMPLIANCE

Student-athlete 2020-21 academic year Opt-Out due to Covid-19 policy

Student-athletes choosing not to participate in intercollegiate athletics during the 2020-2021 academic year due to concerns related to COVID-19 must inform their Head Coach, the Sport Administrator and the Director of Compliance. The student-athlete:

1. Must remain enrolled in classes for both the fall and spring semesters and adhere to all academic standards and requirements. 2. Will remain bound by the Student-Athlete Handbook and the SIUC Student Conduct Code. 3. Must meet all satisfactory academic progress requirements as set by the NCAA and SIUC. 4. Will understand their 2020-21 athletic financial aid will not be impacted by the decision to opt out. a. If athletic financial aid includes room and board, understand that they will not receive those amounts if I choose to remain at home for the academic year per NCAA financial aid guidelines. 5. Will continue to have access to academic services through the Athletic department. 6. Will continue to have access to sports medicine services for rehabilitation and treatment for only those injuries sustained prior to opting out. Note: student-athletes who are currently competing for their team will have priority access in the Athletic Training Room. 7. Will not be permitted to participate in any in-person team activities, participate in voluntary athletics related activities, or access athletics facilities (i.e. weight room, nutrition center, gym, field, track, locker room, etc.). a. I will be permitted to participate in virtual team activities, if conducted. 8. I will not receive the benefits associated with participation, including but not limited to team travel, per diem, equipment, apparel, entertainment and meals. 9. I am still bound by all NCAA rules and regulations, including, but not limited to extra benefits, drug testing, amateurism, recruiting and sports wagering. a. Opting out does not permit me to speak to other institutions about transferring and participating in athletics. 10. The 2020-2021 academic year will count towards my 5-year NCAA eligibility clock. a. I understand there is no guarantee in receiving athletics aid in my 5th year.

If circumstances change and a student-athlete decides to opt back in, s/he will need pre-approval from the Head Coach, Sport Administrator and Athletic Trainer. The student-athlete will be required to comply with University and Athletics health and safety protocols prior to your return to team activities.

Amateurism A student-athlete (SA) must be an amateur in order to remain eligible to compete in intercollegiate sport. A SA loses amateur status and thus shall not be eligible for intercollegiate competition in a particular sport if the SA:  Uses athletics skill (directly or indirectly) for pay in any form in that sport.  Accepts a promise of pay even if such pay is to be received following completion of intercollegiate athletics participation.  Signs a contract or commitment of any kind to play professional athletics, regardless of its legal enforceability or any consideration received.  Receives, directly or indirectly, a salary, reimbursement of expenses or any other form of financial assistance from a professional sports organization based upon athletics skill or participation, except as permitted by NCAA rules and regulations.  Competes on any professional athletics team and knows (or had reason to know) that the team is a professional athletics team, even if no pay or remuneration for expenses was received.  Enters into a professional draft or an agreement with an agent.

Agents NCAA rules do not prohibit discussions or meetings with agents. On the other hand, there are strict rules that, if violated, will jeopardize your chances for intercollegiate eligibility. Please be careful, if an individual contacts you and wants to market your athletic ability, make sure you research that potential agent before making a written or oral agreement.

A student-athlete will lose their amateur status and will not be eligible for intercollegiate competition in a particular sport if:

 The student-athlete signs a contract or commitment of any kind to play professional athletics, regardless of its legal enforceability.  The student-athlete competes with a professional sports team or competes as a professional in an individual sport and receives any compensation for participation.  The student-athlete or family member negotiates, signs, or enters into any written or oral agreement with an agent.  The student-athlete or family member accepts or receives any extra benefits from an agent or anyone who wishes to represent the student-athlete.  The student-athlete uses his or her athletic skills for pay or the promise of pay.

To minimize the possibility of such eligibility problems, Southern Illinois University asks that:  All communication made by sports agents with a current student-athlete or their family be arranged through the Saluki Athletic Compliance Office  Sports agents are required to complete an Agent Registration form and provide copies of professional league players' association applications for those associations in which they belong to the Saluki Athletic Compliance Office

Extra Benefits Student-athletes who accept benefits or expenses not authorized by NCAA rules from coaches, boosters or institutional staff members must be declared ineligible immediately for competition.

Benefits defined: An extra benefit is any special arrangement by an institutional employee or a representative of the institution’s athletic interests (including fans) to provide a student-athlete or the student-athlete’s relative or friend a benefit not expressly authorized by the NCAA legislation. Extra benefit regulations pertain to prospects as well.

Benefits are not permissible for student-athletes. Examples of non-permissible extra benefits include, but are not limited to the following:

 Transportation or use of a vehicle  Use of a telephone or credit card for personal reasons  Gifts and loans of money or co-signing a loan 39 Southern Illinois University

Occasional Meals A student-athlete or an entire team in a sport may receive an occasional meal in the locale of the institution on infrequent and special occasions from an institutional staff member. An institutional staff member may provide reasonable local transportation to student-athletes to attend such meals. However, any type of occasional meal must be pre-approved by the Saluki Athletic Compliance Office. We have documentation procedures, which must be strictly adhered to prior to the approval of an occasional meal.

A student-athlete may receive an occasional family home meal from a representative of athletics interests (booster) on infrequent and special occasions under the following conditions:  The meal must be provided in an individual’s home (as opposed to a restaurant) or on the SIU campus and may be catered.  A representative of the institution’s athletics interests may provide reasonable local transportation to student-athletes to attend the meal function only if the meal function is at the home of that representative.  Student-athletes must receive prior approval before receiving a meal from a booster.

Gambling The NCAA has established strict guidelines and sanctions concerning involvement in gambling for a student-athlete. NCAA Bylaw 10.3 states that student-athletes and staff members of the athletic department shall not knowingly: 1. Provide information to individuals involved in organized gambling activities concerning intercollegiate athletics competition; 2. Solicit a bet on any intercollegiate or professional team; 3. Accept a bet on any team representing the institution; 4. Solicit or accept a bet on any intercollegiate competition for any item that has tangible value. This is not limited to cash bets. It is also illegal to bet for sneakers, meals, clothes or any other item. 5. Participate in any gambling activity that involves intercollegiate athletics or professional athletics, through a bookmaker, a parlay card, or any other method employed by organized gambling.

NCAA Penalties For Involvement in Gambling: 1. Student-athlete’s involvement in activities designed to influence outcomes or win/loss margins shall result in permanent loss of all eligibility. 2. Student-athlete’s involvement in accepting or soliciting bets on their own institution shall result in permanent loss of all eligibility. 3. Student-athlete’s involvement in any other type of sport wagering through organized gambling shall result in at least one year loss of all eligibility.

Outside Competition NCAA regulations govern all athletic activities that occur outside of a sport's regular playing season.  Unapproved participation or receipt of extra benefits may adversely affect a student-athlete's eligibility.  Written approval is required prior to any outside competition or summer activity.  All student-athletes should check with the Compliance Office prior to participating in non-collegiate, amateur competition during the summer period.  Contact the Compliance Office for regulations pertaining to your sport.

Playing and Practice Seasons Time Limits for Athletically Related Activities . Daily and Weekly Hour Limitations – Playing Season A student-athlete’s participation in countable athletically related activities is limited to a maximum of four hours per day and 20 hours per week. You must be given one day off per week. Note: A day of competition counts as three hours, regardless of how long the competition lasts.

. Weekly Hour Limitations – Outside Playing A student-athlete’s participation in the off-season will be limited to a maximum of eight hours per week of countable athletically related activities.

. Vacation Periods and Between Terms Daily and weekly limitations do not apply to countable athletically related activities occurring during SIU official vacation periods (as listed in the SIU official calendar), and during the academic year between terms when classes are not in session.

Time Management Plan  Requires the creation of a time management plan for each sport with an annual review.  Must start the first day of the term or first day of Countable Athletically Related Activities (CARA), whichever comes first.  A minimum of one-week notice given to SA’s about any mandatory team activity.  A minimum of 24 hours must be given to SA’s with any schedule changes.  The coaching staff, a senior athletics department staff member, and at least one student-athlete will create time management plans.  Each plan will have an annual review with the Athletic Director, Faculty Athletics Representative, Head Coach, and SA from the team. 41 Southern Illinois University

9 p.m. to 6 a.m.  Prohibits countable athletically related activities during a continuous eight-hour period between 9 p.m. and 6 a.m.  There is a continuous nine-hour period between these hours where CARA is not permitted. But Institution may schedule travel within these nine hours.  This eight-hour period may be determined on an individual basis.  There are exceptions to this rule when it comes to academics, athletic training, drug testing, mental health evaluations, and SAAC meetings.

Seven-Day Break • A mandatory seven-day break after the season is required. • Some activities can be included with the seven-day break life skills activities, award shows, individual coaches and player meetings, and NCAA speakers. • Vacation days may be used to fulfill this requirement.

