V. MINE SITE INFORMATION a. Description of current land use activities at the Mine Site:

No current land uses at the site, there is evidence that some recreational users dirt bike on and around the site without permission.

b. Describe the telephone service at mine site (land-line, cell phone coverage, cell phone booster station), including where the location of any land lines or location of cell phone coverage.

No reception at or near site. There is spotty reception uphill from adit 10 if you keep driving up the road towards camp 2 pit. In order to get consistent cell phone reception, you must drive back down New Idria Rd to Panoche to reach reception. (See VIII)

c. Description of Mine Site features that could pose a threat to the environment:

• Known (AMD) from Adit 10. • Very significant tailings/calcine/waste rock piles throughout site contain elevated metals concentrations, particularly mercury. • Surface water at and near site is acidic and contaminated with mercury and other metals.

d. Description of site access constraints:

High clearance, 4WD vehicle recommended. Parts of the site are protected by chain link fencing with locked gates.

e. Description of potential hazards for first responders: Investigations into the extent of contamination are not complete but work completed up to this point show widespread mercury contamination at this site. Use caution. • Structures have not been maintained and are not structurally sound. Do Not Enter. • Large tailings/calcine/waste rock piles cover site. Assume all soil contacted contains mercury unless sampling demonstrates otherwise. See XIII:Photos • Furnace and vent stack from the furnace are known to be a source of airborne mercury. Monitoring equipment such as a mercury vapor meter must be used if working within the fence line in the vicinity of these locations. • Surface water at and near site is contaminated with mercury and other metals. Do not use or drink water unless sampled and approved. Not an appropriate source of firefighting water due to low pH and high levels of contaminants

VI. EXISTING & POTENTIAL DISCHARGES AND SURFACE WATER FEATURES (attach additional pages if needed) Description of existing and potential discharges to water from the Mine Site. Description of downstream surface water bodies that could be affected by these discharges.

AMD from adit 10 is discharging into a limestone filled settlement pond and discharges from the pond into the San Carlos Creek. It is unknown at this time which downstream water bodies are affected by these discharges.

VII. KNOWN DOWNSTREAM DOMESTIC WATER USERS WITHIN ONE MILE OF MINE SITE None known. Local residents have alternative water supplies.
