Malaysia Situation summary  As of 6 December 2020, there have been a total of 72 694 confirmed cases of COVID-19 in Malaysia.  In the past week, 8209 new cases have been reported, a 15.5% decrease of cases compared to the previous week. Of the new cases reported, only 32 (0.4%) were imported.  In the past week, 25 new deaths have been reported, bringing the total number of COVID-19 deaths in Malaysia to 382. Upcoming events and priorities  The National Disaster Management Agency (NADMA) is prepared to re-activate the low-risk COVID- 19 quarantine and treatment centre at the Malaysia Agro Exposition Park Serdang (MAEPS) if needed. The opening of the centre will be subject to the decision of the Ministry of Health (MOH).  The MOH has rejected the proposal for a green work bubble to allow factory workers who have tested negative and are still undergoing their 14-day quarantine to return to work early.  Authorities are currently reviewing data to evaluate the need to shorten mandatory quarantine periods for international travellers into Malaysia.  state government has established three initiatives to curb the spread of COVID-19 in the state. It includes: o Isolating International Airport (KLIA) cargo workers. o Identifying five locations to open low-risk COVID-19 quarantine and treatment centres. o Increasing coordination with district health officers to expedite foreign workers’ screenings.  The MOH has spent an estimated total of RM 1.17 billion on the nation’s fight against COVID-19. The funds have been used for the procurement of devices, medical supplies, consumables, personal protective equipment (PPE), test kits and reagents, as well as other services. National transmission assessment

At the national level, Malaysia remains in Stage 2, localized community transmission. The total number of cases reported decreased by about 15% from 9711 cases in the previous week to 8209 new cases reported in the past seven days. Of the new cases, 0.4% were imported cases and 99.6% were locally acquired cases. Selangor and Sabah continue to be the two states with the greatest number of new cases reported. In the week ending 6 December 2020, Selangor reported 2879 cases (35.1% of total) and Sabah reported 2093 new cases (25.5% of total). While most cases are linked to existing clusters, 1517 (18.5%) cases were reported as unlinked at the time of reporting. Of these, 577 and 516 were reported in Sabah and Selangor, respectively. The number of cases reported in other states is also on the rise with an increase in geographical spread, especially in FT Kuala Lumpur with 981 cases (12.0% of total), with 684 cases (8.3% of total) and with 562 cases (6.8% in total) reported. The previously set state-level conditional movement control order ends this week on 6 December 2020, and further extensions have been detailed for selected locations. Testing rates have been increased to an average of 19 533 tests per day with a test positivity rate of 6.0%. There was an increase in the seven-day average for daily number of cases requiring critical care (ICU), 122 this week compared to 113 last week, and an increase in the seven-day average for daily number of cases needing ventilator support, 49 this week compared to 45 last week. The proportion of fatal cases was 0.30% (25/8209) this week compared to 0.23% (22/9711) last week.

 The MOH reported that as of 30 November, 35.7% of all cases recorded in Malaysia have been unlinked community infections.  In the past week, 25 new deaths were reported in Malaysia. Of these, 10 (40%) were recorded in Sabah. Nine (36%) cases were reported in women, and three (12%) cases were reported in non- Malaysians.

Sabah  Between 1 October and 1 December, Sabah accounted for 71.7% (27 043/37 728) of cases in Malaysia. o 20.3% (5481) of cases were among foreigners. o 56.7% (15 343) of cases were among men. o 21.9% (5919) of cases were among children aged 18 years and below. o 12.9% (3478) of cases were among children aged 12 years and below. o 0.89% (234) of cases were among infants aged 1 year and below.

Clusters  Of the 388 clusters identified in Malaysia, 187 are currently active. In the past week, 29 new clusters were identified, and 25 clusters were declared closed.  The Teratai cluster in Selangore is the largest cluster to date, with 5178 positive cases reported, out of a total of 6637 screened. As of 2 December, the Teratai cluster has resulted in second generation of infections. o A total of 152 COVID-19 cases linked to the cluster have been reported within the community outside of the enhanced movement control order (EMCO) area. o The second generation of cases involve the transmission of infection from stay-at-home local factory workers to their family members.

Strategic approach

Non-pharmaceutical interventions (NPI)  Following the extension of several conditional movement control orders (CMCOs) across Malaysia, the government has announced several new directives: o Inter-district and inter-state travel is now permitted without police approval except for EMCO areas. o Buses, trains, ferries and passenger boats are permitted to operate at 50% capacity. o Police will no longer carry out roadblocks, effective 7 December onwards, with travel only restricted to EMCO areas. o Vehicles will no longer have a passenger limit.

 MOH authorities acknowledged the shortcomings in the work process related to COVID-19 quarantine release orders and have improved this situation.  Malaysian authorities have imposed restrictions on the entry of foreign nationals into Malaysia based on the scoring of a risk assessment matrix that determines the risk of COVID-19 transmission through importation. The matrix includes factors such as: o number of COVID-19 cases at country of origin o death rate per 1 000 000 people in the last 14 days o death toll o recovery index.  Sabah o Effective 7 December, Mount Kinabalu and other national parks will reopen in phases to public. The initial number of hikers will be limited to 50% of the original capacity. o Water activities such as scuba diving, snorkelling, fishing, sea walking, jet skiing, fun rides, water rafting and canoeing are permitted at 50% capacity. 5 of 3

o Classes involving study tuition, dance, arts, Quran recitation and language are permitted to operate at a maximum of five people in attendance at any one time. o Busking, musical and singing performances at closed locations within buildings are permitted with limited performance time and audience numbers. o Effective 7 December, 50% of civil servants will start to report for duty at their offices. o The movement of small boats from islands to the mainland is permitted from 06:00 to 18:00 daily.  Sarawak o Christmas parades and combined Christian services will not be held in Kuching and Miri this year. o Effective 4 December, all mosques and prayer halls in the state are allowed to hold the five daily congregational prayers after obtaining permission from the Sarawak Islamic Council. o Effective 7 December, all spas, wellness and reflexology centres are allowed to resume operations from 10:00 to 24:00 hrs daily.  Kelantan o Wedding ceremonies and other public gatherings are now banned in the district of Kota Baru. o All mosques and prayer halls at Madrasah Ad-Diniah Al Falahiah in Pasir Puteh, and Istana Negeri Kubang Kerian have been ordered to close.  Johor o A total of 55 mosques and prayer halls in Batu Pahat have been temporarily closed following a rise in COVID-19 cases.

Surveillance and testing  A total of 16 425 senior citizens and staff at elderly care homes across Malaysia have undergone COVID-19 screenings.  Selangor: targeted community screening was conducted among residents around specific locations in Meru, Klang on 2 December 2020.  The MOH has clarified that 100% of people in any area placed under EMCO will be tested for COVID- 19. Additionally, 5% of the population placed under targeted EMCO (TEMCO) areas will also be tested.

Health-care capacity  The MOH is considering the need to mobilize health-care workers previously deployed to Sabah back to Peninsular Malaysia following the rise in cases in the region. Risk communication and community engagement  Malaysian authorities have issued a warning to companies that have not been complying with standard operating procedures (SOPs) throughout the CMCO period and will thereby face immediate revocation of their operating permits and possible legal action.  The MOH has stated pandemic fatigue among the public as a contributing factor to the rise of COVID- 19 cases.  Kelantan: Flood evacuees who were placed in temporary relief centres and whose houses are located in COVID-19 cluster areas will not be permitted to return before sanitizing their houses.
