Athlete Medical Release Parent hereby authorizes Crested Butte Mountain Sports Team and/or their named coaches, to secure any hospital, medical, dental or surgical care, treatment and/or procedures for the above, named athlete. The following are general rules for participating with the Crested Butte Mountain Sports Team. These rules are guidelines for your conduct as a CBMST athlete and parent. As a CBMST athlete, you are expected to act in a mature and responsible manner as your actions reflect on yourself, your parents, coaches, and the program/team as a whole. Your behavior directly affects the team's reputation, standing in the community and ability to access better training opportunities and cooperation from the mountain. These rules must be adhered to. Any rule infraction may result in disciplinary action as set forth.

Dear Parents, Please know it is a privilege for us to be entrusted with the opportunity to coach your child and the rest of the competitive skiers and snowboarders of the Gunnison & Crested Butte area and beyond. We at CBMST are going to do all we can to help your child reach his or her full potential on and off the slope. We will be focused on instilling a love for and an appreciation of being safe, the learning process, and respect for others. Through these, your child can best realize their athletic potential and develop character traits that will serve them throughout their lives in whatever they choose to do. We want what is best for your child – their health, well-being, and future as an athlete and a person. You, the parents will play a vital role in your child’s success with the Mt Sports Team. We can only accomplish our goals if we have your help and support on and off the hill!

ON-HILL POLICIES AND BEHAVIOR 1. Remember, we are ambassadors for the area, and we need to act as such at all times. Please show respect for your coaches, teammates, public skiers, and employees of Crested Butte Mountain Resort. 2. Foul language on the hill or in the club room is unacceptable. Directing profanity towards your teammates or Coaches directly or indirectly, on-hill or off, will be taken very seriously and will be dealt with accordingly. 3. All athletes will arrive at training with a good positive attitude. Athletes will be expected to be attentive and energetic or they will not be welcome to train. 4. Always carry your season pass. It is not our responsibility to get a replacement ticket if you forget your pass! 5. Be courteous when presenting your ski pass at all resorts. The lift attendant is simply doing their job! 6. Obey the skier's responsibility code. The general public comes first at all resorts. Typically, you will be the better skier or rider, be respectful to those learning the sports we all know and love. 7. No fast skiing in any SLOW zones, pass privileges may be revoked. 8. Lift line privileges will be managed by the staff and are a privilege that may be revoked or suspended at any time. Athletes must be with a coach to access this privilege. 9. Athletes should always present themselves with an appropriate, respectful, athletic demeanor. 10. Always use the restraining bar on any lift that is equipped with one, and please sit back & sit still. 11. In case of emergency, call the ski patrol 970-349-2236 or go to a lift attendant for assistance.

TRAINING ATTIRE: 1. Helmets must be worn at all times and fastened correctly. 2. Training attire is different for every sport. Please check with your head coach during the first weeks of training to find out what is expected of the athletes. Team jackets are supplied by the Crested Butte Snowsports Foundation please contact them about obtaining a jacket.

EQUIPMENT Equipment needs vary by sport and level. Please check with your head coach in the fall to find out what will be expected for the athlete’s individual needs.

Your equipment is your responsibility. Take an active interest in learning proper tuning techniques, we are happy to help! By the time you are 13, you should be able to maintain your /board without the help of your coaches or parents and not just the ski racers! Take care of your equipment and it will take care of you. ACADEMIC ELIGIBILITY School work is equally important, if not more so than competing in sports! 1. Each student athlete must maintain at least a cumulative “C” (70%) in each class for a given semester in order to continue to train and compete with CBMST. If acceptable grades are not maintained at all times, the privilege of training and racing may be taken away. Please refer to the CBCS- CBMST Cooperative Program and the CBCS Student Athletic Handbook for more details. We cannot stress the importance of this enough! 2. It is the athlete's responsibility to inform and communicate completely, well in advance (2-week min.), with their teachers prior to absence from class for any races or special training where they will be absent from school. 3. It is the athlete's responsibility to make up missed assignments and/or find a tutor if needed.

