a) Main objectives

General objectives: To improve the quality of life and health of populations affected by diabetes, HIV and TB by consolidating an integrating multi-actor approach for the prevention and management of Diabetes/TB and Diabetes/HIV comorbidities in Mali, Burkina Faso Faso and the Union of the Comoros.

Specific objectives (SO): • SO1: Strengthen political, strategic and coordination frameworks for the prevention and integrated management of Diabetes/TB and Diabetes/HIV comorbidities in the 3 countries (Mali, Burkina Faso and Union of the Comoros) • SO2: Strengthen the initial training and continuing education of health professionals for the management and therapeutic education of patients with comorbidities (Diabetes/TB, Diabetes/HIV) in the 3 countries (Mali, Burkina Faso and Union of the Comoros) • SO3: Strengthen community prevention actions among PLWHIV, people with diabetes and TB carried out with CSOs and their community networks in the 3 countries (Mali, Burkina Faso and Union of the Comoros) • SO4: Develop transversal actions of capitalization and communication based on the methodologies developed and the results obtained in the three countries (Mali, Burkina Faso and Union of the Comoros)

1 b) Direct beneficiaries

• 9 national diabetes, TB and HIV national programs (3 per ) • 3 CCMs (1 per country) • 72 patient associations and CSOs • A community network of 48 community PEPs trained in the 3 countries • 60 students in medical specialization • 616 healthcare professionals working in 2nd and 3rd referral structures in 3 countries • In total, 5,000 patients with diabetes and TB or diabetes and HIV to access better quality of care in the three countries • 35,000 people with diabetes (15,000 in Mali, 5,000 in Burkina Faso and 1,000 in the Union of the Comoros) have access to awareness raising on HIV and TB • 21,000 PLHIV (10,000 in Mali, 10,000 in Burkina Faso and 1,000 in the Comoros) access awareness raising on TB and diabetes risk factors.

c) Main partners

This project will be carried out with the Ministries of Health (MoH) of the 3 countries, the national hospitals (in particular the national reference services for the management of the 3 pathologies of the CHU of Yalgado (), of the hospital of Mali (), the CHU du point G (Bamako) and the National Hospital El Maarouf (Union of the Comoros), patient associations, CCMs and the national NGO Walé.
