Summary of Field Note Categories 93 10 ABSTRACT This exploratory study used Diagnostic Content Validity (DCV) and the Clinical Diagnostic Validation (CDV) models proposed by Fehring (1986) to clinically identify and validate the defining characteristics for Alteration in Tissue Perfusion: Cardiac. The literature based Kelly Cardiac Assessment Tool (KCAT) was designed as the data collection tool. The diagnostic content validity of the KCAT was 0.70. Twenty subjects, 18 years old and older were selected from a population who were admitted as inpatients in a southwestern university affiliated hospital. Data were collected through patient interviews, independent nurse assessment, and review of laboratory data. Using the steps described in Fehring*s CDV model (1986) one major defining characteristic and 13 minor defining characteristics were clinically validated. The tool CDV score was 0.62. The nursing diagnosis Alteration in Tissue Perfusion: Cardiac was clinically validated and one major and 13 minor defining characteristics were identified. 11 CHAPTER 1 INTRODUCTION The leading cause of death in the United States is myocardial infarction. This year an estimated 1,500,000 people will have a myocardial infarction and of those about 540,000 persons will die. There are more than 300,000 new cases of angina per year. Approximately 4,870,000 people today have a medical history of myocardial infarction, angina or both (American Heart Association, 1989). Nurses provide care to patients experiencing myocardial infarctions and myocardial ischemia in all stages of their disease. Nurses are involved with activities related to prevention of myocardial infarctions and ischemia, the care of patients during the acute phase,

    Cardiac Nursing - Acute/Episodic Care Career Pathway Competency Statements Cardiac Nursing Competency Statements January 2004 (Blank) 1 Foreword by Professor David Thompson In an era of the so-called ‘knowledge economy’, the concept of ‘lifelong learning’ and the belief that ‘education matters’, there is a need to ensure that skills (be they basic, core or key) as well as knowledge are developed. Indeed, Skills for Health, a government-backed body which has the support of the four UK health departments, the independent health sector, voluntary sector and staff organisations, has as its key aim the development of skills for all those people employed by the NHS or in the independent health sector. One of the key building blocks for Skills for Health is the use of ‘competency frameworks’. These frameworks identify the skills, experience and training which will help ensure consistency and quality across healthcare in the UK. The frameworks comprise a set of statements identifying what people or teams need to know and be able to do to deliver that service. These Cardiac Nursing – Acute/Episodic Care Career Pathway: Competency Statements, developed by the British Association for Nursing in Cardiac Care (BANCC) in collaboration with the Royal Brompton and Harefield NHS Trust and Thames Valley University, are an important contribution to nursing and work in conjunction with the Skills for Health competency framework being developed for prevention, heart failure and rehabilitation in coronary heart disease. Well-defined competencies from entry to practice through to advanced practice are needed in nursing, and they need to be refined to ensure greater accountability to patients and clients and their families and carers, within the profession, in relation to other health professionals, and with regard to the various contexts of practice.

ABSTRACT Educational competency is an important professional qualification for nurses. Applying knowledge to nursing care for special patients is an integral part of professional nursing in special units in hospitals. This study was conducted to assess cardiac care nursing educational competencies using modified Delphi method. Participants were 60 CCU nurses from hospitals in Shahid Sadoughi University of Medical Sciences in Yazd selected through purposive and snowball sampling. Data was collected using semi-structured interviews during the first round and questionnaires during the second and third rounds. Data was analyzed using conventional content analysis method in the first round and quantitative descriptive analysis in the second and third rounds. For panelists the most important educational competencies to empower Cardiac Care Unit (CCU) nurses were 37 qualifications 22 including personal characteristics (tolerance, carefulness, smartness, work ethic, lifelong self-learning, self- confidence, responsibility), professional characteristics (being experienced and trained, being skilled, speed and accuracy), the ability to adapt to extreme conditions, quick and accurate forecast and management of heart problems, being interested in educating the patients, students and colleagues, having the ability to interpret ECG, control dysrhythmia, cardiac blocks and myocardial infarction.