TEAM ROOM AND OFFICE- 1. Please be respectful of all locker room space. 2. Every day one team will be responsible for locker room clean up. 3. Wrestling, snowball throwing, pole fights, name calling and roughhousing will not be tolerated. 4. The team and coaches' offices are off-limits unless invited. 5. Tuning benches are there for everyone's use. Please clean up after yourself. 6. Hard goods can be stored in locker room overnight. Soft goods and boots must return home with the athletes daily. 7. CBMST is not responsible for lost tuning supplies or equipment. 8. Borrowing other teammates' equipment or items without permission will be treated as stealing and will immediately be treated as a 3rd infraction as described below. 9. Only the coaching staff may use video equipment, cameras, drills, etc. 10. If eating lunch in the building, athletes are responsible for cleaning up after themselves. 11. Drugs of any kind are illegal and their use is prohibited by CBMST members. Alcohol, marijuana, illegal drugs and tobacco are illegal for those under the age of 21, and their use is strictly prohibited. CBMST will enforce these laws and will contact the police if we find anyone breaking these laws. This includes traveling for competitions and training. 12. No pets are allowed in the building, team room, or on the hill. 13. There will be no recording of any athletes of any nature in the team room. 14. Certain coaches of MST are mandatory reporters for child abuse and SAFESPORT certified.

PENALTIES FOR RULE INFRACTIONS CBMST will enforce the above code of conduct. Any disrespect or disregard for these rules will be met with serious consequences. 1. Infraction 1: Dismissal from practice for the day. 2. Infraction 2: Suspension from practice and races for the week. 3. Infraction 3: Release from the team with NO MONETARY COMPENSATION.

Team travel and transportation will not be possible due to COVID restrictions. Parents will be required to travel with their student athletes.

Parents and Athletes, please review these guidelines to make yourself aware of CBMST travel policies. Athletes competing at an event – even if they do not travel with the team or stay with the team – will be required to share in the cost of the coach’s travel expenses (team gas & food). These costs are not included in your program’s tuition and are additional costs as well as additional program fees on longer trips. For example: If you are a 2 day/wk. athlete on a 5 day trip there will be an additional 3 day program fee @$65/day. - Athletes must have notified all of their teachers a minimum of 2 weeks in advance and provide them with a tentative schedule for the season ASAP. School work missed should be done during the trip or prior to and the students should not request extra time to complete assignments. - - Athletes may be charged a coaching fee for the difference of the days of the trip and the days of programming they have enrolled for. (i.e. A 1-day athlete goes on a 3-day trip they will be billed for the additional 2 days of coaching while on the trip.) - - Any athlete under the age of 13 must be accompanied by a parent or guardian. Parents are encouraged to be present during competition for any age athlete and any competition. - - Athletes must be invited/approved by the attending coach in order to travel with the team and/or attend the competition as a CBMST athlete. - - A trip summary and expense work sheet will be provided for the away competition 14 days prior to the event. - - The trip roster will be finalized 5 days prior to departure. Any changes in the roster after 5 days will result in a minimum $50 fee or more depending on the expenses and circumstances of the trip. -1. Be on time for all departures. 2. Have your equipment ready prior to departure. Make a check list! 3. Assist in loading, unloading and cleaning the team vehicles. 4. Ensure that all entry fees are paid in advance and athletes are provided adequate funds for incidental costs such as lift tickets, food, etc... 5. Travel and lodge with CBMST coaches during out-of-town events, unless otherwise approved and pre-arranged. 6. Respect boundaries - no boys in girls’ rooms and no girls in boys’ rooms unless supervised by a coach or parent. 7. Follow curfew restrictions designated by coaches and all other Athlete code of conduct rules. - CBMST will do everything possible to supply transportation to away competitions, but due to increasing schedule demands of other disciplines and a growing number of athletes attending this is not possible.

- Failure to adhere to any of these guidelines may result in travel suspension and/or training suspension.

