HJW led the quantitative effectiveness synthesis and compiled the overall systematic review report. BS led the qualitative synthesis of barriers and enablers. JH, BS, MV and HJW conducted the search, using EndNote reference management software. Decisions on inclusion for impact evaluation studies were made by JH and HJW, with conflicts resolved through discussion and consensus.
