I can confirm that the contents of your Regulation 28 Report have been shared with the relevant staff to enable us to provide you with a comprehensive response. In your report you highlight that your matters of concern were as follows: (1) Mr Nyeruke’s medical conditions were not made known to the Trust. In consequence, he had worked on wards where patients had infections involving multi-resistant organisms. Given his compromised immune state, this situation involved risk to both patients and Mr Nyeruke himself. In the absence of information concerning a particular staff member’s medical condition there is a risk of transmission of infections either to or from the staff member. (2) There is scant evidence as to whether Mr Nyeruke underwent appropriate training in respect of PPE such as masks before being permitted to work on a ward involving infectious diseases. The difficulties involved (where a support worker supplied by a nursing agency is only in the hospital for a brief period) are acknowledged. -

Our ref: 2387/21 Figures for incidents of drink spiking in your region over the last 5 years (year by year) I would appreciate it if the figures can be broken down to the nearest city/town. Can you also tell me the number of prosecutions there have been for the above offences and how many of those resulted in a conviction? Please see the attached document. West Yorkshire Police receive reports of crimes that have occurred following a victim having their drink spiked, crimes such as rape, sexual assault, violence with or without injury and theft. West Yorkshire Police take all offences seriously and will ensure that all reports are investigated. Specifically for victims of rape and serious sexual offences, depending on when the offence occurred, they would be offered an examination at our Sexual Assault Referral Centre, where forensic samples, including a blood sample for toxicology can be taken, with the victim’s consent, if within the timeframes and guidance from the Faculty for Forensic and Legal Medicine. West Yorkshire Police work with support agencies to ensure that all victims of crime are offered support through the criminal justice process, including specialist support such as from Independent Sexual Violence Advisors. We, the Local Government Boundary Commission for England, having carried out our initial review of the electoral arrangements for the metropolitan borough of Calderdale in accordance with the requirements of section 63 of, and Schedule 9 to, the Local Government Act 1972, present our proposals for the future electoral arrangements for that borough* 2. In accordance with the procedure laid down in section 60(1) and (2) of the 1972 Act, notice was given on 27 August 1975 that we were to undertake this review. Currently under construction in the city centre are the hotel on Greek Street, Education campus on Hunslet Road, Ruth Gorse Academy, City House’ Hilton Leeds Arena hotel, Merrion House, 6 Queen Street, 3 Sovereign Square, Victoria Gate, 5 Wellington Place, Central Square on Wellington Street. Outside the city centre are the incinerator on Pontefract Lane and UTC on Hunslet Road. Three city centre schemes over £100m each are proposed or currently on hold: Latitude, Caddick development at Quarry Hill, Temple Quarter and Wellington Place. -

Lesser Whitethroat 1987 – There was patchy distribution along the Aire, Calder and Wharfe Rivers. The earliest were two at Esholt Sewage Works on 29th April but the main arrival was in May with up to six seen at Elland Gravel Pits. Lesser Whithroats were also noted between May and August at: Buck Wood, Thackley; Fagley Beck; Knotford Nook; Heaton Woods; Priestley Green; and Lindley Wood Reservoir. A single with a mixed flock of Goldfinches and tits at Bingley Bog, Old Hills, on 12th September was the last record. 1988 - There was only a handful of records. The first was a male at Knotford Nook on 1st May with other singles at Otley Gravel Pits on 8th, Calverley Woods on 25th and on territory at Lindley Wood Reservoir from 31st until at least 5th June. The last was a male at Knotford Nook on 20th and 21st September. 1989 – A male at Otley Gravel Pits on 6th May was the first; it eventually left up the River Wharfe. Passage was evident between 15th and 21st May as several wandering males were encountered as follows: one at Wagon Lane, Bingley, was last seen on 18th, another at Otley Gravel Pits on 17th and singles at Black Carr Wood on 16th and 21st. Two August reports, possibly referring to birds of this species, were one at Lindley Wood Reservoir on 3rd and another at Shell Lane, Calverley, on 10th. The last were two at Poplars Farm and a single at Beckfoot Lane, Bingley, all on 24th September, the latter being a product of that day’s local ‘Bird Race’.