Freeride & Snowboard Outline IFSA (International Freeride Ski/Snowboard Association) www.freeskiers.org - The overarching sanctioning body for North American Freeride events. Good resource for information like the rulebook, Jr Series schedule, Rankings, Membership information The IFSA began in 1996 and began sanctioning junior events in the 2010-2011 season. Explosive growth and a need for sanctioning in Region 2 at the FWQ Adult level led the IFSA to partner with the Freeride World Tour in the 2014-2015 season. Since that time, the IFSA has unified events across North and South America to form the Junior and FWQ Series within Region 2. The explosive growth of the sport has led to an incredible amount of evolution within the IFSA organization and to the sport of competitive Freeride itself.

RMFS (Rocky Mountain Freeride Series) www.rockymountainfreerideseries.com RMFS was created in 2016 to create a cohesive group within the Rocky Mtn. Region (Colorado and New Mexico) to benefit junior athletes through the freeride competitions under the governing body of IFSA. We have over 500 athletes and are the largest region within IFSA.

USASA (United States Ski and Snowboard Association www.usasa.org USASA is the sanctioning body for a wide variety of freeride competitions except for big mountain freeride events. They have regional circuits which qualify athletes for the USASA National events held in the spring. Our regional area is SW Colorado and this includes , rail jams, halfpipe, , snowboard alpine (GS & SL), and boarder/skier cross events. A USASA competitor membership is required to participate in any USASA competitions.

