Golder Associates Ltd. (Golder) was retained by the City of Ottawa (“City”) to complete a Phase One Environmental Site Assessment (“Phase One ESA”) of the property located at 159 Forward Avenue in Ottawa, Ontario (herein after referred to as the “Site” or “Phase One Property”) as shown on Figure 1. For reporting purposes, Site north has been defined such that Forward Avenue has a north-south axis. At the time of the Site visit, conducted on April 26, 2019, the Site consisted of a 0.42-acre parcel of land developed with one (1) building. The exterior areas were paved with the exception of a playground area and minor landscaped area with trees and planters. It is understood that this Phase One ESA is being carried out for due diligence purposes and that an RSC is not required at this time. The Phase One ESA was completed in accordance with Ontario Regulation (O. Reg. 153/04), as amended, and included a review of available current and historical information regarding the Site and surrounding properties, a Site reconnaissance, interviews, evaluation of readily available information, and reporting, subject to the limitations outlined in Section 9.0 of this report. The Site has been used for institutional and residential purposes since it was developed prior to 1902. As such, the Site is not considered an enhanced investigation property as defined by O. Reg. 153/04. Based on the information collected as part of this Phase One ESA, the following potentially contaminating activities (PCA) that may have resulted in an area of potential environmental concern (APEC) were identified: 28. Gasoline and Associated Products Storage in Fixed Tanks - The Site formerly had an on-Site UST. The UST was removed in 2002 and verification samples of excavation backfill and an excavation groundwater sample had detectable total petroleum hydrocarbons which may exceed current standards. 28. Gasoline and Associated Products Storage in Fixed Tanks – Fuel oil spill with confirmed soil contamination adjacent to the Site at 154 Hinchey Avenue. 30. Importation of Fill Material of Unknown Quality and Origin – 1.75 meters of fill containing brick and wood was documented in the UST remediation report. Based on the information collected as part of this Phase One ESA, three APECs were identified on the Site as follows: APEC 1- Potential for impacts in soil and/or groundwater in the vicinity of the former heating oil UST. The tank was located at the rear (east side) of the Site building and was removed in 2002. PHCs were measured in soil and groundwater at concentrations that could exceed current standards. APEC 1 is described by the former location of the tank and the immediate surrounding area. APEC 2 – 1.75 metres of fill was documented in the wall of the UST excavation undertaken in 2002. The fill was noted to contain brick and wood. The extent of the fill may be limited to the former UST area or extend Site wide as it was likely placed during the development of the Site outside the building. APEC 3 – Potential for soil and/or groundwater impacts in the vicinity of the eastern property line as a result of an off-site release of fuel oil. It is noted that the volume of fuel released is unknown and the location of the property is cross gradient to the Site thereby reducing the potential to have impacted the Site. ii May 2019 19120465 Given that APECs have been identified on the Site during the Phase One ESA, a Phase Two ESA would be required at the Site to investigate the potential for subsurface impacts associated with these APECs in order to obtain an RSC. There were no material deviations to the Phase One ESA requirements set out in O.Reg. 153/04 that would cause uncertainty or absence of information that would affect the validity of the Phase One