Fourteen Discretionary Days  Fourteen additional days off during the off-season and the regular academic year when classes are in session is required.  These days off will be used at the discretion of the SA.  These days don’t need to be the same per SA or consecutive days.  No recruiting activities (meals, tours, and hosting) can take place during this period.

Other  Require a day off per week during preseason and vacation periods.  Prohibit off-campus practice during vacation periods outside the championship season and unrelated to away- from-home competition

Complementary Admissions • All SIU students receive free admission to all regular season (excluding post-season tournaments) men’s and women’s athletic events. • Student-athletes must present their student identification cards to gain admission to events. • Your support of the efforts of your fellow student-athletes is equally important as their support of your efforts. • NCAA rules allow SIU to provide a maximum of four complimentary admissions per home or away contest to a student-athlete in the sport in which the individual participates. • Complimentary admissions shall be provided only through a pass list for those individuals designated by the student-athlete. • Tickets are non-transferable. • Player’s guests must come to the designated entrance and show proper identification. • Student-athletes may not receive payment for complimentary admissions, nor exchange them for any item of value. • Any student-athlete found to have sold or exchanged complimentary tickets for cash or any item of value, automatically loses his/her eligibility. • Complimentary admissions may be provided to members of SIU’s team for all contests in a tournament in which the team is participating, provided that all contests are held at the site at which SIU’s team is participating.

Hosting Prospective Student-Athlete Hosting a prospective student-athlete is a privilege, which carries certain responsibilities. These responsibilities include being aware of the prospect’s itinerary and the required campus meetings the prospect is obligated to attend. Additionally, a student host must understand and abide by all applicable NCAA, Conference and SIU regulations.

Keep in mind the following rules:  Do not provide alcohol to a prospect.  Do not provide or encourage the use of drugs.  Do not engage in any behavior that could be considered a violation of criminal law.  Do not participate in any form of gambling activities.  Do not visit adult entertainment establishments, or any equivalent.  Do not purchase any souvenirs such as a t-shirt or hat with host/entertainment money.  For additional information, please see your head coach or the Saluki Athletic Compliance Office.

FINANCIAL AID POLICIES & PROCEDURES

What does an athletic scholarship cover? Athletic scholarships include the values of which are varying combinations of tuition, fees, books, room and board.

Who gets an athletic scholarship? Athletic scholarships are available to student-athletes who demonstrate outstanding athletic ability as well as the ability to academically succeed in the University classroom. A student-athlete at SIU who has been awarded an athletic scholarship is responsible for putting forth full effort in each course in which s/he is enrolled. This includes attending class, completing assignments, meeting with an athletic academic advisor, completing study hall requirements, requesting tutors, and making use of any other resources available to help complete each course to the best of that person’s abilities.

When will I be informed about my scholarship status? Student-athletes who receive athletic aid will be informed no later than July 1 of each year of the exact amount of aid they will be offered for the following year.

Can I lose my athletic scholarship? Continued receipt of an athletics scholarship is at the University’s discretion and is conditioned upon the student-athlete putting forth full effort in practice, in competition and in the classroom. Anything less than a student-athlete’s best effort may result in the reduction or non-renewal of their athletic scholarship for the subsequent year.

What happens if I am injured? If you become injured, your obligation is to pursue treatment and recovery with the same full effort. Your scholarship may not be reduced or cancelled due to injury.

Are there any circumstances where I would have to pay back scholarship money? In the event a student-athlete on athletic aid receives a D, F, or INC in any class for which they did not put forth the appropriate effort as listed above, the student-athlete may be required by the athletics department to repay the cost of that course (up to tuition, fees, room and board and books). Also, if a student-athlete on athletic aid drops a class after the University’s official add/drop period, and incurs a financial penalty for dropping the course, that student-athlete may be responsible for paying that penalty, depending on the circumstances under which the course was dropped.

What if my aid is reduced or not renewed? If your athletics aid is reduced or not renewed for the following year, you have a right to an opportunity to appeal the decision. See Grievances and Appeals section.

How long does my scholarship last? These scholarships are granted for a period of one academic year and may be upgraded, downgraded, continued, or discontinued in accordance with NCAA regulations after each year of participation.

If you receive room and board as part of your athletic scholarship: You must report the room and board portion of the scholarship as income for federal and state income tax.

If you receive books as part of your athletic scholarship: You must return ALL books to the SIU Bookstore before the deadline at the end of each semester. Any non-returned books will be charged to the individual student-athlete. Please see the Compliance Office with any questions or concerns regarding book returns.

Am I able to retain other sources of financial aid (non-athletic)? Yes, but do keep in mind that this aid is subject to the language of your athletics financial aid agreement.

In addition, you and the athletic financial aid office would need to determine if the provision of the outside aid or other institutional aid would adversely affect your scholarship equivalency and/or the team NCAA scholarship limit. In the event that it would affect your team limit or individual limit, you would remain at the scholarship equivalency originally agreed to in your athletic financial aid agreement.

Summer School Aid for Degree Progress  The purpose of summer financial aid is to assist student-athletes in making progress towards their degree (and not to pursue an additional minor, second degree, or master’s degree).  It is awarded at the discretion of the coach and the athletic director (or designee).  Factors that may be considered in deciding to award the summer financial aid include: o The availability of funds o Fulfillment of academic and team obligations o Aid will be provided first to those who need course credit to remain eligible to play  Summer financial aid is not an entitlement or part of a student-athlete’s academic year financial aid agreement.  Any student-athlete wishing to receive athletically-related financial aid for summer school should meet with their head coach.  Student-athletes are required to submit a summer school application, available from the Saluki Athletics Compliance Office.  See page 28 of this document for the summer athletics aid policy.

The following NCAA and SIU rules apply to summer aid: 1. Summer financial aid may be awarded only to attend SIU’s summer term. 2. A returning/continuing student-athlete must have been enrolled at SIU for a minimum of one semester during the student-athlete’s previous academic year. 3. The student-athlete may not receive athletically related financial aid to attend SIU’s summer session unless the student-athlete received such athletically related aid from SIU during the student-athlete’s previous semester. (Exception: a walk-on who has never previously received any athletically-related financial aid from SIU may receive summer aid if he or she will be receiving athletically-related financial aid during the upcoming academic year). 4. Summer aid may be awarded only in proportion to the amount of the athletically related aid received during the student-athlete’s previous academic year (e.g., someone who received 50 percent in athletic aid is eligible for a maximum of 50 percent of summer athletic aid).

5. Changes to a student-athlete’s summer schedule must take place prior to the end of the 2nd week of classes, or the student-athlete will be responsible for any charges associated with the late add and/or drop. All changes to a student- athlete’s summer schedule must also be approved by the Associate Athletic Director for Student Services. 6. Additional costs will not be covered by a student-athlete’s summer aid. For example, summer aid will not cover certain portions of Individualized Learning Program (ILP) courses. The goal of summer school is for the classes to be taken in the classroom.

7. Student-athletes who do not put forth the appropriate effort (class attendance, study time, etc) for their summer school classes will repay their summer financial aid (including tuition, fees, room & board, books) for any class in which they receive a D, F, or INC.

Fifth Year Aid Financial assistance is available for fifth-year student-athletes who have completed their eligibility.  The purpose of fifth-year financial aid is to assist student-athletes in finishing their degree and graduating.  Student-athletes requesting such aid must complete and submit a Fifth Year Aid Application along with a graduation plan obtained from their academic advisor on campus.  Applications are available from the Saluki Athletics compliance office.

 Provision of fifth-year aid is at the discretion of each coach, subject to availability of funds and final approval of the student-athlete’s application request by the Director of Athletics or the Associate Athletics Director.  See page 29 of this document for the full 5th year aid policy.

Fifth Year Aid Work Program: As a condition of receiving fifth-year financial aid, you will be expected to work within the Athletic Department in some capacity during the terms in which you receive your award, based on your class schedule and as determined by Athletic Department staff.

Additional Financial Assistance The Financial Aid Office processes many sources of financial aid in the form of grants, scholarships, student work, and loans. https://fao.siu.edu/  The basis for this aid is usually academic excellence and/or financial need.  Student-athletes must complete a Free Application for Federal Student Aid Form (FAFSA) each year in order to be eligible for on-campus employment and to determine eligibility for other types of aid.  All financial aid awards received by a student-athlete are subject to NCAA regulations and must be approved by the athletic department before the student-athlete can receive the award.  Please see the Saluki Athletics Compliance Office for more information.

The following are a few of the forms of financial aid available to student-athletes.

Pell Grant The Pell Grant is a federal aid program designed to provide assistance to undergraduate students who demonstrate financial need. The amount of the grant is based on the expected family contribution (EFC), the level of funding, and the cost of education. Students enrolled less than full-time will receive a reduced grant award. Eligibility must be established each year by filing the current FAFSA at www.fafsa.ed.gov.

