APAN 43: 2017 LEARNING EXPERIENCE

Firstly, i would like to thank TEMDEC for inviting me to this wonderful meeting at the beautiful city of New , India. The meeting was held at the India Habitat Center (IHC) from 12th to 17th February 2017. The event is a 6 days event and I had joined the Medical Working Group Session. The event consists of preparation and testing on 12th, medical technology related session from 13th -16th and Engineering session from 16th to 17th February 2017.

This is my fourth participation in APAN meeting after APAN 40 (Kuala Lumpur), APAN 41 (Manila) and APAN 42 (Hong Kong). I was invited as trainer, presenter and also as chairperson for the Technology and Engineering session. We had started with our program since the first day (12 Feb 2017) for setup and test the video conferencing equipment at the main venue. I had the opportunity to explore and know in detail the AV setup at the venue and hands on experience working with venue engineer from India, TEMDEC engineer and other trainer to setup the system and test as preparation for the live event next day.

I was given an opportunity to co chair the Technology session with Mr Ti Chuang from National Taiwan University and Mr Dinesh from ILBS, India for the gamification technology and join view software. The topic for gamification was select by me and i had invited a speaker from Malaysia for the presentation. It was an eye opening to all participants that the technology can also be applied to medical and telemedicine activity as well.

On the following day, each one of us was assigned under 5 teams which will take in charge of Venue, Presentation, Audio, Video and Network. Each of the group will take turn to in charge one of the role during each of the session for the APAN. This is totally new format and it can give the engineer the hands on experience to gained the skill on handling telemedicine session as well as experiences to troubleshoot the problem during the event. I was assigned as Trainer under group 5 with 2 trainees which are Mr Nabil from Singapore General Hospital and Mr Ishaq from JIPMER.

For the medical session, I had gained and get to know the advancement of rural telemedicine technology which were presented by few delegates from other country on how they had applied to improve the healthcare services as well as research and medical training at the rural area. This is totally a new thing to me as Malaysia still do not started the telemedicine services to rural areas.

I was assigned as the chief engineer with Mr Shunta for Endoscopy (ECE) session on Tuesday, 14th February 2017 1.30 – 3.00 p.m assisted by few more venue engineers with the specific roles. As a chief engineer, i have to make sure the technical condition at the venue and remote participant are ready and acceptable for the live session.

The background work had started from few weeks before the APAN meeting and technical preparation was already conducted. The final test was done one hours before the event to make sure the event is run smoothly. There are 7 centers are joining the session using Vidyo software as usual. The overall ECE event was run successfully despite the Vidyo Room hardware at event venue disconnected few seconds times during the live session and power trip from Univeristy Malaya.

On the last day of the meeting (Friday), which is my area of interest, is for the Engineering workshop. We had a very good and interactive session about the session review of the 4 days program in the morning. The session review is to discuss the issue and problem encountered during the live session, so we can further improve the technical and engineering to make the session even better in the future.

I had presented the session review of the Endoscopy (ECE) event and discussed with other engineers about the problem and future improvement of the event. The session is very interactive as the engineers had expressed the opinion/comments about the problem and suggestion to improve the telemedicine experience. I was also been given an opportunity to close the Engineering session and this is a new experience to me as this will usually been done by Prof. Dr Shimizu himself!

Finally, i think this is a very fruitful meeting and I had learnt a lot from others and at the same time share my experience and knowledge with other participants. I had also very happy to meet old and new friend during this symposium and expand my networking contacts to other countries. I hope to join the meeting again next time.

Trip Report by Ti-Chuang Chiang (NTU):

My trip to 43rd APAN – New Delhi, India. From 11~18 February. 2017.

On 12th afternoon. All trainer and trainee met at the venue to support videoconferencing devices setup still evening. It’s a good opportunity for us to learn how to solve the problem as a team. The setup work continued till next day morning.

In the afternoon, I chaired the Technology session with my new cochairmen Mohamad Zahir Ahmad from Malaysia. This is our first time cooperation. The speaker Dr. Ali Imran recommended by Zahir gave all participants from 12 remote connecting sites and local venue audiences a very good introduction about Gamification. The new video server platform “JoinView” was demonstrated on the second part of the technology session. One nice demonstration was from Mexico by our engineer partner De Dios Harold. We had a wonderful discussion among us and planned the future use of this server.

