REVIEWS Advances in immunosuppression for renal transplantation Antoine Durrbach, Helene Francois, Severine Beaudreuil, Antoine Jacquet and Bernard Charpentier Abstract | The development of immunosuppressants with minimal adverse and nephrotoxic effects is important to improve outcomes, such as acute and chronic antibody-mediated rejection, after organ transplantation. In addition, the application of expanded criteria for donors and transplantation in immunized patients necessitates the development of new therapies. Drug development over the past 10 years has generally been disappointing, but several new promising compounds have been or are being developed to prevent acute and chronic transplant rejection. In this Review, we report on several compounds that have been developed to remove allogenic T cells and/or to inhibit T-cell activation. We also discuss compounds that interfere with antibody-mediated rejection. Durrbach, A. et al. Nat. Rev. Email: [email protected] Received: 14 July 2015, Revised and Accepted: 24 August 2015 ABSTRACT Psoriasis is an autoimmune disease and further classed as a chronic inflammatory skin condition serving as a global burden. A moderate to severe psoriasis can be treated with conventional therapies. Less efficacy, poor patient compliance, and toxicity issues were the major problems associated with conventional therapies. The introduction of biologic therapy has a great impression on psoriatic treatment duration and enhanced quality of life in psoriasis patients. The new biologic therapies are tailor-made medications with the goal of more specific and effective treatment; less toxicity. The biologic therapy is aimed to target antigen presentation and co-stimulation, T-cell activation, and leukocyte adhesion; and pro-inflammatory cascade. They act as effective and safer substitute to traditional therapy. Secukinumab, certolizumab, itolizumab, golimumab, ustekinumab, adalimumab, infliximab etanercept, alefacept, etc. are the approved biologic with the global market. This review briefs about psoriasis pathogenesis, traditional treatments, and biologic therapies potential. Keywords: Psoriasis, Biologic, Non-biologic treatment. FAST FACTS AND CONCEPTS #93 CANNABINOIDS IN THE TREATMENT OF SYMPTOMS IN CANCER AND AIDS L Scott Wilner MD and Robert M Arnold MD Introduction The healing properties of cannabis have been asserted for centuries. Popular claims notwithstanding, there are no data to support the use of marijuana in the treatment of asthma, anxiety, depression, epilepsy, glaucoma, alcohol withdrawal, or infection; and limited data to support using cannabinoids as analgesics. Recent scientific studies of cannabinoids for symptom management have focused on nausea/vomiting and appetite stimulation. Terminology Cannabis sativa is the Indian hemp plant. Marijuana is a psychoactive substance derived from the plant. Cannabinoids are the biologically active compounds in the plant. THC is delta )-9 tetrahydrocannabinol, the major cannabinoid. Dronabinol is synthetic THC and the main ingredient in the Schedule 3 drug Marinol. Nabilone is an engineered THC analog that forms the basis of the Schedule 2 drug Cesamet. Pharmacology Cannabinoids act on cannabinoid receptors: the CB1 receptor in the CNS and on the CB2 receptor localized primarily to immune cells. Dronabinol and nabilone are well absorbed orally, but first pass metabolism and protein binding limit bioavailability. Dronabinol has a faster onset of action (~30 minutes), while nabilone has a longer duration of action (typically 8 – 12 hours, but potentially as long as 24 hours in some patients). Alternative delivery systems – including inhalers, suppositories, and transdermal patches – are being evaluated. Anti-emetic Use Dronabinol and nabilone are FDA approved for the treatment of nausea and vomiting associated with cancer chemotherapy in patients who have failed to respond to conventional antiemetics. -

Sung MD PhD* ABSTRACT Background Our objective was to determine whether, compared with control interventions, pharmacologic interventions reduce the severity of fatigue in patients with cancer or recipients of hematopoietic stem-cell transplantation (hsct). Methods For a systematic review, we searched medline, embase, the Cochrane Central Register of Controlled Trials, cinahl, and Psychinfo for randomized trials of systemic pharmacologic interventions for the management of fatigue in patients with cancer or recipients of hsct. Two authors independently identified studies and abstracted data. Methodologic quality was assessed using the Cochrane Risk of Bias tool. The primary outcome was fatigue severity measured using various fatigue scales. Data were synthesized using random-effects models. Results In the 117 included trials (19,819 patients), the pharmacologic agents used were erythropoietins (n = 31), stimulants (n = 19), l-carnitine (n = 6), corticosteroids (n = 5), antidepressants (n = 5), appetite stimulants (n = 3), and other agents (n = 48). Fatigue was significantly reduced with erythropoietin [standardized mean difference (smd): –0.52; 95% confidence interval (ci): –0.89 to –0.14] and with methylphenidate (smd: –0.36; 95% ci: –0.56 to –0.15); modafinil (or armodafinil) and corticosteroids were not effective. -

Health Products Regulatory Authority Summary of Product Characteristics 1 NAME OF THE MEDICINAL PRODUCT Audaval 0.1% Ointment 2 QUALITATIVE AND QUANTITATIVE COMPOSITION One gram of ointment contains 1 mg of betamethasone (0.1% w/w) as valerate. For a full list of excipients, see section 6.1. 3 PHARMACEUTICAL FORM Ointment Opaque ointment. 4 CLINICAL PARTICULARS 4.1 Therapeutic Indications Audaval preparations are indicated for the treatment of: eczema in children over 1 year elderly and adults; including atopic and discoid eczemas; prurigo nodularis; psoriasis (excluding widespread plaque psoriasis); neurodermatoses, including lichen simplex, lichen planus; seborrhoeic dermatitis; contact sensitivity reactions; discoid lupus erythematosus and they may be used as an adjunct to systemic steroid therapy in generalised erythroderma. In general, ointment preparations are particularly appropriate for dry, lichenified or scaly skin conditions whereas a cream preparation may be more suitable in the case of moist or weeping lesions. 4.2 Posology and method of administration For topical use only. If no improvement is seen after two to four weeks, the diagnosis should be reconsidered and specialist referral may be necessary. Adults, adolescents and the elderly A small quantity of Audaval should be applied to the affected area one to three times daily as directed by physician until improvement occurs. It may then be possible to maintain improvement by applying once a day, or even less often, or by using the appropriate ready diluted (1 in 4) preparation, Audaval RD 0.025% Ointment. Allow adequate time for absorption after each application before applying an emollient. If no improvement is seen within two to four weeks, reassessment of the diagnosis, or referral, may be necessary. -