Federal Work Study (FWS) Students awarded and accepting FWS should review the Job Listing Board and obtain a Student Employment Referral at the Financial Aid Office (FAO) at SIUC. https://studentjobs.siu.edu/_common/documents/fws-quick-facts.pdf

Illinois Monetary Award Program (MAP) Grant The State of Illinois Monetary Award Program (MAP) Grant offered by the Illinois Student Assistance Commission (ISAC) assists undergraduate students in need who are enrolled at least half-time. Appeals Process Student-athletes, coaches and administrative staff are encouraged to keep lines of communication open in order to aid in the informal resolution of problems. There may, on occasion, be situations where a student-athlete believes that an action taken by, or a decision made by, a member of the athletics department coaching or administrative staff is inappropriate or unreasonable. If informal discussion with the coach or staff member is insufficient to resolve the problem, the student-athlete may need to speak with an athletic administrator to determine how best to approach the situation.

Financial Aid Appeal If you receive an athletics scholarship and your athletics aid is reduced or not renewed for the following year, you have the right to request a hearing. If your aid is cancelled during the year, you have a right to appeal the cancellation. You may request a hearing through the University Financial Aid Advisory Committee by contacting the Director of Financial Aid ([email protected]; 618-453-4334) to request a copy of the specific procedures. The decision of the appeals committee is final and is based on a determination of whether the decision to recommend nonrenewal, reduction or cancellation was clearly unreasonable.

Student-Athlete Transfer Policy

Notification of Transfer Any student-athlete that wishes to transfer from Southern Illinois University (“University”) to another 4- year NCAA institution must first obtain authorization through the notification of transfer process from the Compliance Office prior to speaking with athletics representatives from another institution. A student-athlete must submit the request through the JumpForward Software in order in order for the Compliance Office to enter the student-athlete’s name into the Notification of Transfer system within two (2) business days. Requests completed outside of JumpForward will not be considered written notification for purposes of NCAA Bylaw 13.1.1.3.1. After written notification has been received, a member of the Compliance Office will send an email notifying the student-athlete they have been added to the NCAA Transfer Portal. This email will specify what services and facilities are still available to the student-athlete. It is suggested that all student-athletes wishing to transfer discuss this with their coaching staff first. At this time, the student-athlete will also be notified whether his/her athletics aid will be reduced/canceled at the end of the regular academic term in which written Notification of Transfer is received. If a student-athlete provides written Notification of Transfer between regular academic terms (winter break, summer break), his/her athletics aid may be reduced or canceled immediately. Certification in Writing regarding the One-Time Transfer Exception (Release) In order for student-athletes to have access to the One-Time Transfer Exception, it must be certified in writing that the original institution does not oppose the use of the exception. In the absence of the One- Time transfer Exception, a student-athlete will not be able compete immediately at the next 4-year institution in sports other than baseball, basketball and football. The student-athlete will have to serve an academic year in residence before becoming eligible to compete. In the sports of baseball, basketball or football, student-athletes do not have access to the One-Time Transfer Exception. Students-athletes will not be granted an exception until the conclusion of the semester of their request. For the exception to be provided, the student-athlete must be eligible in accordance with all University, Conference, and NCAA rules and regulations. Student-athletes requesting a “release” should keep in mind that those who leave the University under 47 Southern Illinois University

disciplinary suspension or are academically ineligible, cannot compete or receive athletics aid until they have fulfilled one year in residence at the second institution. The release will not be provided until all academic records have been approved. Approval of the use of the One-Time transfer Exception is based on approval and final authorization by the Head Coach, Sport Administrator, and Director of Athletics for all NCAA and NAIA member institutions to which the student-athlete wishes to transfer. If a request(s) for the one-time transfer exception is denied, the student-athlete will have the opportunity for a hearing to appeal the denial.

MVC Intra-Conference Transfer Policy 4.2 INTRA-CONFERENCE TRANSFER (6/14) 4.2.1 Triggers for Intra-Conference Transfer Rule. The following individuals shall be subject to the provisions of the MVC intra-conference transfer rule: 4.2.1.1 Any prospective student-athlete or student-athlete who has signed a written offer of athletically- related financial aid or has received athletically-related financial aid from any member institution of the MVC (including affiliate members in the applicable sport); and/or 4.2.1.2 Any student-athlete who has engaged in intercollegiate competition (as defined in NCAA rules) at any member institution of the MVC (including affiliate members in the applicable sport). An individual not meeting either of these conditions (for example, a student-athlete who has never accepted or received athletically- related aid and has never competed at any MVC institution) is not subject to the rule. The intra-conference transfer rule applies to both direct (4-4) and indirect (4- 2-4, 4-4-4, etc.) transfers. For purposes of the remaining provisions of this rule, an individual who has triggered the application of the intra-conference transfer rule shall be referred 19 to as the “student-athlete” even if that individual is still a prospective student-athlete under NCAA rules. 4.2.2 Definitions. For purposes of the intra-conference transfer rule: 4.2.2.1 The MVC institution from which the student-athlete is transferring shall be referred to as the “initial institution” and the MVC institution to which the student-athlete is transferring shall be referred to as the “certifying institution.” 4.2.2.2 The “residence requirement” shall be based upon full-time enrollment during a regular academic term and shall be fulfilled in the same manner as for a transfer student-athlete serving an academic year of residence under NCAA eligibility legislation. The residence requirement may be fulfilled at any collegiate institution (not only the certifying institution) or a combination of collegiate institutions. Enrollment at a non- collegiate institution (e.g., prep school) and periods of non-enrollment may not be used to satisfy the residence requirement. 4.2.2.3 The “official date of transfer” shall be the date upon which the student-athlete satisfies one of the conditions affecting transfer status under NCAA legislation to an institution other than the initial institution. 4.2.2.4 The “involved sport(s)” shall be any sport(s) in which the student-athlete signed an offer of athletically-related aid from the initial institution, received athletically-related aid from the initial institution, or competed for the initial institution. 4.2.3 Eligibility. Unless a waiver or appeal is granted, a student-athlete who has triggered the application of the intra-conference transfer rule may not compete in any sport at the certifying institution until after that student- athlete has served a one-year residence requirement from the official date of transfer. The intra-conference transfer rule does not impact a student-athlete’s ability to practice or receive financial aid at the certifying institution. 4.2.4 Impact on Other Regulations. Once a student-athlete has triggered the application of the intra- conference transfer rule, it continues to apply until the residence requirement is fulfilled, a waiver is granted pursuant to Bylaw 4.2.5, or an appeal is granted pursuant to Bylaw 4.2.6 and its subsections. Other circumstances (e.g., NLI release, initial eligibility status, graduation from initial institution) do not automatically negate or lessen the residence requirement under the intra-conference transfer rule. All NCAA rules regarding transfers (e.g., permission to contact, one-time transfer exception) shall remain in effect. 4.2.5 Waiver Procedure. Extenuating circumstances may warrant consideration of a waiver of the intra- conference transfer rule; such circumstances might include a student-athlete who is not receiving athletically- related aid at the initial institution (or whose athletically-related aid is being cancelled at the initial institution) but can transfer to the certifying institution and receive athletically-related aid. Upon request from a student-athlete, the Director of Athletics (or designee) of the initial institution must inform the student-athlete in writing of the initial institution’s decision to uphold the residence requirement or to grant a waiver. A copy of the written decision shall be provided to both the student-athlete and conference office. If this process does not result in 48 Southern Illinois University

relief from the residence requirement, the initial institution shall inform the student-athlete in writing of the right to an appeal under Bylaw 4.2.6.1. 4.2.6 Appeal Process. 4.2.6.1 Institutional Appeal. If the residence requirement under the intra-conference transfer rule is not waived by the initial institution, the first step in the appeals process shall be an institutional hearing. The hearing must be conducted by the institution in accordance with NCAA requirements for an appeal of the one-time transfer exception. A copy of the written decision shall be provided to both the student-athlete and the conference office. 4.2.6.2 Appeal to Academics/Eligibility/Compliance Committee. If the institutional appeals process does not result in the residence requirement under the intra-conference transfer rule being waived by the initial institution, the certifying institution may appeal the initial institution’s decision to the Academics/Eligibility/Compliance Committee. The Academics/Eligibility/Compliance Committee shall have the authority to uphold the initial institution’s determination or to grant full or partial relief to the student-athlete. In cases in which a member of the Academics/Eligibility/Compliance Committee is a representative of either the initial or certifying institution, that member shall be replaced by an appropriate representative of an institution not involved in the transfer at the discretion of the chair of the Academics/Eligibility/Compliance Committee. The involved 21 student-athlete shall be provided the opportunity to actively participate in the appeal. 4.2.6.3 Appeal to Joint Committee. If the relief requested by the certifying institution is not granted by the Academics/Eligibility/Compliance Committee, the certifying institution may appeal the AEC Committee’s decision to the Joint Committee. No party other than the certifying institution may appeal the Academics/Eligibility/Compliance Committee’s decision. The Joint Committee shall have the authority to uphold the Academics/Eligibility/Compliance Committee’s determination or to grant full or partial relief to the student-athlete. Representatives of the initial and certifying institutions shall be recused from the voting. The decision of the Joint Committee shall be final.