RESORT ACTIVITY, & EQUIPMENT RENTAL RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISKS & INDEMNITY AGREEMENT WARNING: PLEASE READ CAREFULLY BEFORE SIGNING! THIS IS A RELEASE OF LIABILITY & WAIVER OF CERTAIN LEGAL RIGHTS INCLUDING THE RIGHT TO SUE OR CLAIM COMPENSATION 1. Each person participating in the Activity (defined below) is referred to as “Participant.” I, the undersigned, am a Participant and, if a Participant is a minor/infant, I am the Participant’s parent or legal guardian. I understand that participating in ski and snowboard school, skiing, snowboarding, uphill access/skinning, racing, training, snowshoeing, snowmobiling, cross-country skiing, biking, tubing, ice skating, pond skimming, sledding, sleigh rides, riding the alpine coaster, zip lining, using the ropes/challenge courses, trampoline or climbing wall, resort and vehicle tours, using freestyle or terrain park features, participating in other recreational activities, renting equipment and using the ski area facilities, including but not limited to use of the parking lots, walkways, lodges, restaurants, lifts, ski slopes, trails, and other equipment, for any purpose (the “Activity”), can be HAZARDOUS AND INVOLVE THE RISK OF PHYSICAL INJURY AND/OR DEATH. 2. I understand the dangers and risks of the Activity and that Participant ASSUMES ALL INHERENT DANGERS AND RISKS of the Activity, including those of a “skier” (as may be defined by statute or other applicable law) or other winter sports participant. 3. Exposure to COVID-19, disease and sicknesses (“sickness”) is an inherent risk of the Activity. Every Participant has the responsibility to enjoy a safe and healthy experience. The resort cannot eliminate the risk that a Participant is exposed while engaged in the Activity. The Participant agrees it is his or her responsibility to (1) follow all instructions, signage, warnings, and guidelines; (2) stay home if sick or experiencing symptoms of any sickness; (3) wear a face covering and maintain at least 6 feet physical distancing from other guests, including in lift lines; and (4) wash and sanitize hands frequently. 4. I expressly acknowledge and assume all additional risks and dangers that may result in property damage, physical injury and/or death, which may be above and beyond the inherent dangers and risks of the Activity, including but not limited to: Falling or loss of balance; icy, slick or uneven surfaces; avalanches, cornices and crevasses; collisions with natural or man-made objects, other people, and/or other motor or over-snow vehicles; bumps; moguls; tree wells and stumps; downed timber and other forest growth; rocks, drainage channels, streams, creeks, holes, debris, and other rugged mountainous terrain; marked and unmarked obstacles; unmaintained or unmarked trails/roads or trail obstructions; the negligence or failure of Participant, Ski Area employees, or other guest to act safely (including an instructor’s selection of terrain that exceeds Participant’s ability) or within their own ability including failure to stay within designated areas and comply with signage; falling snow or ice from natural or man-made sources; equipment malfunction, failure or damage; improper use or maintenance of equipment; misloading, entanglements, or falls from ski lifts; varying visibility, storms, lightning, hail, snow and other adverse weather; becoming lost or separated; lack of shelter; limited access to and/or delay of medical attention; Participant’s health condition, physical exertion, exhaustion, dehydration, hypothermia, altitude sickness, other sickness, or frostbite; and/or mental distress from exposure to any of the above. I UNDERSTAND THAT THE DESCRIPTION OF THE RISKS IN THIS AGREEMENT IS NOT COMPLETE AND VOLUNTARILY CHOOSE FOR PARTICIPANT TO PARTICIPATE IN AND EXPRESSLY ASSUME ALL RISKS AND DANGERS OF THE ACTIVITY AND THE POSSIBILITY OF PERSONAL INJURY, DEATH, PROPERTY DAMAGE AND LOSS RESULTING THEREFROM, WHETHER OR NOT DESCRIBED HERE, KNOWN OR UNKNOWN, INHERENT OR OTHERWISE. 5. Participant assumes responsibility for maintaining control at all possible times while engaging in the Activity and for reading, understanding and complying with all signage, including instructions on the use of lifts. Participant must have the physical dexterity and knowledge to safely load, ride and unload the lifts. I understand that a minor Participant may use the ski lifts without an adult present or may ride the ski lifts with non-employee guests. I understand that snowmobiles, equipment, and snow-grooming equipment may be encountered at any time, and that falls, collisions, and injuries are common. 6. If Participant rents equipment, I agree to accept for use the equipment rented “AS IS”, accept responsibility for the care of the equipment during the rental period, and agree to be responsible for the replacement at full value of any equipment not returned or returned in a damaged condition. I have not made any misrepresentations with regard to Participant’s height, weight, age and/or ability level and represent that Participant will be the only person using the equipment. I UNDERSTAND THAT THE BINDING SYSTEM CANNOT GUARANTEE THE USER’S SAFETY. The binding system will not release at all times or under all circumstances where release may prevent injury or death, nor is it possible to predict every situation in which it will release. I further understand that the downhill system DOES NOT REDUCE THE RISK OF ANY TYPE OF INJURY TO PARTICIPANT. I understand that in snowboarding, cross-country skiing, snowshoeing and other sports utilizing equipment with non-release bindings, the binding system will not ordinarily release during use. I agree to assume and accept any and all known and unknown risks of injury to Participant while using this equipment. 7. In consideration for allowing Participant to participate in the Activity, I AGREE, to the greatest extent permitted by law, TO WAIVE ANY AND ALL CLAIMS AGAINST AND TO HOLD HARMLESS, RELEASE, INDEMNIFY, AND AGREE NOT TO SUE Vail Resorts, Inc., The Vail Corporation, Trimont Land Company, Heavenly Valley, Limited Partnership, VR US Holdings, Inc., VR US Holdings II, LLC, VR CPC Holdings, Inc., VR NW Holdings, Inc., VR NE Holdings, LLC, Whistler Blackcomb Holdings Inc., Blackcomb Skiing Enterprises Limited Partnership, Whistler Mountain Resort Limited Partnership, Peak Resorts, Inc., each of their affiliated companies and subsidiaries, the resort owner/operator, land owner, activity operator, the equipment manufacturer, The Burton Corporation, Beaver Creek Resort Company, the United States, Her Majesty The Queen In Right Of The Province Of British Columbia and all their respective insurance companies, successors in interest, commercial & corporate sponsors, affiliates, agents, employees, representatives, assignees, officers, directors, and shareholders (each a “Released Party”) FOR ANY INJURY, INCLUDING DEATH, LOSS, PROPERTY DAMAGE OR EXPENSE, WHICH I OR PARTICIPANT MAY SUFFER, ARISING IN WHOLE OR IN PART OUT OF PARTICIPANT’S PARTICIPATION IN THE ACTIVITY, INCLUDING BUT NOT LIMITED TO, THOSE CLAIMS BASED ON ANY RELEASED PARTY’S ALLEGED OR ACTUAL NEGLIGENCE OR BREACH OF ANY CONTRACT AND/OR EXPRESS OR IMPLIED WARRANTY OR BREACH OF ANY STATUTORY OR OTHER DUTY OF CARE, INCLUDING ANY DUTY OF CARE UNDER THE OCCUPIERS LIABILITY ACT IN BRITISH COLUMBIA. I UNDERSTAND THAT NEGLIGENCE INCLUDES FAILURE ON THE PART OF ANY RELEASED PARTY TO TAKE REASONABLE STEPS TO SAFEGUARD OR PROTECT AGAINST THE RISKS, DANGERS AND HAZARDS OF THE ACTIVITY.

