David Miller BVSc Hons MMedVet [Med] Johannesburg Specialist veterinary Centre [email protected] INTRODUCTION Diabetes mellitus (DM) is a common endocrine disorder, these dogs have insulin deficiency, occurs with an incidence of between 1 in 100 to 1 in 500 dogs. This results in a decreased ability of cells to take up and utilize not only glucose, but also amino acids, fatty acids, and electrolytes The majority of dogs with diabetes mellitus will be successfully stabilised and will remain stable for long periods of time. The most appropriate therapy in dogs is the administration of insulin. We will discuss the roles of the various insulin preparations currently available and develop a logical approach to the initial and long-term management of diabetes. The key to successful management of the diabetic patient lies in close communication with the pet owner and prompt recognition and treatment of concurrent disorders. The most common cause for instability is failure(s) in the daily management of the patient. When investigating instability always start by checking the daily routine before embarking on a search for complex causes of instability. KEY FACTS 1. Insulin is still the mainstay of therapy in all dogs with diabetes mellitus. 2. Diet is an important part of diabetic management, especially in obese patients. 3. Oral hypoglycemics may be helpful in combination with insulin to improve glycaemic control. 4. Autoimmune disease, pancreatitis, and amyloidosis are the most common causes of diabetes in dogs and cats. Successful management of the diabetic patient involves many factors. -

Feline Diabetes: What You Need to Know Feline diabetes (diabetes mellitus) is a condition where the body is unable to produce enough insulin to balance blood sugar, or glucose, which is a main source of energy for cats, much like it is in humans. Left untreated through insulin treatment and/or diet changes, cats can become unwell. Risk factors for diabetes in cats include being older, neutered males, obese, and physically inactive; poor or inadequate diet can significantly worsen the condition. With proper nutrition and medical management, including regular check-ups, your foster cat or forever friend cat can live a healthy, happy, normal life. Signs of Diabetes • Weight loss. To get the energy it needs, the body turns to other sources, like breaking down the body’s protein and fats, in order to feed glucose-starved cells. Despite an increased appetite, this can result in sometimes severe weight loss. • Excessive thirst and urination. High levels of glucose in the blood can cause the body to excrete excessive amounts of glucose in the urine. This can lead to increased urinary water loss, which can lead to dehydration, and a compensatory increase in thirst. Urine in diabetic cats can often be pungent. • Physical changes. The cat’s eyes may look sunken and blood shot; if diabetes is left untreated it can cause vision loss and even blindness. Cats with prolonged and untreated diabetes may experience damage to the nerves in the hind limbs, which can cause weakness and walking flat on their hind legs, and some cases lead to paralysis. Managing Diabetes Insulin therapy and dietary management are the two key treatments for feline diabetes. -

