Further reproduction prohibited without permission. ABSTRACT The ADHD Rating Scale-IV is a screening instrument designed to assess attention-deficit/hyperactivity disorder in children. It is comprised of 18 items and has both a home and school version for parents and teachers to complete, respectively. A sample consisting of 178 parent- teacher dyads of children aged 6-12 years of age participated in the study. Psychometric investigation of these scales revealed high internal consistency, adequate test-retest reliability, and appropriate correlations with other measures of ADHD. Principal components factor analysis revealed two factors for both scales: (1) Inattention-hyperactivity and (2) Impulsivity- hyperactivity. Discriminant function analyses and base rate analyses revealed that these scales can correctly separate true ADHD children from normal controls but that it has significant difficulty differentiating other clinical populations. It was concluded that both scales have psychometric properties strong enough to be used as a screening measure. iv Reproduced with permission of the copyright owner. CHAPTER 1 INTRODUCTION Attention-Deficit Hyperactivity Disorder (ADHD) is a childhood disorder characterized by significant problems with attention span, impulse control, and motor activity. As one of the most common disorders of childhood, its prevalence has been estimated at three to five percent of school aged children and is diagnosed more often in males than females (Diagnostic and Statistical Manual of Mental Disorders (4th ed.) (DSM-IV); American Psychiatric Association, 1994). ADHD has also been shown to co-exist at high rates with other psychiatric disorders of childhood (Biederman, Newcorn, & Sprich, 1991; Jensen, Martin, and Cantwell, 1997) . However, the very nature of a disorder clearly indicates a categorical or medical approach that assumes a psychiatric disease identifiable by a set of discrete diagnostic criteria. Currently, many investigators support a dimensional or factorial approach to classification which employs the psychometric paradigm (Edelbrock & Costello, 1988; Steingard, Biederman, Doyle, & Sprich-Buckmisster, 1992) . ADHD has had and continues to have a historic evolution, both in terms of etiologies and diagnostic criteria. DuPaul (1992) outlined a specific strategy for conducting ADHD assessments whereby the ADHD Rating Scale (DuPaul, 1991) is utilized as a screening instrument. This instrument was based upon the diagnostic criteria for ADHD 1 Reproduced with permission of the copyright owner. Further reproduction prohibited without permission. according to the revised third edition of the DSM (American Psychiatric Association, 1987) . This screening instrument has since been updated to resemble the current criteria for ADHD as set forth by the recent fourth edition of the DSM. This scale is now referred to as the ADHD Rating Scale-IV (DuPaul, in preparation). Although this new instrument was expected to have similar psychometric properties to its predecessor, proper investigation of this instrument was warranted. Therefore, the purpose of this study was to psychometrically investigate this new scale. Exploration of the factor structure of this ADHD Rating Scale-IV was the first goal of this study. Its predecessor, the ADHD Rating Scale yielded two factors for both the home and school versions of the scale: inattention-hyperactivity and impulsiveness-hyperactivity. Likewise, it was hypothesized that this two factor structure would remain for this latest version of the scale. The next step in this investigation involved assessing the reliability of the scales.

