Quirynen, David Herrera, Wim TeugHels & Mariano Sanz 1 ABSTRACT Peri-implant diseases are defined as inflammatory lesions of the surrounding peri-implant tissues, and they include peri-implant mucositis (inflammatory lesion limited to the surrounding mucosa of an implant) and peri-implantitis (inflammatory lesion of the mucosa, affecting the supporting bone with resulting loss of osseointegration). This review aims to describe the different approacHes to manage both entities and to critically evaluate the available evidence on their efficacy. THerapy of peri-implant mucositis and non-surgical therapy of peri-implantitis usually involve the mecHanical debridement of the implant surface by means of curettes, ultrasonic devices, air-abrasive devices or lasers, with or without the adjunctive use of local antibiotics or antiseptics. THe efficacy of these therapies Has been demonstrated for mucositis. Controlled clinical trials sHow an improvement in clinical parameters, especially in bleeding on probing. For peri-implantitis, the results are limited, especially in terms of probing pocket depth reduction. Surgical therapy of peri-implantitis is indicated wHen non-surgical therapy fails to control the inflammatory cHanges. -

We searched relevant electronic databases for eligible randomized controlled trials. Two review authors independently screened, extracted data, and assessed risk of bias. We conducted meta-analysis, assessed heterogeneity, and assessed certainty of evidence using GRADEPro software. We included 19 studies (n = 1350 participants), representing 18 randomized controlled trials and found very little or no impact of antibiotics on inflammatory biomarkers. A meta-analysis of eight studies demonstrated a mean reduction of 0.26 mm in the periodontal pockets at three months (mean difference [MD] 0.26, 95%CI: 0.36 to 0.17, n = 372 participants, moderate certainty of evidence) in favor of the − − − antibiotics. -

Guided tissue regeneration Methods: This study included 48 one- or two-wall infrabony defects from 24 patients (age: 30–65 years) seeking treatment for chronic periodontitis. Defects were randomly divided into two groups and were treated with a combination of DFDBA and barrier membrane, either alone (combined treatment group) or with local doxycycline (combined treatment + doxycycline group). At baseline (before surgery) and 3 and 6 months after surgery, the pocket probing depth (PPD), clinical attachment level (CAL), radiological bone fill (RBF), and alveolar height reduction (AHR) were recorded. Analysis of variance and the Newman–Keuls post hoc test were used for sta- tistical analysis. A two-tailed p-value of less than 0.05 was considered to be statistically significant. -

To address this issue, further research on developing new membranes with better separation characteristics, such as membrane modification, is needed. Many of the current innovative modified materials are still in the phase of in vitro and experimental studies. A collective review on new trends in membrane modification to GTR/GBR is needed due to the widespread use of polymeric membranes and the constant development in the field of dentistry. Therefore, the aim of this review was to present an overview of polymeric membrane modifications to the GTR/GBR reported in the literature. -

Marazita is professor and vice chair of the Department of Oral Biology in the University of Pittsburgh School of Dental Medicine and the Director of the Center for Craniofacial and Den- tal Genetics. With over 400 publications and almost 35 years of continuous NIH-funding, Dr. Marazita is a world leader in the use of statistical genetics and genetic epidemiology for understanding craniofacial birth defects and oral-facial development. In 1980, Dr. Marazita earned a Ph.D. in Genetics from the University of North Carolina, and in 1982, she completed post-doctoral train- ing in craniofacial biology at the University of Southern California. Before coming to Pittsburg, Dr. Marazita had faculty appointments at UCLA and the Medical College of Virginia. She is also a diplo- mate of the American Board of Medical Genetics and a Founding Fellow of the American College of Medical Genetics. At the University of Pittsburgh, Dr. Marazita has held numerous other appointments in the School of Dental Medicine, including assistant dean, associate dean for research, head of the Division of Mary L Marazita, Ph.D. Oral Biology, and chair of the Department of Oral Biology. Given her international reputation and commitment to the oral sci- ences, Dr. Marazita has held important roles in the National Institutes of Health (NIH), including the National Institute of Dental and Craniofacial Research (NIDCR), and the National Human Genome Research Institute (NHGRI). Clinical P RACTIC E Idiopathic Gingival Fibromatosis Associated with Generalized Aggressive Periodontitis: A Case Report Contact Author Rashi Chaturvedi, MDS, DNB Dr. Chaturvedi Email: rashichaturvedi@ yahoo.co.in ABSTRACT Idiopathic gingival fibromatosis, a benign, slow-growing proliferation of the gingival tissues, is genetically heterogeneous. This condition is usually part of a syndrome or, rarely, an isolated disorder. Aggressive periodontitis, another genetically transmitted disorder of the periodontium, typically results in severe, rapid destruction of the tooth- supporting apparatus. The increased susceptibility of the host population with aggressive periodontitis may be caused by the combined effects of multiple genes and their inter- action with various environmental factors. Functional abnormalities of neutrophils have also been implicated in the etiopathogenesis of aggressive periodontitis. We present a rare case of a nonsyndromic idiopathic gingival fibromatosis associated with general- ized aggressive periodontitis. We established the patient’s diagnosis through clinical and radiologic assessment, histopathologic findings and immunologic analysis of neutrophil function with a nitro-blue-tetrazolium reduction test. We describe an interdisciplinary approach to the treatment of the patient.
