Chapel Hill 2009 ___________________________ Advisor signature/printed name ________________________________ Second Reader Signature/printed name ________________________________ Date The Key to Increasing Breastfeeding Duration 2 Abstract Experts and scientists agree that human milk is the best nutrition for human babies, but are healthcare professionals (HCPs) seizing the opportunity to promote, protect, and support breastfeeding? Not only are HCPs influential to the breastfeeding dyad, they hold a responsibility to perform evidence-based interventions to lengthen the duration of breastfeeding due to the extensive health benefits for mother and baby. This paper examines current HCPs‘ education, practices, attitudes, and extraneous factors to surface any potential contributing factors that shed light on necessary actions. Recommendations to empower HCPs to provide consistent, evidence-based care for the breastfeeding dyad include: standardized curriculum in medical/nursing school, continued education for maternity and non-maternity settings, emphasis on skin-to-skin, enforcement of evidence-based policies, implementation of ‗Baby-Friendly USA‘ interventions, and development of peer support networks. Requisite resources such as lactation consultants as well as appropriate medication and breastfeeding clinical management references aid HCPs in providing best practices to increase breastfeeding duration. Introduction The link between breastfeeding duration and subsequent development, cognition, educational, mental, psychomotor and behavioural functioning of the infant has been the subject of much scientific enquiry. Indeed, the effect of feeding on infant health and development was first discussed more than half a century ago when breastfed babies were reported to have better cognitive outcomes in childhood than artificially fed babies (Hoefer and Hardy 1929). Some studies have found striking results pertaining to the relative advantages that breastfeeding can confer on child neurodevelopment (Oddy, Kendall et al. 2003; Vohr, Poindexter et al. 2006; Kramer, Aboud et al. 2008). Breastfeeding has previously been associated with improvements across neurodevelopmental domains for low birthweight babies in comparison with not breastfeeding at all (Vohr, Poindexter et al. 2006). There is strong evidence that kangaroo mother care (KMC) reduces mortality among babies with birth weight <2000 g (mostly preterm). KMC involves continuous skin-to-skin contact, breastfeeding support, and promotion of early hospital discharge with follow-up. The World Health Organization has endorsed KMC for stabilised newborns in health facilities in both high-income and low-resource settings. The objectives of this paper are to: (1) use a 12-country analysis to explore health system bottlenecks affecting the scale-up of KMC; (2) propose solutions to the most significant bottlenecks; and (3) outline priority actions for scale-up. Methods: The bottleneck analysis tool was applied in 12 countries in Africa and Asia as part of the Every Newborn Action Plan process. Country workshops involved technical experts to complete the survey tool, which is designed to synthesise and grade health system “bottlenecks”, factors that hinder the scale-up, of maternal-newborn intervention packages. -

Issues in maximizing neonatal health are examined using the Sustainable Development Goals developed by the United Nations as a framework. Recommendations: Interventions that can help optimize neonatal, infant, and child health in the future are reviewed, including increasing preventative healthcare (immunizations, malaria prevention, exclusive breastfeeding for the fi rst 6 months of life), enhancing point-of-care interventions (including umbilical cord care, antenatal corticosteroids if preterm birth is anticipated, and antibiotic therapy), enhancing nutritional interventions (to decrease diarrheal diseases and decrease wasting, stunting, and underweight), and building systems capacity. Clinical Implications: In an increasingly global world where wars, climate change, civil unrest, and economic uncertainty all infl uence health, it is important that nurses understand global health problems common for neonates, infants, and children and current recommendations to enhance their health. PAEDIATRIC RESPIRATORY REVIEWS (2005) 6, 134–152 REVIEW Why babies should never sleep alone: A review of the co-sleeping controversy in relation to SIDS, bedsharing and breast feeding James J. McKenna* and Thomas McDade University of Notre Dame, Notre Dame, Indiana, IN 46556, USA KEYWORDS Summary There has been much controversy over whether infants should co-sleep or co-sleeping; bedshare with an adult caregiver and over whether such practises increase the risk of bedsharing; SIDS or fatal accident. However, despite opposition from medical authorities or the SIDS; police, many western parents are increasingly adopting night-time infant caregiving breast feeding; patterns that include some co-sleeping, especially by those mothers who choose to mother–infant breast feed. This review will show that the relationships between infant sleep patterns, relationship infant sleeping arrangements and development both in the short and long term, whether having positive or negative outcomes, is anything but simple and the traditional habit of labelling one sleeping arrangement as being superior to another without an awareness of family, social and ethnic context is not only wrong but possibly harmful. We will show that there are many good reasons to insist that the definitions of different types of co-sleeping and bedsharing be recognised and distinguished. We will examine the conceptual issues related to the biological functions of mother–infant co-sleeping, bedsharing and what relationship each has to SIDS. At very least, we hope that the studies and data described in this paper, which show that co-sleeping at least in the form of roomsharing especially with an actively breast feeding mother saves lives, is a powerful reason why the simplistic, scientifically inaccurate and misleading statement ‘never sleep with your baby’ needs to be rescinded, wherever and whenever it is published.