Yeguez2 Abstract Objective: Diagnostic and Statistical Manual of Mental Disorders (5th ed.; DSM-5) A-criteria for ADHD were expanded to include new descriptors referencing adolescent and adult symptom manifestations. This study examines the effect of these changes on symptom endorsement in a sample of adolescents with ADHD (N = 259; age range = 10.72-16.70). Method: Parent ratings were collected and Diagnostic and Statistical Manual of Mental Disorders (4th ed., text rev.; DSM-IV-TR) and DSM-5 endorsement of ADHD symptoms were compared. Results: Under the DSM-5, there were significant increases in reported inattention, but not hyperactivity/impulsivity (H/I) symptoms, with specific elevations for certain symptoms. The average adolescent met criteria for less than one additional symptom under the DSM-5, but the correlation between ADHD symptoms and impairment was attenuated when using the DSM-5 items. Impulsivity items appeared to represent adolescent deficits better than hyperactivity items. Results were not moderated by demographic factors. Conclusion: In a sample of adolescents with well-diagnosed DSM-IV-TR ADHD, developmental symptom descriptors led parents to endorse slightly more symptoms of inattention, but this elevation is unlikely to be clinically meaningful. The standardization, reliability, and validity data were presented for the ADHD Rating Scale-IV Preschool Version. Parent and teacher ratings were collected on 902 and 977 children 3 to 5 years of age, respectively. Raw scores for the Inattention, Hyperactivity/Impulsivity and Total scores were converted to T-Scores and percentile ranks. Reliability coefficients ranged from 0.80 to 0.95 indicating good test–retest reliability. Concurrent validity with the Conners Teacher Rating Scales: Revised-Short and Conners Parent Rating Scale: Revised-Short ranged from 0.54 to 0.96. The ADHD-Rating Scale-IV appears to be a reliable, valid and user friendly measure for screening preschoolers with behavior problems.

This study aimed to analyze the contribution of early family, behavioral, and neuropsychological factors to long-term functional outcomes in young adults with ADHD. Family (parental mood disorders, family risk index, parenting stress, coherence), behavioral (oppositionism, inattention, emotional lability, hyperactivity), and cognitive factors were examined at baseline. Twelve years later, daily life functioning, quality of life, and executive and behavioral functioning were also evaluated in 61 young adults with a childhood diagnosis of ADHD. Results revealed significant correlations between most of the family and behavioral factors at baseline and the long-term functional outcomes.

Research indicates that some college students may be strongly motivated to feign AHDD symptoms for desired external incentives, such as stimulant medication or academic accommodations. To date, literature examining feigned ADHD has been primarily focused on ADHD specific self-report measures (e.g., CAARS) and continuous performance tests (e.g., CPTs); however, little attention has been devoted to the use of multi-scale inventories in detecting feigned ADHD. For CPT measures, virtually no literature exists on the effectiveness of the TOVA to identify feigned ADHD, despite its frequent clinical use for establishing this diagnosis. The current study utilized a between-subjects simulation design to validate feigning cut scores on ADHD-specific measures using 66 feigners and 51 confirmed ADHD cases. As prior literature suggested, the results convincingly demonstrated that face-valid ADHD assessment measures were easily faked. Across both TOVA modalities (e.g., Auditory and Visual), the ADHD simulators performed significantly poorer than those diagnosed with ADHD.

Power Baylor University Children’s Hospital of Philadelphia, Philadelphia, Pennsylvania, and University of Pennsylvania Comprehensive assessment of attention-deficit/hyperactivity disorder (ADHD) symptoms includes par- ent and teacher questionnaires. The ADHD Rating Scale–5 was developed to incorporate changes for the Diagnostic and Statistical Manual of Mental Disorders (5th ed.; DSM–5; American Psychiatric Asso- ciation, 2013). This study examined the fit of a correlated, 2-factor structure of ADHD (i.e., DSM–5 conceptual model) and alternative models; determined whether ADHD symptom ratings varied across teacher and child demographic characteristics; and presented normative data. Two samples were included: (a) 2,079 parents and guardians (1,131 female, 948 male) completed ADHD symptom ratings for children (N ϭ 2,079; 1,037 males, 1,042 females) between 5 and 17 years old (M ϭ 10.68; SD ϭ 3.75) and (b) 1,070 teachers (766 female, 304 male) completed ADHD symptom ratings for students (N ϭ 2,140; 1,070 males, 1,070 females) between 5 and 17 years old (M ϭ 11.53; SD ϭ 3.54) who attended kindergarten through 12th grade. The 2-factor structure was confirmed for both parent and teacher ratings and was invariant across child gender, age, informant, informant gender, and language.