I also co-chair the Rural Healthcare session on 16th morning with Dr Saroj K Mishra together with remote site’s chairman Dr. Saroj Dhital from Nepal. During other session, I work together with my trainee Repu Daman Chand and Ibrahim Gani as a team. Although my role is team leader but there are many good experiences we can learn from each other. We monitor every session’s videoconferencing quality and provide the records to each session’s chief engineer.

I participated Engineering Workshop from 16th afternoon till 17th morning. Ms. Kuriko Kudo from Kyushu University Hospital joint with me remotely chaired the review session from chief engineers 1. My teammates gave a very good presentation about things they learned during this APAN meeting.

It was a great time can met our old friends and knew more new friends. This meeting will deepen our friendship and build a good team to support Asia Pacific region’s medical community telemedicine activity.

108 Military Central Hospital’s Report

108 Hospital stated to connect the VINAREN network in 2008. We had more require of infrastructure, devices communication and supporting technical. At that time, we basically understood teleconference systems in several workshops. In 2013, 108 Hospital had the LifeSize communications. Images and videos from operating room can transfer to the meeting room which improving many training courses. We gained some experiment through join some events.

During the APAN 43, we are happy when working together in the each workshop sessions. We are not only theoretical but also practical. Each trainee is assigned responsibility to a conference session is very helpful. For me, I am is venue’s engineer for Health care, Rural secssion.

Besides the APAN 43, we had a opportunity to realize international telemedicine event. At the planning stage, engineers prepared teleconferencing systems. Their skill were excellent at diagnose trouble, troubleshoots, work in stressful situations. They had expressed the collaboration, responsibility, flexibility.

In almost sections, there were 5 to 6 station from many institutes, universities, hospitals which updating the information and sharing their experiment. It is shown that there were a lot of attention for medical, education, other research. Finally, all of stations were look forward to join in next time.

We appreciate TEMDEC’s invitation which help us opportunity to change our mind. It is not simple to develop the Telemedicine systems but we have to move on. We hope in the future, we will have more opportunity to attend the training courses of APAN.

APAN 43 Activity Report By Tunggul Laksono

INDIA…. Finally I came to this country, for the first time arriving in India at night we directly go to our hotel. And there is not good area, because so crowded area and noise street (honk every time ). But when in the morning I just realize that nice environment actually, because we are in the traditional market area, and after we confirm my neighborhood is Old Delhi, so we can see the truly India during my stay in India. It’s very nice experience to be able stay one week in New Delhi. Event the city is Dusty city from my opinion. During apan meeting I have much more experience to handle telemedicine because for the first time I take role as Trainer beside Chief Engineer. And Kuriko San also already divide all engineer in to 5 team, and also assign task for each team. This document is some kind as Standard Operation Procedure for medical working group during APAN43. Using this document every team can take position in every teleconference session. So all session can handle smoothly by all engineer team. And during apan43 Kuriko San assign me as Team D and the color for this team is PINK!!! We The PINK Team actually since APAN42 Hongkong. Here I met other engineer face to face, from Vietnam, Malaysia, Singapore, India, and Taiwan and working together as group. But during apan43 our task is much simple because the India Venue Engineer prepare everything for us, thank to India Team. In this meeting I handle the Dental session that held on the third day on Medical Working Group. And the session already held smoothly event though there was national holiday in Indonesia. But the show must go on and only one station cancelled their attendance. All station connected well only from National Taiwan University have webcam problem. And before that drg. Mellisa having some trouble that she was join but can’t turn on her webcam and audio, but after that she can managed. So the meeting is okay. The nice experience during stay one week in India, is so many India culture we can see, and lucky us we meet with the India bride riding horse heading to her wife home with so many music, dancing all over the street. The citizen told me why the male bride is riding horse, the reason is to give him a chance to runaway, because after he got married he will be tighten by her wife, what a nice culture. So many street food, antique shop, gift shop, and so many other thing along my hotel street. Event during the event there is some network problem, it’s seem that firewall configuration in India Habitat Centre need to be adjust must higher or miss route configuration, but so far when we are using TEMDEC Server no problem at day 3, but before that connect to TEMDEC server also unstable. I’m doctor Tai. I’m come from Viet Duc Hospital, Viet Nam. I take part in APAN 43 at the first time. I have some comments : - The Equipment, network, image, sound is very smooth but image sometimes broken but team Engineer have prepare is quickly, so the sessions are very useful and succesful - Thanks you team leader from TemDec, all Engineers and team Doctor set up all conference from everywhere all the world - I hope in the future I will take part in many many conference…. - Thanks you very much and see you in APAN 44 at China.

