Endosalpingiosis-type inclusions are observed within pelvic and periaortic lymph nodes, exclusively in wom- en, with or without concomitant disease of the female genital tract (1). Rare cases have been described in men (6). Although intranodal endosalpingiosis is a well-recognized entity, it may be confused with a metastatic carcinoma, especially in patients with known primary malignancies. It is thus important to be aware that benign intranodal inclusions may occur, and mimic metastasis. However, to our knowledge no such lesions have previously appeared in the Korean literature. This paper presents a case of endosalpingiosis in the pelvic lymph nodes of a 50-year-old woman, with a review of the relevant literature. CASE REPORT A 50-year-old woman was admitted, with a known endome- trial carcinoma, to the department of Gynecologic Oncology Service at our hospital. The included structures may be either glan- dular or nonglandular. Several types of epithelial differentiation have been des- cribed: squamous, renal tubular, colonic and Müllerian. The Müllerian-type of inclusions are much more common, ranging from 5 to 41%, based on lymph nodes sampled in women (1). The glands are lined by an admixture of ciliated, secretory and intercalated cells, with (endometriosis) or without (endosal- pingiosis) associated stroma (7). These inclusions may cause a diagnostic problem, with regard to whether they are benign and incidental, or if they are related to the tumor metastasis in patients with gynecological malignancies (8). The increased frequency of these lymph node inclusions, sampled for primary ovarian malignancies, suggested they were more likely the bland- appearing forms of metastatic tumor (9). However, much of the recent literature considers these inclusions as benign, incidental lesions. Features favoring benignity include: a cap- sular or interfollicular location of the glands, different types of lining cells, including ciliated forms, a lack of significant cellu- lar atypia and mitoses, periglandular basement membranes and the absence of stromal desmoplasia (1,7). Hoskins, MD Radical abdominal hysterectomy was developed in the late nineteenth cen- tury for the treatment of cancer of the cervix uteri and upper vagina. The procedure was originally described in 1895 by Clark and Reis,3°,39but Wertheim is credited for the development and improvement of the basic technique of radical hysterectomy. Wertheim reported extensively on his early results with this procedure and was a pioneer in describing approaches to reducing problems with hemorrhage, urinary tract injury, fistula formation, and sepsis3 Over the past century, the procedure underwent several refinements in indications, patient selection, and surgical technique. Moreover, the advancements in anesthesiology, antibiotics, and preoperative and postoperative medical care have resulted in a significant reduction in morbidity and mortality from this procedure. Several competitors to radical abdominal hysterectomy have emerged over the years. Radiotherapy, discovered around the time that radical hysterectomy was being developed, continues to be used for the treatment of patients with advanced-stage (International Federation of Gynecologic Oncology [FIGO] stages IIB-IV) and selected patients with early stage cervical cancer. Further- more, radical vaginal hysterectomy (Schauta-Amreich) has seen a recent revival since its early description, mainly because of the advancements in laparoscopic pelvic and paraaortic lymphadenectomy, which would accompany the vaginal resection of malignancy. In addition, laparoscopic radical hysterectomy and fertility-sparing radical transvaginal trachelectomy (i.e., removing the cervix and preserving the corpus) with laparoscopic pelvic lymphadenectomy are among the newest techniques to compete with the classic radical abdominal hysterec- tomy for the treatment of early cervical cancer.

Blair Memorial Research Fund) Malignant tumors with an intimate mixture of cellular com­ ponents from two germ layers, such as epithelium and mesothelium, in varying proportions, are of special interest because of their rarity, and because their tissues contain surprising and bizarre combinations of cell structures. Such tumors originating in the thyroid gland, in the mammary gland, and elsewhere, have been reported as carcinosarcomas (Mason and Wells; Ktichens; Saltykow). Wegelin's recent review of these tumors of the thyroid mentioned only a few, and the structure of these varied con­ siderably. Antedating Wegelin's review by several years was the report by Schuppisser in which he stated, as have others, that true carcinosarcomas of the thyroid are rare. He mentioned the two tumors reported by Forster, those reported by Kocher, Jr., Simmonds, Albrecht, and Schmorl, and one in a dog reported by Wells. These summarizing statements and those in the recent account by Mason and Wells indicate how few indeed are the reports of malignant mixed cell tumors of the thyroid. The variations in histologic structure seem to depend upon the degree of differentiation of the epithelial and mesothelial tissues and the proportion of each entering into the composition of the primary growth and its metastases. Some of the accounts report a fairly complete separation of the tissue derivatives in the primary growth, in the recurrence, or in the metastases, so that in these various places elements of only one of the two germ layers were found.