Caring for your diabetic dog Diabetes mellitus is a common problem in both dogs and cats. Caring for a diabetic animal requires some effort, but most pets remain happy and comfortable. Successful patient management requires a team effort between you and your veterinarian. This information is intended as a guideline only; always follow any specific instructions from your veterinarian and always ask if you have questions. What is diabetes mellitus? Diabetes mellitus is a condition in which the body either doesn’t properly produce or use insulin. Insulin is necessary for the movement of glucose from the blood into the cells of the body, so patients with diabetes have high blood sugar levels (also referred to as glucose concentrations). What are the signs of diabetes? Excessive thirst, frequent urination, increased hunger, sudden weight loss and weakness or fatigue are seen in dogs with diabetes. These changes can be minimized when the blood sugar levels are under control. Why is my dog a diabetic? Most likely, dogs become diabetic because of genetic factors, combined with environmental triggers. Many dogs with diabetes are overweight, or were previously, and this may also play a role. Is there a cure for diabetes? No. With very rare exceptions, diabetes in dogs is a permanent condition and life-long therapy is necessary. Even though it’s a multifactorial disease, obesity was found to be the most important risk factor in developing diabetes, obesity increases the chances of developing this disease to up to 4 times. The study was conducted on a number of 9 cats with uncomplicated diabetes with the purpose of monitoring the effects of the commercial dry food (with a high percentage of carbohydrates) and the commercial canned food (with a higher percentage of proteins and a lower percentage of carbohydrates) on the glycemic index. For this study, we considered the occasional administration of raw chicken and turkey meat because it wasn’t given for a long enough period of time. Diabetes in Dogs Affected Animals: Diabetes can occur at any age, but it is most likely to begin at seven to nine years of age. Female dogs are twice as likely to be affected by the diabetes, compared to females. Diabetes is more common in a number of dog breeds, including Keeshond, Pulik, Cairn terrier, miniature pinscher, dachshund, miniature schnauzer, poodle, and beagle. However, any dog can develop diabetes mellitus, or “sugar diabetes.” Overview: Why is insulin so important? The role of insulin is much like that of a gatekeeper: it stands at the surface of body cells and opens the door, allowing glucose to leave the blood stream and pass inside the cells. Glucose is a vital substance that provides much of the energy needed for life, and it must work inside the cells. Without an adequate amount of insulin, glucose is unable to get into the cells. It accumulates in the blood, setting in motion a series of events that can ultimately prove fatal. When insulin is deficient, the cells become starved for a source of energy. In response to this, the body starts breaking down stores of fat and protein to use as alternative energy sources. As a consequence, the dog eats more; thus, we have weight loss in a dog with a ravenous appetite. The body tries to eliminate the excess glucose by excreting it in the urine. However, glucose (blood sugar) attracts water resulting in the production of a large amount of urine. To avoid dehydration, the dog drinks more and more water. Characterized by high concentrations of sugar (glucose) in the blood and urine, diabetes mellitus is one of the more common hormonal disorders of the dog, and the disease almost always requires lifelong insulin treatment. -

Diabetes mellitus is a disease of the pancreas. This is a small but vital organ located near the stomach. It has two significant populations of cells. One group of cells produces the enzymes necessary for proper digestion. The other group, called beta-cells, produces the hormone insulin. Insulin regulates the level of glucose in the bloodstream and controls the delivery of glucose to the tissues of the body. In simple terms, diabetes mellitus is caused the failure of the pancreas to regulate blood sugar. The clinical signs seen in diabetes mellitus are related to the elevated concentrations of blood glucose and the inability of the body to use glucose as an energy source. What are the clinical signs of diabetes and why do they occur? The four main symptoms of uncomplicated diabetes mellitus are increased thirst, increased urination, weight loss and increased appetite. Glucose is a vital substance that provides much of the energy needed by cells, and it must work inside the cells. Insulin attaches to 'receptors' on the surface of body cells and opens "pores" through the cell wall that allow glucose to leave the bloodstream and enter the cell's interior. Studies in different veterinary hospitals of United States suggest increase in cases of canine diabetes and decrease in case fatality rate over time. Insulin is a hormone needed by the body to transport glucose (sugar) out of the bloodstream and into energy producing cells. Glucose is the fuel for cells of the body and is needed for normal body functioning. Lack of an energy source by cells of the body tells the brain that more energy source is needed and the animal then has an increased appetite. Therefore, a common clinical sign of diabetes is a hearty appetite with weight loss rather than weight gain. Another clinical sign that may be seen includes increased drinking and urinations due to result of excess glucose in the urine. Other clinical signs that may be identified include the following: -Sudden blindness due to the development of cataracts -Abnormal gait or weakness Some patients can become severely weakened, depressed, and dehydrated. They may have vomiting and a loss of appetite. These patients are most likely experiencing what is termed diabetic ketoacidosis (DKA). This is a serious condition that requires immediate medical attention since it can be life threatening. Possible causes of the development of diabetes include a genetic predisposition, chronic pancreatitis, obesity, hormonal abnormalities, endocrine disease (including Cushing’s disease), infections, and certain medications (including corticosteroids). Your veterinarian may recommend some of these common diagnostic tests to rule out diabetes mellitus: -Thorough history and physical examination -Blood tests: Complete blood count, chemistry panel and an evaluation of the urine -Abdominal x-rays or ultrasound Treatment of diabetes in dogs is life long and requires twice daily insulin injection.
