Forord Vista Analyse, i samarbeid med EY, har på oppdrag fra Helse- og omsorgsdepartementet og Finansdepartementet utarbeidet et kunnskapsgrunnlag og vurdert mulige tiltak som kan bidra til å begrense utgiftsveksten til legemidler under folketrygden fremover (blåreseptordningen). Arbeidet er gjennomført i nært samarbeid med oppdragsgivers prosjektgruppe bestående av Carolin Hagen, Helse- og omsorgsdepartementet (HOD), Vibeke Riekeles/Hanne Han- son, Finansdepartementet (FIN), Hallstein Husbyn, Legemiddelverket (SLV) og Harald Lislevand, Helsedirektoratet (Hdir). Gruppen har vært supplert med Jan Berg (HOD) og Mari Bugge Solheim (FIN). Carolin Hagen har ledet pro- sjektgruppen og vært konsulentgruppens hovedkontakt. Oppdragsgivers styringsgruppe bestående av Ida Ringdal (FIN), Espen Aasen (HOD), Karen Marie Ulshagen (SLV) og Sissel Husøy (Hdir), har hatt det overordnede ansvaret for områdegjennomgangen fra oppdragsgiversiden, og lagt gode og klare rammer for gjennomføringen. ATC ATC-systemet er et internasjonalt system for klassifisering av legemidler. ATC-kodene inndeles i fem nivå: et anatomisk, to terapeutiske og to kjemiske. Et spesifikt virkestoff er på laveste nivå. Ethvert virkestoff blir tildelt en kode etter hvilket anatomisk organ legemidlet virker på, og hvilken terapeutisk virkning det har. Begrenset bytte Enkelte legemidler føres opp på byttelisten med begrenset bytte. Det innebærer at legemidlene kan byttes på apotek ved oppstart av behandling, mens ved senere bytte må ansvarlig rekvirent gi tillatelse til bytte. Byttbare legemidler Medisinsk likeverdige legemidler som står på byttelisten, og som kan byttes på apoteket. Bytteliste Offisiell norsk liste over legemidler som anses for å være medisinsk likeverdige, og som kan byttes på apotekene. Statens legemiddelverk vurderer og avgjør hvilke legemidler som skal settes på byttelisten. DDD Definert døgndose (måleenhet). Dokumentbeskyttelse Bestemmelse i legemiddelregelverket som hindrer generikaprodusenter å sende inn søknad om markedsføringstillatelse før originalen har vært på markedet i åtte år i EØS- området. Dokumentbeskyttelse gir en beskyttelse utover patentrettighetene. Generisk legemiddel / ge- Et legemiddel som inneholder samme virkestoff som originallegemidlet og har samme nerika opptak og virkning. Det kan inneholde andre hjelpestoffer (de stoffene som brukes for å lage legemidlene) enn originallegemidlet. GIP Grossistenes innkjøpspris. Den prisen grossistene kjøper produktet av legemiddelprodusentene for. Maksimalpris Alle reseptbelagte legemidler i Norge må ha en maksimalpris fastsatt av Statens legemiddelverk. Markedsføringstillatelse Rett til å selge og markedsføre et godkjent legemiddel i Norge. Markedsføringstillatelse medfører ikke plikt til å selge legemidlet i Norge. Medisinsk likeverdige le- Legemidler som inneholder samme virkestoff, samme dose, tilnærmet samme gemidler legemiddelform og har lik virkning på pasienten, og det vurderes som trygt å sette legemidlet på byttelisten når en tar hensyn til forhold som sykdom/pasientgruppe, fa- ren for alvorlige problemer ved uriktig bruk, behov for spesielt utstyr eller om små for- skjeller i opptak hos den enkelte pasient kan tenkes å medføre problemer. Originallegemiddel Det opprinnelige legemidlet som i en periode har vært patent- og/eller konkurransebeskyttet. Parallellimportert lege- Legemiddel med samme virkestoff og samme opphav, men som importeres via et middel tredjeland av en uavhengig importør. Parallellimporterte legemidler anses å ha samme virkning som de direkteimporterte legemidlene, selv om det kan være forskjeller i utseende og hjelpestoffer. Patentert legemiddel Nye virkestoff innvilges patentbeskyttelse for 20 år. På grunn av tiden det tar å utvikle et legemiddel, gir dette normalt mellom 8 og 12 års beskyttelse etter at legemidlet har kommet på markedet. I disse årene kan legemidler med det beskyttede virkestoffet ikke selges av andre. Trinnpris Reduksjon av refusjonsprisen for legemidler som utløses når det er etablert konkur- ranse mellom to eller flere likeverdige legemidler på det norske markedet. Virkestoff Den kjemiske eller biologiske forbindelsen som gir den medisinske virkningen i et fer- digfremstilt legemiddel. The advice is summarised as follows ADVICE: following a full submission inclisiran (Leqvio®) is accepted for use restricted use within NHSScotland. Indication under review: for adults with primary hypercholesterolaemia (heterozygous familial and non-familial) or mixed dyslipidaemia, as an adjunct to diet: • in combination with a statin or statin with other lipid lowering therapies in patients who are unable to reach LDL-C goals with the maximum tolerated dose of a statin, or • alone or in combination with other lipid lowering therapies in patients who are statin intolerant, or for whom a statin is contraindicated. Conventional treatments generally induce therapeutic effects that are transient because they target proteins rather than underlying causes. In contrast, nucleic acid therapeutics can achieve long-lasting or even curative effects via gene inhibition, addition, replacement or editing. Their clinical translation, however, depends on delivery technologies that improve stability, facilitate internalization and increase target affinity. Here, we review four platform technologies that have enabled the clinical translation of nucleic acid therapeutics: antisense oligonucleotides, ligand-modified small interfering RNA conjugates, lipid nanoparticles and adeno-associated virus vectors. For each platform, we discuss the current state-of-the-art clinical approaches, explain the rationale behind its development, highlight technological aspects that facilitated clinical trans- lation and provide an example of a clinically relevant genetic drug. Statin therapy has been the traditional treatment for LDL-C reduction, in addition to lifestyle modifcations, but studies have shown that a substantial proportion of patients does not reach target LDL-C goals despite receiving maximally tolerated statin medications. Additionally, statin therapy is associated with a few shortcomings as many patients initiated on these medications discontinue treatment within 1 year because of lack of tolerability. Furthermore, guidelines from both the American College of Cardiology and the American Heart Association highlight the importance of obtaining LDL-C goals because of the residual atherosclerotic CVD risk that remains in high-risk populations. That the residual cardiovascular risk remains despite statin therapy highlights the impor- tance of evaluating therapeutic approaches that possess efective lipid lowering that can be used adjunctively with statins. Much focus has been directed towards the proprotein convertase subtilisin/kexin type 9 (PCSK9) pathway, leading to the development of evolocumab and alirocumab, two human monoclonal antibodies directed against PCSK9. These agents have been shown to markedly decrease LDL-C levels and signifcantly reduce cardiovascular risk, but the need for biweekly or monthly subcutaneous injections has generated concerns for patient compliance.
