Although C. burnetii infection is typically associated with acute infection, atypical pneumonia and flu-like symptoms, endo- carditis, osteoarticular manifestations and severe disease are possible, especially when the patient has a suppressed immune system; however, these severe complications are typically neglected. This study reports the sequencing of the repetitive element IS1111 of the transposase gene of C. burnetii from blood and bronchoalveolar lavage (BAL) samples from a patient with severe pneumonia following methotrexate therapy, resulting in the molecular diagnosis of Q fever in a patient who had been diagnosed with active seronegative polyarthritis two years earlier. To the best of our knowledge, this represents the first documented case of the isolation of C. burnetii DNA from a BAL sample. Environmental Nutrition Redefining Healthy Food in the Health Care Sector ABSTRACT Healthy food cannot be defined by nutritional quality alone. It is the end result of a food system that conserves and renews natural resources, advances social justice and animal welfare, builds community wealth, and fulfills the food and nutrition needs of all eaters now and into the future. This paper presents scientific data supporting this environmental nutrition approach, which expands the definition of healthy food beyond measurable food components such as calories, vitamins, and fats, to include the public health impacts of social, economic, and environmental factors related to the entire food system. Adopting this broader understanding of what is needed to make healthy food shifts our focus from personal responsibility for eating a healthy diet to our collective social responsibility for creating a healthy, sustainable food system. We examine two important nutrition issues, obesity and meat consumption, to illustrate why the production of food is equally as important to consider in conversations about nutrition as the consumption of food. There are a number of steps child care providers and staff can take to prevent or reduce the incidents of illness among children and adults in the child care setting. You can also refer to the publication Let’s Keep It Healthy – Policies and Procedures for a Safe and Healthy Environment. Hand Washing Hand washing is one of the most effective way to prevent the spread of illness. Hands should be washed frequently including after diapering, toileting, caring for an ill child, and coming into contact with bodily fluids (such as nose wiping), before feeding, eating and handling food, and at any time hands are soiled. Note: The use of disposable gloves during diapering does not eliminate the need for hand washing. The use of gloves is not required during diapering. However, if gloves are used, caregivers must still wash their hands after each diaper change. Instructions for effective hand washing are: 1. Wet hands under warm, running water. 2. Apply liquid soap. Antibacterial soap is not recommended. 3. Vigorously rub hands together for at least 20 seconds to lather all surfaces of the hands. Pay special attention to cleaning under fingernails and thumbs. 4. Thoroughly rinse hands under warm, running water. 5. -

Disease Reporting in Montana: Frequently Asked Questions Title 50 Section 1-202 of the Montana Code Annotated (MCA) outlines the general powers and duties of the Montana Department of Public Health & Human Services (DPHHS). The three primary duties that serve as the foundation for disease reporting in Montana state that DPHHS shall: • Study conditions affecting the citizens of the state by making use of birth, death, and sickness records; • Make investigations, disseminate information, and make recommendations for control of diseases and improvement of public health to persons, groups, or the public; and • Adopt and enforce rules regarding the reporting and control of communicable diseases. In order to meet these obligations, DPHHS works closely with local health jurisdictions to collect and analyze disease reports. Although anyone may report a case of communicable disease, such reports are submitted primarily by health care providers and laboratories. The Administrative Rules of Montana (ARM), Title 37, Chapter 114, Communicable Disease Control, outline the rules for communicable disease control, including disease reporting. Communicable disease surveillance is defined as the ongoing collection, analysis, interpretation, and dissemination of disease data. Accurate and timely disease reporting is the foundation of an effective surveillance program, which is key to applying effective public health interventions to mitigate the impact of disease. What diseases are reportable? A list of reportable diseases is maintained in ARM 37.114.203. The list continues to evolve and is consistent with the Council of State and Territorial Epidemiologists (CSTE) list of Nationally Notifiable Diseases maintained by the Centers for Disease Control and Prevention (CDC). In addition to the named conditions on the list, any occurrence of a case/cases of communicable disease in the 20th edition of the Control of Communicable Diseases Manual with a frequency in excess of normal expectancy or any unusual incident of unexplained illness or death in a human or animal should be reported. -

Vocal Cord Paralysis What Is Vocal Fold (cord) Paresis And Paralysis? Vocal fold (or cord) paresis and paralysis result from abnormal nerve input to the voice box muscles (laryngeal muscles). Paralysis is the total interruption of nerve impulse resulting in no movement of the muscle; Paresis is the partial interruption of nerve impulse resulting in weak or abnormal motion of laryngeal muscle(s). Vocal fold paresis/paralysis can happen at any age – from birth to advanced age, in males and females alike, from a variety of causes. The effect on patients may vary greatly depending on the patient’s use of his or her voice: A mild vocal fold paresis can be the end to a singer's career, but have only a marginal effect on a computer programmer's career. What Nerves Are Involved In Vocal Fold Paresis/Paralysis? Vocal fold movements are a result of the coordinated contraction of various muscles. These muscles are controlled by the brain through a specific set of nerves. The nerves that receive these signals are the: Superior laryngeal nerve (SLN), which carries signals to the cricothyroid muscle, located between the cricoid and thyroid cartilages. Since the cricothyroid muscle adjusts the tension of the vocal fold for high notes during singing, SLN paresis and paralysis result in abnormalities in voice pitch and the inability to sing with smooth change to each higher note. Sometimes, patients with SLN paresis/paralysis may have a normal speaking voice but an abnormal singing voice. The recurrent laryngeal nerve (RLN) carries signals to different voice box muscles responsible for opening vocal folds (as in breathing, coughing), closing vocal folds for vocal fold vibration during voice use, and closing vocal folds during swallowing. -

WEST VIRGINIA 2017 ZOONOTIC DISEASE SURVEILLANCE REPORT Office of Epidemiology and Prevention Services Division of Infectious Disease Epidemiology 350 Capitol Street, Room 125 Charleston, West Virginia 25301 Miguella Mark-Carew, PhD Jessica Shiltz, MPH Eric Dotseth, MS 2017 Mosquito-borne Disease Surveillance Summary Introduction Mosquito-borne diseases, most of which are viruses, are transmitted through the bite of infected mosquitoes. Surveillance for these diseases in West Virginia focuses on four endemic arboviruses—La Crosse encephalitis virus (LAC), West Nile virus (WNV), St. Louis encephalitis virus (SLE), and eastern equine encephalitis virus (EEE)—and travel-associated diseases such as chikungunya, dengue fever, malaria, and Zika virus (ZIKV). Historically, LAC has been the mosquito-borne disease of most concern in West Virginia, with up to 40 human cases reported in previous years. Most people who become infected with endemic with arboviral infections have no clinical symptoms; however, encephalitis (inflammation of the brain) is a potentially life-threatening complication that is often reported among infected persons who develop symptoms. Symptoms generally begin one to two weeks after a mosquito bite and include fever, headache, myalgia, meningitis, and neurologic dysfunction. There is no specific treatment available for arboviral infections. Environmental surveillance for arboviral diseases monitors local activity in non-human species. Mosquito surveillance is important to understanding the distribution of these vectors and the diseases that they may transmit to humans. Mosquito surveillance is conducted in selected counties across the state from late spring through fall. Horses can become infected with arboviruses resulting in clinical illness. Mosquitoes, dead birds and horses have all been used to help identify WNV and other arboviral disease activity in West Virginia. -
