Whilst it is thought that emergency admission to hospital on the weekend is associated with an increased risk of death, the extent to which this applies to elective admissions is less well known. We investigated the risk of death in elective and elective patients admitted over the weekend versus the weekdays. Methods: Retrospective statistical analysis of routinely collected acute hospital admissions in England, involving all patient discharges from all acute hospitals in England over a year (April 2008-March 2009), using a logistic regression model which adjusted for a range of patient case-mix variables, seasonality and admission over a weekend separately for elective and emergency (but excluding zero day stay emergency admissions discharged alive) admissions. Results: Of the 1,535,267 elective admissions, 91.7% (1,407,705) were admitted on the weekday and 8.3% (127,562) were admitted on the weekend. The mortality following weekday admission was 0.52% (7,276/1,407,705) compared with 0.77% (986/127,562) following weekend admission. Of the 3,105,249 emergency admissions, 76.3% (2,369,316) were admitted on the weekday and 23.7% (735,933) were admitted on the weekend. The mortality following emergency weekday admission was 6.53% (154,761/2,369,316) compared to 7.06% (51,922/735,933) following weekend admission. After case-mix adjustment, weekend admissions were associated with an increased risk of death, especially in the elective setting (elective Odds Ratio: 1.32, 95% Confidence Interval 1.23 to 1.41); vs emergency Odds Ratio: 1.09, 95% Confidence Interval 1.05 to 1.13). Conclusions: Weekend admission appears to be an independent risk factor for dying in hospital and this risk is more pronounced in the elective setting. Given the planned nature of elective admissions, as opposed to the unplanned nature of emergency admissions, it would seem less likely that this increased risk in the elective setting is attributable to unobserved patient risk factors. Logistic regression was used to test for differences in the mortality rate between the two groups, and propensity score adjustments were made. Results: A total of 7442 surgeries were identified, of which, 1386 (19%) took place on the weekend. Of the 947 emergency surgeries, 25% (235) were performed on the weekend. The mortality following emergency weekday surgery was 21‰ (15/712), compared with 55‰ (13/235) following weekend surgery.

Methods: This qualitative study included 22 focus groups within acute wards with a weekend allied health service and 11 telephone interviews with weekend service providers. Data were collected from 210 hospital team members, including 17 medical, 97 nursing, and 96 allied health professionals from two Australian tertiary public hospitals. All were recorded and imported into nVivo 10 for analysis. Thematic analysis methods were used to develop a coding framework from the data and to identify emerging themes. Results: Key themes identified were separated into issues perceived as being enablers or barriers to the effective or cost-effective delivery of weekend allied health services. Perceived enablers of effectiveness and cost-effectiveness included prioritizing interventions that prevent decline, the right person delivering the right service, improved access to the patient’s family, and ability to impact patient flow. Perceived barriers were employment of inexperienced weekend staff, insufficient investment to see tangible benefit, inefficiencies related to double-handling, unnecessary interventions and/or inappropriate referrals, and difficulty recruiting and retaining skilled staff.

Holena* MD, MSCE University of Pennsylvania Introduction: Differential patient outcomes based on weekday or weekend patient presentation (i.e. the “weekend effect”) have been reported for several disease states. Failure to rescue (FTR, the probability of death after a complication) has been used to evaluate trauma care. We sought to determine whether the weekend effect impacts FTR across a mature statewide trauma system. Methods: We examined all 30 Level I and II trauma centers using the Pennsylvania Trauma Outcomes Study (PTOS) from 2007-2015. Patients age >16y with a minimum Abbreviated Injury Score 2 were included; burn patients and transfers were excluded. Our primary exposure was first major complication timing (weekday vs weekend), FTR was the primary outcome. We used multivariable logistic regression to examine the association between weekend complication occurrence and mortality. Results: Of 178,602 patients, 15,304 had a major complication [median age 58 (IQR 37-77) years, 68% male, 89% blunt injury mechanism, median injury severity score (ISS) 19 (IQR 10-29)]. Patient characteristics by complication timing were clinically similar (Table). Major complications were more likely on weekdays than weekends (9.3% vs 7.1%, p<0.001). Pulmonary and cardiac complications were most common in both groups (Table). Death occurred in 2,495 of 15,304 patients with complications, for an overall FTR rate of 16.3%.
