    Terminal Lucidity in People with Mental Illness and Other Mental Disability: An Overview and Implications for Possible Explanatory Models Michael Nahm, Ph.D. Freiburg, Germany ABSTRACT: The literature concerned with experiences of the dying contains numerous accounts reporting the sudden return of mental clarity shortly before death. These experiences can be described as Terminal Lucidity (TL). The most peculiar cases concern patients suffering from mental disability including mental illness or dementia. Despite the potential relevance of TL for developing new forms of therapies and for elaborating an improved under- standing of the nature of human consciousness, very little has been published on this subject. In this paper I present a historical overview and selected case reports of TL of mentally ill or otherwise disabled patients, mainly drawing on the literature available in English and in German. However, this peculiar phenomenon has received little attention in the recent decades. In previous publications, we have presented overviews of cases of terminal lucidity in various neurologic and psychiatric disorders. In this article, we highlight terminal lucidity in persons with mental disabilities, and focus on one of the most remarkable cases that have been reported. We provide biographical background information about the two principal witnesses who reported this case. It concerns the death of Anna Katharina Ehmer, a 26-year-old woman with severe mental disabilities who lived in an institution for people with mental disorders, and who had allegedly never spoken a single word during her life. Yet, she was reported to have sung dying songs for a half hour before she died. The case was reported by the head of this institution and by its chief physician. We consider it difficult to evaluate the authenticity of the case definitively in retrospect. Nevertheless, there are similar cases and a variety of other anomalous brain-related findings we consider worth investigating. Studies into such anomalous cases might improve our concepts of human brain functioning and of mental processing in persons with mental disabilities, and might be of special value for the dying, the bereaved, and caretakers.

We conducted a survey of caregivers who had witnessed at least one case of paradoxical lucidity in the year prior to survey completion. We assessed diagnosis and degree of preexisting cognitive impairment, cognitive state during the lucid episode, and temporal proximity of the lucid episode to death. Detailed case reports of 124 dementia patients who experienced an episode of paradoxical lucidity were received. In more than 80% of these cases, complete remission with return of memory, orientation, and responsive verbal ability was reported by observers of the lucid episode. The majority of patients died within hours to days after the episode. Further prospective study is warranted, as paradoxical lucidity suggests that there may exist a reversible and functional aspect of pathophysiology in severe dementia.     LETTER TO THE EDITOR Some Basic Problems with the Term “Near-Death Experience” To the Editor: The term near-death experience (NDE) has several semantic crite- ria for its application. Some of them are experiential, such as an expe- rience of a tunnel and/or an out-of-body experience (OBE). Other crite- ria for the term concern its connection with clinical death, as noted in the term itself. There are problems in regard to both types of criteria. In regard to the experiential criteria for NDE, the meaning of the term is unclear as to how many of the criteria must be satisfied. The meaning is also indeterminate as to which experiences are most im- portant, either because they occur more frequently or because they are at the core of the experience. Most of the criteria seldom appear; for example, an experience of scenes occurs in only 15% of cases (Fenwick & Fenwick, 1997). In ad- dition, the experiential criteria may be present without an NDE and, as a matter of fact, occur more often with other causes such as drug abuse (Corazza & Schifano, 2010), anaesthesiology, remedies, certain personality traits, or diseases of the brain. Also in regard to the ex- periential criteria, it is unclear whether the experiences survivors re- ported recalling from their resuscitations were actually generated at clinical death or later during the convalescence of brain function. In regard to the criterion that NDEs should be connected with clini- cal death, there are a number of problems. As just mentioned above, the experiential criteria may be present without linkage to clinical death, and moreover, clinical death and resuscitation occur most of- ten without such experiences.

Kaczyńskiego w Lublinie Abstract Terminal lucidity is a term used in the medical literature to determine the improvement of mental functioning shortly before death, even among patients with serious and long-term disorders. In 19th century, cases of mind clarity recovery shortly before death, were often recognized by doctors and interpreted as a sign of an impending death. In 20th century, the interest in this phenomenon decreased and then, virtually disappear. In recent years, on the wave of publications concerning near death experiences and related events such as the end of life experiences, papers about the improvement of mental functioning shortly before death, exponentially grew and got a new name, terminal lucidity. In this paper, an overview of the available literature is presented to outline the historical, phe- nomenological and clinical picture of this phenomenon and its possible implications for medical care and future studies. Streszczenie Terminalna jasność umysłu to termin stosowany w literaturze medycznej do określenia poprawy stanu umysłowego na krótko przed śmiercią, nawet u osób z wieloletnimi zaburzeniami w obrębie funkcjonowania poznawczego. W XIX w. przypadki odzyskania wcześniej zmąconego umysłu na krótko przed śmiercią były często rozpoznane przez lekarzy i interpretowane jako znak zbliżającej się śmierci. W XX w. zainteresowanie tym zjawiskiem spadło, aby praktycznie zniknąć. W ostatnich latach, na fali publikacji dotyczących doświadczeń śmierci klinicznej (near death experiences) i pokrewnych jej zjawisk jak doświadczenia końca życia (end of life experien- ces), liczba prac gwałtownie wzrosła, a poprawa stanu umysłowego na krótko przed śmiercią uzyskała nową nazwę: terminalna jasność umysłu (terminal lucidity).