INTERNATIONAL STUDENTS Upon arrival on campus, each international student-athlete may be required to complete the NCAA General Amateurism and Eligibility form which will be provided to you by the Saluki Athletic compliance office. A meeting with the SIU Nonresident Alien Tax Specialist is also required. The end result of the above procedures is that: 1. Foreign student-athletes receiving on-campus room and board will have 14% of the room and board amount charged to their Bursar account (unless their respective countries have tax-exempt treaties with the U.S.) 2. Foreign student-athletes receiving off-campus room and board will have 14% of the monthly amount payable withheld from their monthly check (unless their respective countries have tax-exempt treaties with the U.S.

STUDENT-ATHLETE HEALTH AND WELLNESS

Nutrition and Performance  Athletic performance and recovery for college athletes is enhanced by attention to nutrient intake.  Developing an ideal nutrition plan for health and performance includes identifying the right quantity, quality and proper timing of food and fluids needed to support regular training and peak performance.  As training demands shift during the year, student-athletes also need to adjust their intake and distribution of essential nutrients while maintaining a properly balanced diet that supports their academic, training and competition needs.

Nutrition/Weight Management For those of you who desire to change your eating habits, a registered dietician is available on campus through the SIU Wellness Center.  The first step is to see the certified athletic trainer in charge of your sport.  Your athletic trainer will give a three-day recall form for you to fill out and then make an appointment with the registered dietician, Lori Trentacosti.  The athletic training staff, as well as a graduate assistant from the Community Nutrition program, is available at any time to discuss any questions you may have about nutrition and weight management.

Sleep When asked what they would do with an extra hour, over 50% of student-athletes say “sleep,” (PAC12 Report, 2017). Why is sleep a problem for student-athletes?  Schedule constraints  Travel  Training and competition  Over-scheduling  Beliefs and attitudes about sleep  Increased sleep needs  How can not getting enough sleep affect my performance?  Decreased accuracy  Decreased reaction time  Decreased speed  Increased likelihood of injury  Increased likelihood of illness  More instances of depression and anxiety  What can I do to get better sleep?  Be consistent. Go to bed at the same time each night and get up at the same time each morning, including on the weekends.  Make sure your bedroom is quiet, dark, relaxing, and at a comfortable temperature  Remove electronic devices, such as TVs, computers, and smart phones, from the bedroom  Avoid large meals, caffeine, and alcohol before bedtime.

Fueling Station

The fueling station is located in the south concessions area of the arena. It will be open daily from 7:00 - 10:00am and 2:00 to 6:00pm. Here are a few guidelines that each athlete needs to be aware of: Please have student ID present when using the station, fueling station is for student-athletes only. Each athlete is limited to 4 items per day including drinks.

1. Please use the fueling station for what it is intended to be, the nutritional items provided are not intended to supplement the 3 major meals of the day (Breakfast, Lunch, Dinner). 2. Please respect the staff of the station, if at any point there is an issue, please see the following, Jim Morrison (GA Fueling Station), David Rule (Head Athletic Trainer), or Meade Smith (Head Strength & Conditioning Coach), Megan Hammel (GA Fueling Station). 3. Please pick-up after yourself.

If you would like further guidance with your nutritional needs, please see your Head Coach, assigned Athletic Trainer, or assigned Strength and Conditioning Coach. Once notified, they will assist you by setting an appointment up with Jim Morrison (GA Fueling Station) or Registered Dietitian on campus.

MENTAL HEALTH

Mental health matters to collegiate athletics and is a key part of student-athlete wellness. Student-athletes are at an increased risk of mental health issues due to high demands of their sports.

 Student-athletes in all three divisions have identified mental health as their #1 health and safety priority.  30% of student-athletes self-report that they are consistently overwhelmed.  73% of student-athletes believe that their coach cares about their well-being.  25% of student-athletes report being exhausted from the mental demands of their sport.

Where can I go for help now?  You have access to Counseling and Psychological Services (CAPS) on campus.  Located in the Student Health Center on the 2nd floor, the Counseling and Psychological Services (CAPS) is available for all SIU students.  The staff works to help students resolve problems that interfere with personal, social, and academic functioning while also emphasizing prevention, development, adjustment, and wellness.  Many students make use of individual counseling where they can discuss personal issues in a private setting that assures them confidentiality.  Generally, individual counseling sessions last 45 minutes.  Students may attend a single session, but most return for several sessions with the same counselor.  No one can be notified that a student is receiving counseling without the student’s written permission.  The per-session fee for session 1-12 is $10.00, and most clients finish therapy within 12 sessions.  To make an appointment, please call (618) 453-5371.  The office is open Monday - Friday from 8:00 am to 4:30 pm  For more information: https://shc.siu.edu/counseling/

Eating Disorders Eating disorders are not simply disorders of eating, but rather conditions characterized by a persistent disturbance of eating or an eating-related behavior that significantly impairs physical health or psychosocial functioning.

The eating disorders most often diagnosed are:  Anorexia nervosa is characterized by persistent caloric intake restriction, fear of gaining weight/becoming fat, persistent behavior impeding weight gain, and a disturbance in perceived weight or shape.  Bulimia nervosa is recurrent binge eating, recurrent inappropriate compensatory behaviors to prevent weight gain (for example, induced vomiting and excessive exercise), and self-evaluation unduly influenced by shape and weight.  Binge-eating disorder is recurrent episodes of binge eating without compensatory behaviors but with marked distress with the binge eating.

MEDICAL POLICIES & PROCEDURES

General Information All student-athletes should be familiar with Intercollegiate Athletics medical policies (outlined below). Failure to comply with these policies may result in inadequate medical treatment or unnecessary costs charged to the student- athlete. Be sure to note differences in insurance policies for athletic related and not-athletic- related injuries and illnesses.

Physicals Pre-participation physicals will be performed only on 1) freshmen and new transfer student-athletes, 2) those student- athletes who request a physical, and/or 3) those student-athletes who the athletic training or medical staff feels should have a physical. All physical examinations will be performed by the team physician or his/her designee. During your physical it is important to report any and all injuries and illnesses that occurred prior to or during time away from SIU in order to help the medical staff provide the best care possible.

SIU Student Health Center  The SIU Student Health Center is open on weekdays from 8:00 a.m. to 4:30 p.m., the last appointment time is 3:45 p.m. https://shc.siu.edu/index.php  You must always try to contact your athletic trainer before calling the Student Health Center yourself.  If you call the Student Health Center yourself, the nurse will not allow you to make your own appointment and you will be told that you must go through the athletic training room to schedule your appointment.  Most injuries and illnesses will be treated at the Student Health Center.  Numerous specialists are available to provide care for student-athletes in addition to the Student Health Center staff.  Brochures on Student Health Center policies, including treatment and insurance, are available in the athletic training room.

Policies-Athletic and Non-Athletic Related Injuries and Illnesses  Report all injuries and illnesses to the athletic trainer immediately.  If you are ill or injured, call the Athletic Training Room 453-HURT (453-4878) before 11:00 a.m. If necessary a staff member will make an appointment for you.  The Student Health Center requires patients to report 15 minutes prior to the appointment time. If you are late for an appointment or miss an appointment without canceling with the Student Health Center in advance, you will be required to pay a $10.00 charge.  An athletic trainer accompanies student-athletes to appointments whenever possible.  Referral to physicians for ALL injuries and illnesses can be made ONLY by the athletic training staff except in cases of extreme emergency. Initial referrals are made to Team Physicians, Dr. Jon Humphrey or Dr. Scott Schonewolf of the Student Health Center. The Team Physicians will make any necessary additional referrals.

If you are injured, keep the following in mind:  The athletic training room is here for your benefit.  You will benefit only if you seek treatment and follow through with the treatment plan as prescribed.  You must be treated in the athletic training room each morning and before and after each practice.  If you do not show up for a scheduled treatment or are late to a scheduled treatment, this will be reported to the head coach and recorded on your treatment record.  Missing treatment will be treated the same as missing practice.  All disciplinary action will be at the discretion of the head coach.  Unless directed otherwise by your head coach or the athletic trainer, all injured student-athletes will be present during practice with the team.

Insurance All student-athletes must purchase and maintain the SIU Student Health Plan, underwritten by AETNA Life Insurance Company, for the 2018-19 academic year. Intercollegiate Athletics requires all student-athletes to have this plan so that the Sports Medicine staff can provide an expedient and high level of care.