In further consideration for allowing Participant to participate in the Activity, I FURTHER RELEASE AND GIVE UP ANY AND ALL CLAIMS AND RIGHTS THAT I MAY NOW HAVE AGAINST ANY RELEASED PARTY AND UNDERSTAND THIS RELEASES ALL CLAIMS, INCLUDING THOSE OF WHICH I AM NOT AWARE, THOSE NOT MENTIONED IN THIS RELEASE AND THOSE RESULTING FROM ANYTHING WHICH HAS HAPPENED UP TO NOW.

8. I ALSO AGREE TO PAY ALL COSTS, INCLUDING ATTORNEYS’ FEES, INCURRED BY ANY RELEASED PARTY IN DEFENDING AN INVESTIGATION, CLAIM OR LAWSUIT BROUGHT BY OR ON PARTICIPANT’S BEHALF WHETHER ARISING IN WHOLE OR IN PART FROM PARTICIPANT’S PARTICIPATION IN ANY ACTIVITY OR FROM ANY MISREPRESENTATIONS OR FRAUDULENT EXECUTION OF THIS AGREEMENT. 9. I represent that Participant is in good health and that there are no special problems associated with Participant’s physical or mental condition. I authorize a licensed physician or other medical care provider to carry out any emergency medical care for Participant which may be necessary and agree to be fully responsible for any costs associated with such care or transport to such care. 10. I grant the Released Parties the right of publicity to own and use any image collected of Participant while participating in the Activity. 11. I agree that any and all claims for loss, injury and/or death arising from Participant’s participation in the Activity shall be governed by the law of the State or Province where the alleged incident occurred and that EXCLUSIVE JURISDICTION of any such claim shall be in a court of competent jurisdiction in the in the State or Province where the alleged incident occurred, except that all cases arising out of an alleged incident at (a) Heavenly Mountain shall be governed by California law and the exclusive jurisdiction of any such claim will be a California court of competent jurisdiction; and (b) Hunter Mountain shall be governed by New York law and the exclusive jurisdiction of any such claim will be in the Supreme Court of the State of New York, County of Greene or in the US District Court for the Northern District of New York. 12. BY SIGNING ON BEHALF OF A MINOR/INFANT OR OTHER PARTICIPANT, I REPRESENT THAT I AM AUTHORIZED TO SIGN ON PARTICIPANT’S BEHALF and/or I AM THE PARENT OR LEGAL GUARDIAN OF THE MINOR/INFANT PARTICIPANT and acknowledge that Participant is bound by all the terms of this Agreement. I understand that the minor/infant Participant would not be permitted to take part in any of the Activities unless I agree to the terms of this Agreement. By signing this Agreement without a parent or legal guardian’s signature, I represent, under penalty of fraud that I am at least 18 years old (US) or 19 years old (Canada). I understand that a minor/infant Participant may use the ski lifts without an adult present or may ride the ski lifts with non-employee guests. 13. I understand that this Agreement will apply for each and every day Participant participates in any Activity during the applicable operating season. I understand that this Agreement is a contract and, to the fullest extent permitted by law, shall be binding on me and my assignees, subrogors, distributors, heirs, next of kin, executors and personal representatives. If any part of this Agreement is deemed to be unenforceable, the remaining terms shall be an enforceable contract between the parties. 14. FOR WILMOT MOUNTAIN ONLY: I understand that, for a fee per person per day in addition to the normal price, Wilmot Mountain offers an optional Agreement that does not require me to sign a Release of Liability. In signing this Release of Liability, I acknowledge I am aware of this option, do not wish to pay this fee, accept the full scope of this Release of Liability and hereby waive my right to purchase the same.