APAN 43 REPORT This is the first time I participate to such a big conference like APAN in the field of telemedicine. I am so surprise at the commodious and spread of telemedicine in Asia – Pacific. I have a little experience when I was a secretary of the Asia Telemedicine Symposium Vietnam – 2016. At the first time, I didn’t have any information about telemedicine but now I know that, it’s very useful to connect the medical centers from all over the world to the aim of improving the healthcare activities quality. I think that in the future, telemedicine will take a very important role in the development of medicine especially in poor countries.

As a young surgeon, I could find many interesting topic in recent APAN, for examble, the session about Minimal Invasive Surgery ang Gastro-intestinal Endoscope. I can stay in the venue and watch many interesting operations and cases from abroad. Thank to the engineer teams, the image, voice and the connection is very smooth, so I didn’t have any trouble to aquire knowledge during the time of the conference. About the engineers, I admired the professional, high-equiped and enthusiasm of them very much. I think the they have taken the most important part in the success of the Medical Working Group of the APAN. I belived that, in the future, they will be more and more professional and they can organize any conference or workshop which connect to anywhere in the world. For myself, as a consultant and coordinator of the Center of Training and Healhcare activities direction of Viet Duc Hospital, Hanoi, Vietnam. The experience from this opportunity will help me much in building and developing the telemedice system in my hospital. Finally, I would like to say thank to TEMDEC for my chance to come to India and join in the APAN. This event will help me much in my career and give me a valuable experience. TRAN DUC TAM ACHIEVEMENT REPORT

As the largest surgical center of Vietnam, Viet Duc Hospital has many opportunities to connect to the telemedicine. At that time, there werea little equipment, and the network was too slow to connect outside our country.

Even though we have been connecting to the APAN several times, we see many problems like: the hard-to-be-heard sound, the unclear video in some stations when the ATS start.

During the APAN43, I had a precious chance to take part in an international telemedicine event. At planning stage, engineers prepared teleconferencing systems. Their skills were excellent at diagnosing troubles, troubleshoots, working under high pressure. They had expressed the collaboration, responsibility, flexibility at work. Moreover, I am exposed to how good quality video and standard sound are.

In each section, there were 5 to 6 stations from many institutes, universities, hospitals updating the information and sharing their experiments. It is shown that there has been a great attention for medical and educational research. Especially, in the session for the technical issues :”APAN tech review” , there were the show of problems that each stations have, so I know the problems that we need to improve.

Finally, I would love to express my sincerely grateful to APAN43, TEMDEC for giving me this chance to participate into APAN43. I have gained valuable knowledge about the system and the technical equipment which are necessary for the telemedicine. Furthermore, I have made new friends all over the world that can help us in connecting telemedicine.

This meeting has provided opportunity for me to learn how to plan, prepare and manage teleconferences as an organizer. At the same time, I discussed the further expansion of the network in Asia-Pacific region. It's also a wonderful opportunity to meet doctors and engineers from Asian regions and deepen our friendship in telemedicine activity. I had a good opportunity to understand the Telemedicine, to have the new friends in workshop, to visit the nice places in New Delhi, India. MEETING ACTIVITIES: I've attended the program of the Medical Working Group (MWG) which consists of the following sessions: 1. Equipment setup, connect and testing everything before the sessions starts (Monday morning, February 13, 2017). I've observed the engineers who prepared this session. 2. I've attended all sessions: Technology, JIMPER, MIS, Endoscopy, HPB, Healthcare, Dental, Ophthalmology-1, Rural Healthcare, Ophthalmology-2 and Engineer Worshop. I participated in the Ophthalmology-1 and Engineer Workshop as a Co-Chair. 3. I've attended the Opening Plenary of APAN43. 4. I've shared my opinions with everybody to deepen the friendship in Telemedicine activity. By the end of the meeting, I was able to know how the Telemedicine programs were organized and performed, to gain some knowledge about technical preparations for Teleconference, about quality of transmitted image and sound... ACHIEVEMENT REPORT TRAN VIETQUANG MINH, Hue Central Hospital - Vietnam Hue Central hospital, a general polyclinic, is under direct control by the Ministry of Health. It’s one of the three biggest general hospitals in Vietnam with healthcare system of 3,000 beds. Hue Central hospital is responsible for direction of healthcare activities for 18 nearby provincial hospitals. Telemedicine is very necessary for Hue Central Hospital, as it helps the sattellite hospitals to get training and technique-transfer from the Hue Central Hospital quickly. Hue Cental Hospital can also get benefits of new medical information from other countries. As the trainnee at the 43th APAN, I had following comment: - The 43th APAN is a good chance for me to link with many collegues among Asian countries. - I had opportunity to joint to technique matters. - I had opportunity to learn much from experienced engineers. - The evaluation at each workshop was very helpful. - Role of chief engineer in monitoring the workshop. - I had opportunity to joint in commentator at engineer session. -I highly appreciated TEMDEC’s invitation, especially Pro. Shimizu and colleagues at Kyushu University Hospital, who gave me opportunity to join in this APAN.

