INFORMATION ABOUT MEDICAL EXAMINATION FOR IMMIGRANT VISA APPLICANTS

All immigrant visa applicants have to undergo a standard medical examination at the assigned clinic. The U.S. Embassy’s panel physician is the Tashkent International Medical Clinic (TIMC), located at #38, Sarikulskaya Str., Tashkent, Uzbekistan (map to the clinic is attached).

To schedule an appointment for your medical examination you need to contact the clinic at least 2 weeks prior to your immigrant visa interview date, which is indicated on your appointment letter. Contact the Tashkent International Medical Clinic at +998 71 291 0142 or 291 0726 for an appointment as soon as you receive these instructions.

What to bring to the medical examination: - your immigrant visa interview appointment letter; - your valid passport; - for children under 16 you are required to obtain a passport for minors under 16 or in the cases where the parent is also immigrating, they can obtain an exit stamp in the travelling parent’s passports; this must be issued and stamped by the Department of Entry, Exit and Citizenship of the Ministry of Internal Affairs of Uzbekistan - one color picture (3X4 cm); - your vaccination record (not applicable if you are a K visa applicant); - your medical examination fee (payable in U.S. dollars according to the schedule of fees listed below)

All medical examinations include: Physical examination, serologic test for Syphilis (not required for persons under 15 unless suspected), full size chest radiograph (not required for persons under 15 years of age nor for pregnant women, unless such persons are suspected of having tuberculosis), bacteriological examination of Sputum (when required) and Immunization.

Schedule of fees for medical examination:

The basic examination fee for each applicant 15 years of age and older is USD 175 in the appropriate currency (including x-ray, serologic test for Syphilis) and USD 80 in the appropriate currency for applicants under the age of 15. However, if a chest x-ray or serologic test for syphilis is required then the fee may exceed the basic examination fee based upon the table of fees below.

For each applicant 15 years of age and older, the medical examination shall include the following:

• Physical examination and documentation US$120 • Serologic test for syphilis US$15 • Chest x-ray and radiology consult US$40 • Bacteriological exam of sputum US$15 (when needed)

Supplemental charges will also be approved for vaccination as required:

K1/K2 and K3/K4 visa applicant must undergo the standard IV medical examination; however, they are not required to submit proof of vaccinations until they adjust status with USCIS.

If you are requested to pay more than the amounts listed above, please contact the Consular Section of the U.S. Embassy in Tashkent immediately.

Results of the medical examination:

The results of the standard medical examination will be given to you in a sealed envelope. DO NOT OPEN THE ENVELOPE! You need to bring it to your immigrant visa interview. Only the Consular Section can open this envelope.
