Severe acute necrotic pancreatitis has a high mortality rate if left untreated, and always requires surgical intervention. The timing of surgical intervention is of importance. Here we present a case of a patient with severe necrotizing pancreatitis with possible gas producing bacteria in the retroperitoneum shown on imaging and cultures. Case Report The patient is a Seventy- two-year old male presenting to the emergency department with complaining of severe epigastric pain for the past 48 hours. The labs and clinical symptoms were consistent with pancreatitis. However, the imaging showed necrotic pancreatitis that required immediate intervention. During the course of six weeks, the patient underwent numerous surgical procedures to debride the necrotic pancreas. The patient was ultimately clinically stable to be discharged and transferred to a skilled-nursing facility, but returned 3 days later with a post- surgical wound infection vs. Conclusion The patient ultimately expired 7 days after his second admission to the hospital due to multi-organ failure secondary to sepsis. anastomotic leak with enterocutaneous fistula. On admission, his history was remarkable for four similar prior episodes over the previous five years that lasted between 3 and 5 days. He denied any constipation, obstipation or associated hematemesis, fevers, chills or urinary symptoms. During the first episode five years ago, he was evaluated at an outlying health centre and diagnosed with peptic ulcer disease and was managed with omeprazole intermittently . His past medical and surgical history was non contributory and he had no allergies and he denied alcohol intake or tobacco use. His HIV serostatus was negative approximately one year prior to presentation. On examination he was afebrile, with a heart rate of 120 (Fig 1B) beats/min, blood pressure 135/78 mmHg and respiratory rate of 22/min. Abdominal examination revealed mild distension with generalized guarding and marked rebound tenderness in the epigastrium.

Simultaneous existence of emphysematous cholecys- titis and acute pancreatitis is even rare from previous report. We described an elderly woman of em- physematous cholecystitis associated with cholelithiasis and acute pancreatitis presenting with a 3 days' duration of epigastric and right upper quadrant pain. Ultrasound and computed tomographic scans of the abdomen showed multiple stones and gas in the gallbladder and mild swelling of the pancreas with ascites. Antibiotics were given and percutaneous drainage of the gallbladder was performed. Subsequently, cholangiography via cholecystostomy was done and revealed no evidence of filling de- fect in the common bile duct. The patient was discharged on the 9th hospital day and the cholecys- tostomy tube was removed on the 7th day after discharge. Elective cholecystectomy was advised, but refused. There was no recurrence of abdominal pain after 6 months' follow-up. This diagnosis has been made based on your clinical history (what you have told us about your symptoms) and blood tests. You may also have had other tests that have helped us to make this diagnosis. For the vast majority of people, acute pancreatitis is a condition which resolves completely after two to three days with no long-term effects. However, for some people (and it may be too early yet to tell in your case) a more severe form of the disease develops called severe acute pancreatitis (SAP). About the pancreas The pancreas is a spongy, leaf-shaped gland, approximately six inches long by two inches wide, located in the back of your abdomen. The body lies behind your stomach, and the tail lies next to your spleen.