The most difficulty for me, I realize, is that my English skills are not very good. Our plan for future: - Sharing experiences with other colleagues. - Contacting with some key Hospitals (Vietnam and in the world) for telemedicine network. - Linking with tele-medicine engineers to join APAN together. - Upgrading telemedicine systems at Hue Central Hospital. - Continuing to joint in the telemedicine organized by Kyushu University Hospital. I highly appreciate TEIN and TEMDEC’s invitation which gives me opportunity to change my mind. It is not simple to develop the Telemedicine systems but I have to try to move on. I look forward to having opportunity to meet together again at the coming APANs.

REPORT of APAN 43 MEDICAL WORKING GROUP NEW DELHI

When did it start Initiative held a session originated from an invitation dental Miss Ryoko Yoshida in December 2016. I further define the field of dentistry that will be selected on the teleconference. I decided to choose Periodontology because many interesting issues that could be discussed related to the rapid advancement in the discipline.

Oral cancer is cancer of the mouth or throat. Each year, an estimated 45,750 people are diagnosed with oral cancer and more than 8,000 will die of the disease. Of all the procedures, available to control oral cancer, none has affected survival as much as has early detection. Unlike other parts of the body, the oral cavity is easily accessible and an oral cancer examination poses relatively little discomfort or embarrassment for the patient. Research shows that cancers can be prevented if people applied everything known about cancer prevention to their lives. In this session, we would like to explore the latest research and the methods employed for the prevention of oral cancer, in which dentist plays an important role in this regard. My next step is to determine the speakers. Of searches from various sources, I got competent speaker candidates because of their great reputation in the field of scientific and clinical works. Of course they are periodontologist. I invited about 10 speakers from various dental institutions in Asia, but only five speakers confirmed, as follows:

Because of I want to expand telemedicine to dental institutions in Indonesia, then I invited two dental schools, namely: 1. University of Sumatra Utara 2. Hang Tuah University.

We also issued a webstream that was accessed by many stations in Indonesian dental community. The information of the video stream was shared in social medias.

To make the meeting more interactive, I was supported by two Co-Chairs from University of Indonesia, Dr Mellisa Adiatman and Dr Aria Kekalih. The final step is to determine the engineers, the system used, and the server used. The composition of the engineer is: 1. Mr Tunggul Laksono (Brawijaya University) - Chief Engineer 2. Based on the various of engineers’ experiences, quality of equipment and different bandwidth at each station, it was decided to use Vidyo System, throgh the server of Kyushu University.

The implementation

The Indonesian Government announced that April 15 2017 was the National Holiday, due to the Governor Election. The University of Sumatera Utara decided to cancel their participation, and the rest of Indonesian stations was still involved. The meeting itself was started on time and on schedule. The connection was good, as well as sound quality. Dr Mellisa Adiatman conducted the discussion session, and Dr Aria Kekalih made summary.

The engineering workshop gave me outstanding knowledge and experience. Continuous evaluation and collaboration between engineers brought the meeting to the success.

Thank God, with God's helps, and support from various parties, the activities of dental teleconference session took place smoothly. All stations were happy with these activities connected to the telemedicine network in Asia Pacific. Hopefully knowledge sharing activities are beneficial for all, and connections in Asia Pacific dentistry is getting stronger.