 The SIU Student Health Plan provides coverage for services provided outside of the SIU Student Health Center including hospital emergency departments and specialist referrals.  Medical coverage through the SIU Student Health Plan, for the 2018-19 school year will provide PRIMARY coverage for any medical claims arising from athletic participation or non-athletic injuries or illnesses.  This means that any other health insurance that is held by the student-athlete is considered SECONDARY.  The student-athlete must provide an Explanation of Benefits on a claim from primary and secondary insurances to the SIU Athletic Training Staff before the SIU Intercollegiate Athletics can make payments on Athletic Claims.

Should an off-campus medical claim be incurred, the following steps must occur:

1. Be sure to present your Student Health Insurance Card when visiting a Medical Provider. 2. Confirm if the Medical Provider is an In-Network Preferred Provider or a private physician. You can find an In- Network Provider by finding a doctor, hospital or pharmacy. 3. If you are being treated by an In-Network Provider, your claim will automatically be submitted to the Plan by the Medical Provider. If you are being treated by a Non-Network Provider, ask how your claim will be filed with the Medical Provider. The Provider may ask for payment immediately. If the Provider does not file the claim directly with Aetna Student Health, then you must file the claim by submitting an itemized bill to us immediately after treatment. Your name, social security number and school name should be written clearly on all medical bills. Always retain copies for your records. 4. Payment on filed claims for covered expenses will be mailed directly to the hospital or doctor unless proof of payment is submitted with the claim. 5. Once a claim is processed, an Explanation of Benefits (EOB) Statement will be mailed to you explaining the benefit paid to the Provider. The Provider will then send you a statement indicating if a balance is due. 6. Bring any Explanation of Benefits (EOB) Statement or statements indicating a balance is due to your Staff Athletic Trainer. Failure to do so could result in out of pocket expenses to the student-athlete or their family. This could also result in delayed payment being referred to collection agencies which can negatively impact credit reports.

Finally, you may be responsible for any additional medical costs on routine injury or illnesses not paid by either insurance. It is essential that the Student-Athlete's Family Insurance Information and Record Release Form requesting parents’/guardians’ insurance information be completed fully and accurately. Also, please include a copy of the front and back of the student-athlete's insurance card with the form. This will greatly assist the staff in processing your medical claims. If you have questions about insurance and/or medical bills, please see a member of the Sports Medicine Staff immediately.

Second Opinions If a student-athlete wishes to seek a second opinion on an athletic injury, they may do so, but it is mandatory that they contact their Staff Athletic Trainer to help coordinate proper physician referrals so that the correct steps are taken to make sure that insurance will cover appropriately. Saluki Athletics will not be responsible for any payment associated with a second opinion or related care.

Dental and Vision Care As a general rule, routine dental and vision care is not covered by the SIUC Student Medical Insurance Plan. Dental services ranging from cleaning to emergency care are available on campus for a nominal fee. As with all other health care, your dental and vision care should be coordinated through the Athletic Training Staff.

Athletic-Related Injuries  The Student Health Center Student Insurance Plan covers athletic related injuries which occur during practice and/or competition in an intercollegiate sport under the direct supervision of an athletics staff member.  The Student Health Center Plan is now Affordable Care Act compliant, and will cover pre-existing conditions.  The SIUC coverage for student-athletes requires that a student-athlete initially seek attention for an athletic injury from an SIUC staff athletic trainer and the Intercollegiate Athletics team physician.  If a student-athlete would seek treatment from an outside physician without an SIUC Referral, the Plan will not make payment.

Non-Athletic Injuries and Illnesses  The SIUC Student Health Center Clinic is the primary care provider for the student.  Appointments are scheduled through the athletic trainer for non-athletic injuries or illnesses (cold, flu, etc.). Appointments can be scheduled through the Saluki Health Portal: https://shc.siu.edu/appointments/.  The student is billed a fee of $10.00 per visit, and this charge is billed on the Bursar’s Account or can be paid at the end of the appointment.  Medication prescribed for non-athletic injuries and illnesses is covered by the Student Health Plan, and a copay may apply.  The student will be billed for medication received at the Student Health Center Clinic/Pharmacy.  This can also be paid through the Bursar Account or in person at the pharmacy.  Please refer to the SIUC Student Health Centers website: https://shc.siu.edu.

Required Record of Immunization  Illinois law requires the SIUC Student Health Center to have a current record of immunization on file for each student.  The record must include dates of immunization for measles, mumps, rubella, and tetanus. International students must have a current record of a tuberculosis test.  If you have no immunization record on file, you will receive a form when you enroll for your physician to complete.  Please return the form to the SIUC Student Health Center.  If you do not receive an immunization form, please contact one of the athletic trainers, and a form will be sent to you.  Failure to comply will result in a hold being placed on your registration.

Emergencies  The Sports Medicine staff discourages students with colds or flu from making unnecessary visits to the emergency room.  In cases of an athletic injury/illness, the athletic trainer determines whether the student-athlete should be sent to the emergency room.  In other cases, the student-athlete should call their athletic trainer or the Student Health Center if they are in doubt as to whether their injury/illness is emergent.

Student-Athlete Pregnancy  Saluki Athletics will be proactive when a student-athlete becomes pregnant.  The procedures for dealing with a student-athlete pregnancy will be consistent with NCAA guidelines.  Institutional grant-in- aid will not be affected when a student-athlete becomes pregnant.  As soon as you learn of your pregnancy, we encourage you to tell your coach, a member of the athletic training staff, an athletic administrator or a Student Health Service physician so that the proper training precautions can be taken to protect your health and that of your baby.  Please note: Female student-athletes are permitted a one-year extension of the five-year period of eligibility under NCAA rules for reasons of pregnancy.  A student-athlete will not be encouraged to terminate a pregnancy because of financial or psychological pressure or fear of losing her institutional grant-in-aid.  For more information, visit the NCAA student-athlete pregnancy page: http://www.ncaa.org/about/resources/inclusion/pregnant-parenting-student-athletes

WEIGHT ROOM POLICIES AND PROCEDURES

1. Troutt-Wittmann Performance Center is for SIU sport athletes, athletic department staff, and coaches only unless approved by the Athletic Director. 2. Athletics Staff and Coaches, with approval of Head Strength & Conditioning Coach, and as designated by Athletic Director, may use the Troutt-Wittmann Performance Center for personal workouts provided they do not interfere with the needs of the student-athletes utilizing the facility. 3. Every athlete, departmental staff, and coach must go through an orientation before training in the facility. 4. The only training programs allowed are those designed or approved by SIU Performance Staff. 5. No food, including gum, in Troutt-Wittmann Performance Center. 6. Drinks (water/Gatorade) must be contained in a sport bottle at all times. 7. Absolutely no tobacco products allowed at any time in performance center. 8. Music will be controlled by performance staff only and must be appropriate. Cell phones and headphones are only allowed during voluntary lift times not during organized team training. 9. It is the athlete’s responsibility to ensure proper nutrition, hydration, and rest prior to and following each training session to ensure training progress. 10. Teams and athletes utilizing Troutt-Wittmann Performance Center will arrive 10 minutes prior to scheduled training time to properly prepare themselves both physically and mentally. 11. All training begins with proper warm-up involving soft tissue work (foam rollers), joint mobility, and thermogenic exercises. Each training session is followed with proper regeneration techniques involving soft tissue, flexibility, and recovery nutrition protocols. Exact warm-up/regeneration protocols will be designed by performance staff. 12. Only excellent technique in all training movements will be tolerated. This means having control of your body, barbell, and dumbbells at all times. Focus and effort is critical in performing excellent technique. 13. Athletes must follow training protocols exactly as they are prescribed. This progression is designed for maximal improvement and peak strength. 14. Spotters must be present during movements that require a spot (squats, horizontal/vertical pressing). 15. Athletes will assist in maintaining a SAFE training environment. This includes using competent spotters and weight collars. 16. Athletes will assist in maintaining a CLEAN training environment. This includes returning all strength dumbbells, plates, and barbells to proper positions, sanitizing training equipment when finished, and returning training records to performance staff. 17. When entering Troutt-Wittmann Performance Center, be prepared to train. Wear appropriate performance attire and shoes tied. Only 100% effort is acceptable. 18. Respect Performance Staff, Facility and Teammates. 19. Remember, SIU Performance Staff is here to ensure that EACH athlete reaches maximal potential! 20. If you cannot make a workout or will be late, you must notify the strength staff. The weight room phone number is 618-453-5454.

Dress Code 21. This is Saluki Country, no other college/university attire will be allowed. 22. All teams with issued training gear must wear it during team organized training time. 23. No jewelry (rings, bracelets or watches, dangling earrings). 24. Appropriate training shoes must be worn at all times (shoes tied, no tear in soles, and no barefoot shoes during speed/agility).

