Request ID Received Date Requester Name Organization Request Description 19-00231-FOIA 1/10/2019 Hagemann, Hannah The Monterey County Herald Pursuant to the federal Freedom of Information Act, 5 U.S.C. 552, I request the following information: all documents, budgets, studies, reports related to the health risks of continued, multi-year exposure to wildfire smoke. I would like to receive the information in electronic format. I would like to receive the information in electronic format. I am making the request as a reporter and student currently studying at the UC Santa Cruz Science Writing Graduate program; this request is made as part of newsgathering and not for commercial use. As a representative of the news media I am only required to pay for the direct cost of duplication after the first 100 pages. I ask that you waive any and all applicable fees associated with this request. Through this request, I am gathering information on firefighter health and safety. Release of this information is in the public interest because it will contribute significantly to the public understanding of government operations and activities, particularly on the realm of firefighter health, and the health risks of continued exposure to wildfire smoke. If you deny this request for a fee waiver, please advise me in advance of the estimated charges if they are to exceed $100. If my request is denied in whole or part, I ask that you justify all deletions by reference to specific exemptions of the Act. I will also expect you to release all segregable portions of otherwise exempt material. I reserve the right to appeal your decision to withhold any information or to deny a waiver of fees. I also ask that you provide expedited review of this request which concerns a matter of urgency. The information sought relates to a currently unfolding story, specifically, that the first study investigating the respiratory health of forest firefighters is happening now. Second, delaying release of the information harms the public interest in knowing what actions are being taken to understand the health risks of firefighters’ exposure to wildfire. Third, the request obviously concerns federal government activity, as the request seeks information on funding, research, and correspondence related to understanding the health risks of firefighting. I certify that my statements concerning the need for expedited review are true and correct to the best of my knowledge and belief. If you deny expedited processing, I expect to receive a response to the request within 20 business days, as the statute requires. 19-00236-FOIA 1/15/2019 Karimi, Adrian Ladah Law Any documents regarding beef E.coli or Salmonella outbreak in Nevada (Date Range for Record Search: From 07/01/2018 To 12/11/2018) 19-00243-FOIA 1/17/2019 Thomason, Aspen Waldon Adelman Castilla Hiestand and Prout respective copies of employment records for individual 19-00244-FOIA 1/16/2019 Yu, Scott Darsana Capital Partners copy of data referenced in speech on E-cigarettes (Date Range for Record Search: From 1/1/2018 To 9/30/2018)

I ran a flight deck on a helicopter carrier and for two years and daily was exposed to long periods of jet and motor gas fuel by vapor and directly on my skin in closed spaces, on the flight deck, as well as to exhaust from jet engines directly and indirectly as residue on the ship. At that time I developed symptoms, which I didn't discover until recently were consistent with exposure to these fuels and exhaust. The symptoms being neurological damage, autoimmune disease, headaches, audio dyslexia. I've come across some studies which support the claim I'm making to the VA. But I'm looking for more studies to support my claim, which have stronger validity for the VA. Thank you, b6 (Date Range for Record Search: From 01/01/1979 To 03/08/2019)

I am trying to put together a historical report on ART in the US and need a few data points that have changed in the reporting. Specifically - IVF %, GEST %, and PGD % changed in 2016 to be all cycles instead of just frozen cycles. Is there historical data for all cycles at each clinic (or ideally frozen and fresh separately)? Also, prior to the 2011 report frozen and egg banking cycles by clinic is not included in the spreadsheet data file, but is shown in the national report. Would it be possible to receive historical data for frozen and egg banking cycles by clinic and by age group prior to 2011?