Report on “APAN43”

This is Ibrahim Gani from Sankara Nethralay Hospital, Chennai, India. I am very happy to be a part of “APAN43” held in Delhi, India from 12th to 17th February 2017 and thanks to the Organizing Team “APAN43” & TEMDEC for provided me this opportunity to attend the meeting. As a technical person, the Engineer Training Program was really informative and helpful to update my technical knowledge related to Tele Medicine technologies. This was my Second APAN meeting and this time you have separated the engineers in to different teams and provided responsibilities to each teams, this is really helped the each team members to understand all the activities like Connecting Sites, Streaming, testing audio, Handling presentation, Etc.

Our EYE 2 sessions are went on smoothly with good audio and video quality. Multiple users accessed the web streaming to view the Ophthalmology session and the participant at the conference venue were able to view the conference participants clearly, almost all the sessions are fine without any major issues. The Infrastructure is very good and the technical team is very friendly and cooperative. Engineers from University of Malaya, Singapore General Hospital, Hue Central Hospital Veit Duc Hospital along with other remote engineers participated in the conferences. Engineers are divided in five parts along with Chief Engineers to monitor and system, technologies. They are supposed to perform like actors during the events and go through the technical details along with quality assessment. Cooperation & Coordination among team is important for successful enterprise programs. I, Repu Daman (STBMI, SGPGIMS Lucknow, India) along with Mr. Gani (Sankar Netralaya, Chennai, India) are part of Team B under chief engineer Mr. Ti-Chaung Chiang of National Taiwan University. Mr. Shunta Toimatsu, Kyushu University was wild card for the entire event. Any body can take the help if stuck in between due to technical failure. Yasuichi Kitamura, Kyushu University Showed Joinview module during the event. Checklist is given by TEMDEC Team during the event along with the link for feedback for each session. All checklists were later on shared with respective Chief engineers by email and also online feed back is also given immediately after the session event. Technologies like Adobe Connect for Chat between engineers were used during the session. Link to streaming solutions, along with technical tips were shared by Adobe Connect apart from email. Vidyo, Zoom and H.323 Video Conferencing Systems were used for live interactive multi-party conferencing. Room has Christe make 4K projector was used to project screen of size approx 12 X 8 Size for HD Display. PCR has H.323 Video Conferencing System, Video Switchers. 100 Mbps bandwidth of National Knowledge Network (NKN) has provided bandwidth during the workshop. On 16th Feb presentation on “Evolution of SGPGI Telemedicine Programme” followed by two videos i.e Virtual Tour of STBMI and Kumbh Mela, Ujjain was shared with all connecting centers and local participants.

My report on “APAN43”

This is Mohamed Ishaq Z - Telemedicine Engineer from Jawaharlal Institute of Postgraduate Medical Education and Research (JIPMER) Pondicherry, India. I am very happy to be a part of “APAN43” held in Delhi, India from 12th to 17th February 2017 and thanks to the Organizing Team “APAN43” & TEMDEC for provided me this opportunity to attend the meeting. The Engineer Training Program was very informative and helped me in gaining knowledge from TEMDEC Engineers technical preparation and coordination to the other centres. The qualities of all the sessions are very clear both video as well as audio clarity. This meeting was very much useful and good experience for me to interact with Engineer/Doctors/Senior members from various countries.

Report for the 43rd Asia Pacific Advance Network Conference (APAN43), India

First and foremost, I would like to thank TEMDEC for inviting me as a trainee to the 43rd Asia Pacific Advance Network conference.

There were many things that I have learnt on this trip. One of which is how a telemedicine conference is set up and how it is run and managed. I learn the importance of testing and how it helps to make the conference a success.

Another important thing that I have learnt is teamwork. Being assigned to a team and learning from my team leader, Mr Zahir, I am further able to understand how things work and how they are set up. I feel that without Mr Zahir, I would not learn as much as I did. Being part of a team has also allowed me to learn about the different aspects of the telemedicine set up such as what to do as the venue engineer, the video engineer, the sound engineer and the presentation engineer. I also learnt what to do when there is a problem and how to troubleshoot them.

I also learnt how to control the output for the video feed such as which video feed to transmit to Vidyo and which to transmit to the local venue.

Lastly, I met a lot of engineers while I was at the conference. I learnt a lot from them and they are able to share their knowledge and skills with me. I look forward to working with them again.
