CLEGG Department of Anatomy, University of Liverpool INTRODUCTION Much of the difficulty experienced in evaluating and comparing work on vascular anatomy is a result of lack of uniformity in nomenclature, and the literature on this particular region of the arterial system is no exception. Previous workers appear to be divided on the question of the existence of a specific artery to the prostate gland (Kraas, 1935; Awataguti, 1939), but the measure of agreement is greater than would appear from a superficial survey of the literature, and differences in results may be explained as resulting from differences in ter- minology, rather than from fundamental anatomical variations. The object of this report is to describe in some detail the blood supply to the human prostate and seminal vesicles, to compare the various systems of nomen- clature used by previous workers, and to provide an accurate representation of the vascular patterns as a basis for surgical practice. MATERIALS AND METHODS Fresh post-mortem material was used whenever possible, the ages of the subjects varying between 36 and 64 years. In no case did the prostate show pathological hypertrophy. After ligating the common and external iliac arteries close to their origin the former was cannulated distally, and an injection of 10-15 ml. of a radio-opaque medium ('Micropaque'; Damancy and Co. Ltd) was made. This injection mass com- bines high density for radiographic purposes with good colour contrast for dissection. The injection was made at pressures varying from 100 to 200 mm.

    12 Rahaf Muwalla Rahaf Muwalla Mothana Mahes Mohammad Al-muhtaseb Posterior abdominal wall Structures of Post.Abdominal wall: • 5 lumbar vertebra & their intervertebral disc. • 12th ribs (floating ribs). • Upper part of bony pelvis (iliac crest). • Muscles: - psoas major inserted on lesser trochanter. - psoas minor in front of psoas major (usually absent). - Quadratus lumborum. - Iliacus which lies in the iliac fossa, inserted into lesser trochanter. • Aponeurosis of transversus abdominis muscles.(not mentioned). Muscles of posterior abdominal wall 1) Psoas major: • Origin: body (lateral side) & transverse process of lumbar vertebra & intervertebral disc. • Insertion: Lesser trochanter of femur (iliopsoas insertion). • Nerve supply: Nerve plexus (T12 subcoastal nerve, L1, L2, L3). • Action: Flexion of hip & thigh. • It Meets with iliacus muscle and they are inserted together (iliopsoas insertion). 2) Iliacus muscle: • Origin: Iliac fossa. • Insertion: Lesser trochanter of femur (iliopsoas insertion). • Nerve supply: Femoral nerve. • Action: Lateral flexion of hip & thigh for lying position. 3) Quadratus lumborum: • Origin: Iliolumbar ligament & iliac crest. • Insertion: 12th rib. • Nerve supply: Nerve plexus (T12 subcoastal nerve, L1, L2, L3). • Action: Fixation of 12th rib during inspiration & lateral flexion of the trunk. ❖Iliolumbar ligament The Iliolumbar ligament is a strong ligament passing from the transverse process of L5 to the posterior part third of the inner lip of the iliac crest. *It gives origin for quadratus lumborum muscle. ❖NOTE: The intercoastal nerves descend from the thorax to the abdomen, between transversus abdominis muscle and internal oblique muscle. We report multiple vascular variations in the left half of the pelvis of an adult male cadaver. Iliolumbar artery arose from the main trunk of the internal iliac artery. Posterior division of the internal iliac artery gave two lateral sacral arteries and a superior gluteal artery. The anterior division of the internal iliac artery gave origin to superior vesical, inferior vesical, inferior gluteal and internal pudendal arteries. The internal pudendal artery gave origin to a common trunk before leaving the pelvis. The common trunk divided into middle rectal artery and deep artery of the penis. The obturator artery took origin from the inferior epigastric artery and descended downward to the pelvis and left the pelvis by passing through the obturator foramen. Most of the other veins accompanying the arteries joined to form a plexus on the superior surface of the pelvic diaphragm. This plexus condensed to form anterior and posterior divisions of the internal iliac vein. Apart from this, the posterior part of the plexus drained directly into the common iliac vein through a large unnamed vein.
