MATERIALS AND METHODS: This was a case-control study of 402 patients, which compared the presurgical pituitary MR imaging features of patients with nonsecreting pituitary adenoma and controls with AH. MR images were compared on the basis of 16 morphologic features besides sex, age, and relation to pregnancy. RESULTS: Only 2 of the 19 proposed features tested lacked prognostic value. When the other 17 predictors were analyzed jointly in a multiple logistic regression model, 8 (relation to pregnancy, pituitary mass volume and symmetry, signal intensity and signal intensity homogeneity after gadolin- ium administration, posterior pituitary bright spot presence, stalk size, and mucosal swelling) remained significant predictors of a correct classification. The diagnostic score had a global performance of 0.9917 and correctly classified 97% of the patients, with a sensitivity of 92%, a specificity of 99%, a positive predictive value of 97%, and a negative predictive value of 97% for the diagnosis of AH. -

However, the facial changes associated with acromegaly can be permanent and cause distress and concern to patients. Case History: We present the case of an acromegalic women, previously treated for hyperpituitarism, pre- senting with persistent facial changes and a large nose. This was successfully addressed with rhinoplasty, clinical photography is provided. Discussion: The nasal changes associated with acromegaly are challenging but can be successfully treated with rhinoplasty. We discuss the few cases previously mentioned in the literature and the pathophysiology involved in the changes of facial appearance found in acromegalic patients. Management of Sheehan’s is appropriate replacement of hormones, in LH - replacement hormones and in some circumstances, steroids and surgical biopsy. The key of course is being seen by an endocrinologist with expertise in pituitary and not accepting the overwhelming features of hypopituitarism as just ‘normal’. If features of Diabetes Insipidus are present the diagnosis is usually easier, as severe thirst and passing copious amounts of urine will be present. Sheehan’s syndrome Sheehan’s syndrome is a rare condition in which severe bleeding during childbirth causes damage to the pituitary gland. The damage to pituitary tissue may result in pituitary hormone deficiencies (hypopituitarism), which can mean lifelong hormone replacement. What causes Sheehan’s syndrome? During pregnancy, an increased amount of the hormone oestrogen in the body causes an increase in the size of the pituitary gland and the volume of blood flowing through it. This makes the pituitary gland more vulnerable to damage from loss of blood. If heavy bleeding occurs during or immediately after childbirth, there will be a sudden decrease in the blood supply to the already vulnerable pituitary gland. -

After an uneventful third delivery, 24 years before admission, she could not lactate and developed oligomenorrhea and then amenorrhea. Laboratory evaluation revealed panhypopituitarism and pituitary cell antibodies were positive. Both CT scans and MR images showed empty sella. Whereas descriptions of giants have existed in the lay literature for millennia, the first attempt at a medical description was published by Johannes Wier in 1567. How- ever, it was Pierre Marie, in 1886, who established the term “acromegaly” for the first time and established a distinct clinical diagnosis with clear clinical descriptions in 2 patients with the characteristic presentation. Multiple autopsy findings revealed a consistent correlation between acromegaly and pituitary enlargement. In 1909, Harvey Cushing postulated a “hormone of growth” as the underlying pathophysiological trigger involved in pituitary hypersecretion in patients with acromegaly. This theory was supported by his observations of clinical remission in patients with ac- romegaly in whom he had performed hypophysectomy. In this paper, the authors present some of the early accounts of acromegaly and gigantism, and describe its historical evolution as a medical and surgical entity. There are, for this reason, undoubtedly innumerable states, some pathological, others bordering upon the pathological, in which dysfunction of the pituitary gland is involved. However, our present state of knowledge of pituitary dysfunction without demonstrable morphologic changes in the pituitary gland is such that it is dangerous speculation to attempt to ascribe many bizarre conditions, as claimed by uncontrolled and popular endocrinology, to improper secretion of the pituitary gland. Indeed, there must be a considerable margin of safety in the quan- tity of functioning glandular tissue of the anterior lobe of the pituitary body since one not infrequently sees patients with partial destruction of this organ by tuberculosis, syphilis or embolic phenomena without any detectable symptoms ascribable to loss of pituitary secretion. However, sufficient experimental data and clinical and pathological evidence exist to make possible the recognition of two important classes of pituitary disturbances, namely, hyperactivity and hypofunction of this gland. Of all the verified diseases of the pituitary gland producing one or the other of these groups of symptoms, the commonest is the adenoma- tous tumor.2 The type of clinical response to such a tumor varies with the type of cell from which the tumor arises. The aim of management is usually to replace the target-hormone of hypothalamo-pituitary-endocrine gland axis with the exceptions of secondary hypogonadism when fertility is required, and growth hormone deficiency (GHD), and to safely minimise both symptoms and clinical signs. Adrenocorticotropic hormone deficiency replacement is best performed with the immediate-release oral glucocorticoid hydrocortisone (HC) in 2–3 divided doses. However, novel once-daily modified-release HC targets a more physiological exposure of glucocorticoids. GHD is treated currently with daily subcutaneous GH, but current research is focusing on the development of once-weekly administration of recombinant GH. Hypogonadism is targeted with testosterone replacement in men and on estrogen replacement therapy in women; when fertility is wanted, replacement targets secondary or tertiary levels of hormonal settings. Thyroid-stimulating hormone replacement therapy follows the rules of primary thyroid gland failure with L-thyroxine replacement. -