Supplemental Nutrition The supplemental nutrition offered in the weight room (protein, milk, provided snacks) are intended only for student athletes.

Behavioral Guidelines Disciplinary actions will be based on the severity of the facility rules and regulations violation. The discipline may include physical workouts, cleaning duties, or suspension from the facility. The Head Strength and Conditioning Coach will consult with Sport’s Head Coach when determining disciplinary actions.

Covid-19 Policy

This policy is based on CDC, Jackson County Health Department, NCAA, State of Illinois, and institutional guidance, and will be updated as necessary when new Guidance from those entities becomes available.

Student-athlete Positive COVID-19 Test Protocol  Results are obtained in one of two ways: o SIH notifies Dr. John Humphrey; or o Dr. John Humphrey gathers the information from Epic  Dr. Humphrey will notify David Rule (Director of Sports Medicine) and Kerri Blaylock (SWA) of the positive test result  David Rule will notify the student-athlete’s athletic trainer who then will notify the Head coach.  Kerri Blaylock will notify Liz Jarnigan (Director of Athletics) and Ben Newman (Chief of Police)  Dr. Humphrey will notify the Jackson County Health Department to ensure they have received the information  The Jackson county health department will commence contact training with the department of athletics supporting the process  If a student-athlete is concerned they may have been exposed to covid-19, s/he will be able to get tested. However, per Jackson county health department guidelines, the test can only occur after a 5 day isolation period subsequent to the exposure.  The student-athlete must request the test through the team’s athletic trainer who will then consult with Dr. Humphrey  Test results will only be accepted if the test is administered within this protocol

DRUGS, ALCOHOL AND TOBACCO POLICY

The athletics department does not condone the illegal or irresponsible use of alcohol under any circumstances. The use of alcohol, all tobacco products, and illicit drugs is prohibited in all sports during practice, competition, and while on Saluki Athletic-sponsored trips. Any student-athlete violating this rule may be subject to disciplinary action by his/her team, coach, Saluki Athletics and/or the University. A student-athlete who uses tobacco products during practice or competition shall be disqualified for the remainder of that practice or competition.

DRUG AND ALCOHOL EDUCATION AND TESTING PROGRAM

Saluki Athletics at Southern Illinois University is concerned with the health, safety and welfare of the student-athletes who participate in its programs and represent the University in competitive athletics.  Random drug testing is appropriate to ensure the health and safety of our student-athletes, to promote fair competition in intercollegiate athletics, to affirm compliance with the NCAA rules and regulations on drug and alcohol abuse, and to identify student-athletes who are improperly using drugs or alcohol and assist them before they harm themselves or others, or become physiologically or psychologically dependent.  The athletics program recognizes its responsibility to provide educational programming that will support positive decision-making processes.  Saluki Athletics will encourage and support programming which educates student-athletes on the physiological and psychological dangers inherent in the misuse of drugs and alcohol, inform student-athletes about the local, state, and federal laws concerning the use and possession of alcohol and drugs, and reinforce alternative activities supporting a drug or alcohol-free lifestyle.  A drug and alcohol program which embraces an educational screening, and (if necessary) rehabilitation component, will facilitate a positive decision-making process for our student-athletes.

Conditions of Eligibility  All student-athletes must sign an NCAA form by which the student-athlete consents to be tested for the use of drugs prohibited by NCAA legislation.  In addition, all student-athletes must sign a University consent form to allow for testing and limited release of test results, as set forth in these procedures, as a condition of eligibility (See Appendix A).  This policy is not to be construed as a contract between the institution and the student-athletes at Southern Illinois University. However, the signed consent and notification forms shall be considered affirmation of the student-athlete's agreement to the terms and conditions in this policy. Southern Illinois University may amend this policy at any time.  Southern Illinois University’s institutional drug testing program is separate and distinct from the NCAA Drug Testing Program.

Education Program  Saluki Athletics, in collaboration with the Student Health Program’s Wellness Center and the SIUC Counseling and Psychological Services provides confidential, professional and voluntary assistance and support to student- athletes regarding personal problems that might adversely affect their academic or athletic performance.  The educational efforts of Saluki Athletics, the Student Health Program’s Wellness Center and the SIUC Counseling and Psychological Services address the harmful effects of substance abuse as well as alternative behaviors to cope with the stresses of being a student-athlete.  Educational programs are offered to increase awareness, prevent problems and promote the general well-being of the student-athlete in fulfillment of the mission of the Saluki Athletics Alcohol and Drug Education and Testing Program and our obligations as a NCAA member institution.

Testing Procedures Saluki Athletics will conduct substance abuse testing in accordance with the accepted procedures identified in this document. Selection for testing will be based on: 1) Random selection 2) A prior positive test as outlined in the Sanctions section below 3) Reasonable cause as suspicion of misuse/abuse 4) Other reasonable cause as outlined in the Reasonable Cause section.

SIUC Intercollegiate Athletics retains the right to test entire teams at the discretion of the Head Coach, with approval from the Director of Athletics, Assistant AD for Sports Medicine, and the Team Physician. A baseline test will be performed for all incoming freshmen and transfer student-athletes during their initial pre-participation physical exam.  As part of this testing, a student-athlete will be asked to take a urine test to detect illegal drugs, non-prescribed drugs, abuse of prescribed drugs, alcohol abuse, use of narcotics and/or performance enhancing drugs (PEDs), including nutritional supplements, at such times and places as directed by Intercollegiate Athletics.

What is tested for during drug tests? The drug or drug classes to be tested for include, but are not limited to the following:  Alcohol  Stimulants  Anabolic agents  Nutritional supplements  Diuretics  Street drugs  Peptide hormones and analogues  *not limited to items on this list

Who gets tested? Testing may be conducted on all student-athletes on the squad list, including student-athletes actively participating, student-athletes with medical disabilities, student-athletes who are red shirted, student-athletes who are partial or non- qualifiers, and those who have exhausted their eligibility, but are still receiving athletic aid.

When can I be tested? Unannounced testing may occur at any time of any day during the academic year, summer session or after the start of preseason practice, with a minimum of two (2) hours’ notice. Notice will be provided in writing by the Assistant Athletic Director (AD) for Sports Medicine or their designee or head coach.

What is considered a positive test?  Evidence of substances listed above in urine sample.  The refusal to provide a sample will be considered a positive test. All test results accumulate during the entire time that an individual is a student-athlete at SIUC.  The use of banned methods including catheterization, urine substitution, and/or tampering with or modification of renal excretion by the use of diuretics or related compounds. Any urine sample tested for the presence of banned substances may also be tested for the presence of substances used to alter the integrity or validity of urine samples.  A positive finding for these substances or other methods of tampering will be considered a positive test.  Any attempt to tamper with a urine sample during the collection process will be treated as a positive test.

Positive alcohol test  A positive test for alcohol for any student-athlete under 21 years of age will result in the sanctions set forth in this policy.  Any student-athlete, 21 years of age or older who tests positive for alcohol, may be subject to sanctions pursuant to the rules of the applicable sport.  Additionally, a saliva test or breathalyzer may immediately be required upon determination by a Coach that a student-athlete appears to be currently under the influence of or impaired by alcohol. If a student-athlete refuses to submit to such a test, they will receive sanctions for a positive test result.

Intercollegiate Athletics may also issue a positive test-finding to any student-athlete upon conviction or plea of the following: 1. Driving under the influence (DUI, DWI) or other motor vehicle violations involving alcohol or drugs. 2. Drunk and disorderly. 3. Other violations of law involving alcohol or drugs 4. Violations of the SIUC Student Conduct Code involving alcohol or drugs. All urine samples for testing by SIUC will follow Specimen Collection Procedures outlined in Appendix C.

Collected samples will be coded for confidentiality and will remain under the supervision of the Assistant AD for Sports Medicine or his/her designee prior to being shipped to the testing facility. Identification information for each sample will remain under the constant supervision of the Assistant AD for Sports Medicine until results are returned to him/her from the testing facility.

Reporting Results  For student-athletes who have a positive finding, the laboratory will contact the Assistant AD for Sports Medicine or a designee via fax or secure internet access.  A positive test result will be reviewed by the Team Physician if necessary to determine if there is an acceptable explanation for the result.  The Assistant AD for Sports Medicine or their designee and the Head Coach will notify the student-athlete of the finding.  The student-athlete will receive written notification of a positive result along with notification of their right to appeal the finding.  The results of the testing program become a part of the student-athlete’s medical record and are considered to be confidential.  Records and other information shall remain in the confidential possession of the Assistant AD for Sports Medicine or their designee and may be released only as stated in the consent form and as set forth in these Procedures.  In the event of a valid subpoena to release the record and in accordance with applicable law, the affected student-athlete will be notified before compliance with the subpoena.

