Patients’ care will often involves multiple providers and follow-up requires persistence by patients and clinicians alike, therefore ideal outcomes are often difficult to achieve. The need for better disease management policies and practice is growing. This is due to the changing demographic profile of the population, the increasing cost of managing people in acute care hospitals and the availability of new technologies and services. All these changes enable a different care paradigm which is more cost effective and provides people with chronic conditions an improved quality of life. Management of the NCDs therefore offers an excellent opportunity to practice chronic disease management - a systems approach designed to ensure excellent care. The NCD team has developed a comprehensive approach to chronic disease care. We would like to describe the NCD Program in Ampangan Health Clinic which represents many typical government health clinics in Malaysia and the processes by which it was developed. -

This would involve giving up three rich and well-developed areas—the State of Selangor (includin g Kuala Lumpur, capital of the Federated Malay States), the State of Negr i Sembilan, and the colony of Malacca—but he thought that Kuala Lumpu r could be held until at least the middle of January . He intended that the III Indian Corps should withdraw slowly to a line in Johore stretching from Batu Anam, north-west of Segamat, on the trunk road and railway , to Muar on the west coast, south of Malacca . It should then be respon- sible for the defence of western Johore, leaving the Australians in thei r role as defenders of eastern Johore. General Bennett, however, believing that he might soon be called upo n for assistance on the western front, had instituted on 19th December a series of reconnaissances along the line from Gemas to Muar . By 1st January a plan had formed in his mind to obtain the release of his 22nd Brigade from the Mersing-Jemaluang area and to use it to hold the enem y near Gemas while counter-attacks were made by his 27th Brigade on the Japanese flank and rear in the vicinity of Tampin, on the main road near the border of Malacca and Negri Sembilan . Although he realised tha t further coastal landings were possible, he thought of these in terms of small parties, and considered that the enemy would prefer to press forwar d as he was doing by the trunk road rather than attempt a major movement by coastal roads, despite the fact that the coastal route Malacca-Muar- Batu Pahat offered a short cut to Ayer Hitam, far to his rear . -

Keywords: N03 --N, land use, well-water. ABSTRAK Satu tinjaun umum mengenai aras N03 -- N dalam air perigi telah dlj'alankan di empat kawasan yang berlainan bent uk gunatanahnya. Empat kawasan yang dipilih ialah kawasan pertanian dengan temakan lembu, kawasan hutan, kawasan bandar dan kawasan pertanian sahaja. Sampel air diambil dari 22 perigi yang terdapat dalam kawasan tersebut dan kandungan N03 - -N dianalisakan. Hasil dari data yang didapati di lapangan dan ujian statistik menunjukkan aras N03 -- Nair perigi dari kawasan gunatanah yang berlainan kecuali kawasan pertanian + temakan, menunjukkan perbezaan yang ketara di mana kawasan pertanian mempunyai aras N03 -- N yang tertinggi diikuti oleh kawasan bandar dan hutan. ABSTRACT A general survey on N03 -- N levels in well-water under different landuses was conducted. Four areas were selected to represent different landuses namely mixedfarming (crop and animal production), forest, urban, and crop production only. Water samples from a total of 22 wells in the selected areas were collected and analysedfor N03-- N. The field and statistical data showed that the differences in N03 -- N levels in well- waterfrom different landuses were significant except for agriculture + animal farming. The highest N03 -- N level was observed in agricultural areas followed by urban and forested areas.
