We hope the directory will put people in touch with the many services available in the tri-county area. Our staff is available to assist with additional questions you may have concerning benefits, programs and services for senior citizens. In the Lansing area, call (517) 887-1440. Its mission is to “promote and preserve the in- dependence and dignity of the aging population”. TCOA funds a variety of programs, such as Adult Day Care, Elder Abuse Education, Home Health Aide, Homemaker, In-Home Respite Care, Legal Assistance, Long-Term Care Ombudsman, and Volunteer Transportation. Eligibility: Individuals age 60 and older and persons with disabilities 18+ years living in Clinton, Eaton and Ingham Counties. Many programs have specific income limits. TCOA directly provides the following services: Caregiver Support Caregiver Support is available to provide counseling for caregivers and their families on planning for the future, including housing, finances and long term care options. Case Coordination Case Coordination is available for people who can benefit from limited support. A staff person conducts an assessment and can arrange in-home care for eligible persons who need some assistance with personal care, homemaking, chores or respite. Crisis Services for the Elderly Crisis Service responds to non-medical emergencies, such as utility shut off notices and prescription drug issues, for older adults 60+ in the tri-county area 24-hours a day, seven days a week. Staff will assess the situation and help arrange necessary services. After hours number: 800-282-7622. Housing Assistance Housing information is available for individuals 60 and older and for persons with disabilities age 18 and older. The housing specialist assists individuals transitioning from a nursing facility back into the community by locating suitable housing. Information on avoiding evictions, resolving tenant landlord issues and locating new housing can also be provided. Information & Assistance Staff provides information that relates to older adults and persons with disabilities on community resources and supports in the tri-county area. Options Counseling Offers older adults and their caregivers assistance in planning to meet needs that arise as individuals seek to remain in the community as they age. This can include identifying needs, providing information regarding community resources and creating a plan of action. Options Counseling can take place through both phone and in-person conversations and has no income eligibility requirement. Tri-County Office on Aging – Direct Services cont. 4 Kinship Care/Respite Program Provides caregivers (55+) with a break from the responsibilities of caring for children 18 years of age or younger. Persons must meet specific eligibility criteria. Michigan Medicare Medicaid Assistance Program (MMAP) Counselors can help you to understand Medicare & Medicaid enroll in Medicare prescription drug coverage, review supplemental insurance needs, apply for Medicaid or Medicare Savings programs, identify and report fraud and abuse or scams, and explore long term care insurance. Call 800.803.7174 for more information. Project Choices (MiChoice) Offers an alternative to nursing home care. A Nurse and Social Worker team assist eligible individuals with their families and friends, to design a plan of care. Coordinating and providing key services, while respecting each individual's right to choose their own options, allows clients the opportunity to remain at home or live in the setting of their choice. The Nursing Facility Transition Service Provides options to nursing facility residents by offering an alternative to institutional care. Nursing facility residents who have a