“Safe Harbor” Program  A student-athlete eligible for the Safe Harbor Program may refer themselves for voluntary evaluation and treatment for alcohol or other drug problems.  A student-athlete is not eligible for the Safe Harbor Program after they have been informed of an impending drug test or after having received a positive finding on an SIUC or NCAA drug test.  SIUC Counseling and Psychological Services will work with the student-athlete to prepare a Safe Harbor treatment plan, which may include confidential drug testing.  If the student-athlete tests positive for a banned substance upon entering the Safe Harbor Program, that positive test will not result in any administrative sanction unless the student-athlete tests positive in a subsequent retest or the student-athlete fails to comply with the treatment plan.  The team physician may suspend the student-athlete from play or practice if medically indicated.  A student-athlete will be permitted to remain in the Safe Harbor Program for a reasonable period of time, not to exceed thirty (30) days, as determined by the treatment plan.  A student-athlete will not be permitted to enter the Safe Harbor program thirty (30) days prior to NCAA or Conference post-season competition.  If a student-athlete tests positive for any banned substance after entering the Safe Harbor Program or fails to comply with the Safe Harbor treatment plan, the student-athlete will be removed from the Safe Harbor Program, an initial Safe Harbor positive test will be treated as a first positive and a subsequent positive as a second positive, subject to the sanctions explained in this policy.  While in compliance with the Safe Harbor Program treatment plan, the student-athlete will not be included in the list of student-athletes eligible for random drug testing by SIUC.  Student-athletes in the Safe Harbor Program may be selected for drug testing by the NCAA.  The Director of Athletics, the Assistant AD for Sports Medicine, the student-athlete’s Head Coach and the Team Physician may be informed of the student-athlete’s participation in the Safe Harbor Program.  The athletic trainer assigned to that sport also may be notified, if medically appropriated.  The assistant coach(es) also may be informed at the discretion of the Head Coach.  Other university employees may be informed only to the extent necessary for the implementation of this policy.

Testing Prior to NCAA Championship  Teams and/or individuals likely to advance to NCAA Championship competition are subject to additional testing.

 Testing may be required of all team members or individual student-athletes at any time within thirty (30) days prior to participation in NCAA Championship competition.

 If a student-athlete tests positive, they will not be allowed to compete at the NCAA Championship event unless they subsequently test negative prior to departure for the event.

 A positive test result from NCAA drug testing will be treated as a positive finding under the SIU policy and appropriate sanctions will apply.

Sanctions after Positive Results – SIU Policy Intercollegiate Athletics reserves the right to set the detection cut-off level it deems appropriate in the determination of a “positive” for drug testing purposes. Even if a specimen is below the cut-off level for finding a positive result and the appropriate sanction level, any detection of the presence of a banned substance will subject the individual to additional testing and wellness counseling. Although this policy will impose specific sanctions, nothing in this policy precludes a head coach from enforcing a more restrictive team policy or team rule. A positive test on a pre-participation screening will not cause the student-athlete to be sanctioned as described by the drug testing policy, but will enroll the student-athlete in the drug testing program. Any further positive tests will make the student-athlete eligible for the sanctions as described in the drug testing policy.

A. First Positive Test Result The Assistant AD for Sports Medicine, Team Physician, Director of Athletics and Senior Associate AD will be notified. The Director of Athletics or his designee will notify the Head Coach. The Head Coach and the Assistant AD for Sports Medicine or his/her designee will inform the student-athlete of the positive finding. The student- athlete will be subject to additional testing whenever SIUC Intercollegiate Athletics conducts testing over the next twelve (12) months with a minimum of two (2) hours’ notice. If the student-athlete is under 21 years of age, a mandatory conference call will be conducted with the student-athlete’s parent/guardian/spouse, the Director of Athletics or his/her designee, and the Head Coach. Any loss-of- competition sanction imposed will carry over into the following year if it is not possible to complete the sanction in the current year. The student-athlete will be required to have an evaluation at the SIUC Counseling and Psychological Services office. The evaluation may also occur at an external agency with prior approval by the Assistant AD for Sports Medicine. Recommendations for treatment from Student Wellness or any external agency will be forwarded to the Director of Athletics. An appointment for evaluation must be made by the student-athlete within a week of notice to the student-athlete of the positive test result. The student-athlete must comply with the treatment recommendations in order to return to/continue to participation in his/her sport. The recommended treatment must be completed within a reasonable period of time (as determined by the individual conducting the evaluation) following the positive test result. The Assistant AD for Sports Medicine will be informed of compliance with the treatment plan. The student-athlete is responsible for all costs incurred for services not provided on campus or not covered by insurance. Any student- athlete who has a positive drug test will be required to complete community service for the offense. The community service must be completed within sixty (60) days of notification of the positive test.

B. Second Positive Test Results The Assistant AD for Sports Medicine, Team Physician and Director of Athletics will be notified. The Director of Athletics or his designee will notify the Head Coach. The Head Coach and the Assistant AD for Sports Medicine or his/her designee will inform the student-athlete of the positive finding. The student-athlete will be subject to additional testing whenever SIUC Intercollegiate Athletics conducts testing over the remainder of the student- athlete’s eligibility with a minimum of two (2) hours’ notice. If the student-athlete is under 21 years of age, a mandatory conference call will be conducted with his/her parent/guardian/spouse, the Director of Athletics or his/her designee, and the Head Coach. The student-athlete will be suspended immediately upon positive notification for the number of the next scheduled contests equal to 20% of the total number of regularly scheduled contests in that sport. (Suspension from an athletic contest does not relieve the student-athlete from any other responsibilities as a team member, including, but not limited to attendance at all practices or other team events.) Any loss-of-competition sanction imposed will carry over into the following year if it is not possible to complete the sanction in the current year. An appointment for evaluation at Counseling and Psychological Services must be made by the student-athlete within a week of notice to the student-athlete of the positive test result. The student-athlete must comply with the treatment recommendations in order to return to/continue participation in his/her sport. The recommended treatment must be completed within a reasonable period of time (as determined by the individual conducting the evaluation) following the positive test result. The Assistant AD for Sports Medicine will be informed of compliance with the treatment plan. The student-athlete is responsible for all costs incurred for services not provided on campus or not covered by insurance.

C. Third Positive Test Result The Assistant AD for Sports Medicine, Team Physician and Director of Athletics will be notified. The Director of Athletics or his designee will notify the Head Coach. The Head Coach and the Assistant AD for Sports Medicine or his/her designee will inform the student-athlete of the positive finding. The third positive test finding will result in the student-athlete’s termination of team membership. A recommendation will be made for cancellation and non- renewal of athletics scholarship aid at the earliest possible moment consistent with Conference and NCAA rules. If the student-athlete is under 21, a mandatory conference call will be conducted with the student-athlete’s parent/guardian/spouse, the Director of Athletics or his/her designee, and the Head Coach.

Immediate Termination from Team Membership A student-athlete will be terminated from team membership upon conviction of trafficking/dealing in any illegal substance. Termination will also include a recommendation of non-renewal of any athletics scholarship at the earliest possible moment consistent with Conference and NCAA rules.

Referral Process  In the event of a positive test, the student-athlete will be required to seek counseling and comply with the treatment recommendations of the counselor.  Part of the referral process will be to sign a waiver and release of information that allows the Director of Athletics or their designee to contact parents/guardians and allows release of any treatment plan or recommendations to the Director of Athletics or their designee and parents/guardians.  This waiver will also allow the Director of Athletics or their designee to monitor compliance with the treatment plan.  Failure to comply with the treatment recommendations of the Student Wellness office or external counseling agency will subject the student-athlete to suspension from the team for a period of time designated by the Director of Athletics.

Appeal Process Student-athletes who test positive under the terms of the Drug and Alcohol Education and Testing Program will be entitled to a hearing prior to imposition of any sanction.  Requests for a hearing must be made to the Director of Athletics in writing within forty-eight (48) hours of notification of a positive test result.  If the forty-eight hours would end on a weekend, the request must be made by noon on the next business day.  Appeals will be heard by a three-person Appeals Committee comprised of the Team Physician, the Faculty Athletics Representative and the Senior Associate Athletic Director or their designees. The Committee will make a recommendation on each appeal to the Director of Athletics.  The decision by the Director of Athletics or their designee regarding the sanctions to be imposed will be final.  The student-athlete may have an advocate or other representative present.  The student-athlete must present his or her own case.  The hearing should take place no more than seventy-two (72) hours after the written request is received.  Either the student-athlete or other parties involved may request an extension upon a showing of good cause.  These proceedings shall include an opportunity for the student-athlete to present evidence, as well as to review the results of the drug test.  The proceedings shall be confidential.  During the appeals process, the student-athlete may continue to compete until a final decision has been rendered by the Director of Athletics or their designee.

