Our Housing & Community Services ministry features more than a dozen independent living senior apartment communities located throughout our service area. This ministry also provides its housing residents, as well as community members, with service coordination, which helps older adults access needed programs and services that help them continue to live independently in the setting of their choice. Our Partners 2 2018 Highlights Genacross at Home reaches one-year milestone In December 2018, Genacross at Home marked a year of providing skilled nursing care, therapy services, and medical social services to people in their homes. The home health ministry is flourishing as part of the continuum of care offered by Genacross Lutheran Services. During its first year, ministry operations and staffing were stabilized, a Medical Director was added, and the agency exceeded growth expectations. To this point, the majority of the referral and census growth has come from Genacross long-term care ministries, ensuring that Genacross is providing continued care to patients leaving its skilled nursing centers and returning home. The home health agency is certified to provide services in Fulton, Henry, Lucas, Wood, Ottawa, Sandusky, and Erie counties. Genacross has invested in an electronic medical record system called MatrixCare, an integrated clinical and financial software that improves clinical outcomes and maximizes the time of caregivers. Genacross clinical staff have spent numerous hours training on the new system, which has been successfully implemented on the campuses and integrated into the financial platform as well. Another technological change on the care campuses is the use of automated dispensing units (ADUs) for medications. Genacross has partnered with AlixaRx, a national pharmacy services company, to provide onsite pharmacy services. Previously, medications were ordered from off-site pharmacies and delivered later the same day or even the next day, while now the ADUs provide immediate access to a variety of medications. Each ADU dispenses prescription and over-the-counter medications in several different Nurse Racquel Johnson demonstrates the forms. The units cut down on the wait time for automated medicine dispensing unit at the Wolf Creek Campus. resident distribution and promote continuity of care that can result in better patient outcomes. The group is open to anyone, and the informational sessions help individuals learn how to interact and support loved ones, friends, and neighbors with dementia and other cognitive deficits. Since Luther Pines formed its Dementia Friends, several other Genacross housing sites have also begun hosting groups. Genacross is making a difference for people touched by dementia.