A student-athlete who tests positive for a banned substance by the laboratory retained by the University may, within 72 hours following receipt of notice of the laboratory finding, contest the finding.  Upon the student-athlete’s request for additional testing of the sample, the Director of Athletics will formally request the laboratory retained by the University to perform testing on specimen B.  The student-athlete may choose to be present at the opening of specimen B at the laboratory.  If the student-athlete does not wish to be present at the opening of specimen B, but desires to be represented, arrangements will be made for a designee to attend the opening of the specimen.  The student-athlete or their designee will attest to the sample number and the integrity of the security seal prior to the laboratory opening of specimen B.  Specimen B findings will be final, subject to the results of any appeal.  If specimen B results are negative, the drug test will be considered negative.  The student-athlete will be responsible for all costs associated with specimen B analysis (e.g. laboratory costs, travel costs).

Responsibility of the Student-Athlete  Each student-athlete is responsible for availing themselves of the educational opportunities provided by Saluki Athletics regarding substance abuse.  If the student-athlete tests positive under the guidelines set forth in the SIUC Testing Program, it will be the student-athlete’s responsibility to seek counseling and comply with recommendations of the counselor.  The student-athlete is responsible for all costs incurred for services not provided on campus or not covered by insurance.

Testing For Reasonable Cause Nothing in this policy shall prevent or limit the Southern Illinois University Carbondale’s right to require a student-athlete to submit to testing when there is cause to do so.  For purposes of this section, “cause” shall be defined as behavior, conduct, or performance by the student-athlete which leads the University to conclude that there is the likelihood that the student-athlete is taking or is under the influence of illegal drugs, banned substances (as defined above) or alcohol.  Among the indicators which may be used in evaluating a student-athlete’s behavior, conduct, or performance are: class attendance, significant GPA changes, athletic practice attendance, increased injury rate or illness, changes in physical appearance, academic/athletic motivational level, emotional condition, mood changes and legal involvement.  Reasonable cause may be based on information received that a student-athlete is using illegal drugs or alcohol.  In such cases, the determination that “cause” exists to require a student-athlete to submit to drug testing will be made only after consultation between the Director of Athletics, Assistant AD for Sports Medicine, and the Head Coach of the sport.  All must agree that the observations, behavior, conduct, or performance of the student-athlete are such that testing for cause is required to protect the health of the student- athlete, the health of others and/or to protect the integrity of the sport.  In such cases, less than two hours’ notice may be given.

Nutritional Supplements Saluki Athletics does not encourage the use of nutritional supplements.  Nutritional supplements are not regulated by the federal government and are not tested for quality like prescription and over-the-counter medications.  The product claims made by many nutritional supplement manufacturers have not been based on scientific research in many cases.  Many nutritional supplements have not been subject to research by unbiased independent researchers in order to substantiate performance claims.  The potential adverse and/or harmful effects of these substances have not been completely studied, but serious adverse effects have been reported in some instances.  As there are no federal government labeling requirements for nutritional supplements, some products may contain NCAA banned substances, which are not listed on the label.  It is important for student-athletes to remember that each STUDENT-ATHLETE WILL BE HELD RESPONSIBLE FOR EACH AND EVERY SUBSTANCE THAT ENTERS THEIR BODY.  If you are taking or are thinking about taking a nutritional supplement, please check with your athletic trainer or strength and conditioning coach so the validity and safety of the nutritional supplement can be researched.

I hereby consent to have a sample of my urine collected and tested for the presence of drugs or alcohol in accordance with the Southern Illinois University Carbondale Intercollegiate Athletics Drug and Alcohol Education and Testing Program.

I understand that selection for testing will be based on random selection, a prior positive test as outlined in the Program, reasonable suspicion of misuse/abuse or other reasonable cause.

I understand that any urine samples will be sent only to a licensed medical laboratory for actual testing, and that the samples will be coded to provide confidentiality.

I hereby authorize the release of such testing results to the Director of Athletic, Team Physician, Head Coach, Head Athletic Trainer and other University officials as deemed appropriate. I understand that these results will also be made available to me.

Do not rely on this list to rule out any label ingredient. 1. Stimulants: amphetamine (Adderall); caffeine (guarana); cocaine; ephedrine; methamphetamine; methylphenidate (Ritalin); synephrine (bitter orange); dimethylamylamine (DMAA, methylhexanamine); “bath salts” (mephedrone); Octopamine; hordenine; dimethylbutylamine (DMBA, AMP, 4-amino methylpentane citrate); phenethylamines (PEAs); dimethylhexylamine (DMHA, Octodrine) etc.

Exceptions: phenylephrine and pseudoephedrine are not banned.

2. Anabolic Agents (sometimes listed as a chemical formula, such as 3,6,17-androstenetrione): androstenedione; boldenone; clenbuterol; DHEA (7-Keto); epi-trenbolone; testosterone; etiocholanolone; methasterone; methandienone; nandrolone; norandrostenedione; stanozolol; stenbolone; trenbolone; SARMS (ostarine, ligandrol, LGD-4033); etc.

Appendix A

Collection Procedures

1.Only those persons authorized by the institution will be allowed in the collection room.

2. When arriving to the collection room, the student-athlete will provide photo identification or a client representative will need to identify the student-athlete. The student-athlete will then print his or her name and arrival time on the Roster Sign-In Form.

3. The student-athlete will select a Custody & Control Form (CCF) from a supply of such and work with the institutional collector to complete the necessary information before proceeding with the specimen collection process.

4. The student-athlete will select a specimen collection beaker from a supply of such and will be escorted by the institutional collector (same gender) to the restroom to provide a specimen. The student-athlete will rinse his or her hands with water (no soap) and then dry their hands. Then the student-athlete will place a specimen barcode from the Custody & Control Form onto the beaker.

5. The institutional collector will directly observe the furnishing of the urine specimen to assure the integrity of the specimen.

6. The student-athlete will be responsible for keeping the collection beaker closed and controlled.

7. Fluids and food given to student-athletes who have difficulty voiding must be from sealed containers (approved by the institutional collector), opened and consumed in the collection room. These items must be free of any other banned substances.

8. If the specimen is incomplete, the student-athlete must remain in the collection room until the sample is completed. During this period, the student-athlete is responsible for keeping the collection beaker closed and controlled.

9. If the specimen is incomplete and the student-athlete must leave the collection room for a reason approved by the institutional collector, the specimen must be discarded.

10. Upon return to the collection room, the student-athlete will begin the collection procedure again.

11. Once an adequate volume specimen is provided; the institutional collector will escort the student-athlete to the specimen processing table.

12. The specimen processor will instruct the student-athlete to closely observe the specimen processing steps and will then measure the specific gravity.

13. If the urine has a specific gravity below 1.005, no value will be recorded on the CCF and the specimen will be discarded by the student-athlete with the institutional collector observing. The student-athlete must remain in the collection room until another specimen is provided. The student-athlete will provide another specimen.

14. Once the specimen processor has determined the specimen has a specific gravity above 1.005 the sample will be processed and sent to the laboratory.

15. If the laboratory determines that a student-athlete’s sample is inadequate for analysis, at the client’s discretion, another sample may be collected.

16. If a student-athlete is suspected of manipulating specimens (e.g., via dilution, substitution), the institutional collector will collect another specimen from the student-athlete.

17. Once a specimen has been provided that meets the on-site specific gravity, the student-athlete will select a sample collection kit from a supply of such.

18. The specimen processor will open the kit, demonstrate to the student-athlete the vials are securely sealed, open the plastic and open the A vial lid. The processor will pour the urine into the A and B vials and close the lids. The specimen processor should pour urine into vials above the minimum volume level (35 mL in A vial; 15 mL in B vial) and pour as much urine as possible into the vials using care not to exceed the maximum levels (90 mL in A vial; 60 mL in B vial).

19. The specimen processor will securely close the lids on each vial and then seal each vial using the vial seals attached to the CCF; assuring seals are tightly adhered to the vials with no tears or loose areas.

20. The specimen processor must then collect all necessary signatures (collector, donor, witness, and collector/specimen processor) and dates/times where indicated on the CCF.

21. The specimen processor will place the laboratory copy of the CCF in the back pouch of the plastic bag and the vials in the front pouch of the same bag. The bag should then be sealed. The sealed bag with vials will then be placed in the sample box. The box will then be sealed.

22. The student-athlete is then released by the institutional collector.

23. All sealed samples will be secured in a shipping case. The collector will prepare the case for forwarding. When two split samples are collected and packaged, care must be taken to assure one sample is placed in the shipping container for shipment to the “drugs of abuse” laboratory and one sample is placed in the shipping container for shipment to the “anabolic steroids” laboratory.

24. After the collection has been completed, the samples will be forwarded to the appropriate laboratory and copies of any forms forwarded to the Sport Drug Testing Department.

25. The samples then become the property of the client.

26. If the student-athlete does not comply with the collection process, the institutional collector will notify the appropriate institutional administrator and Drug Free Sport.
