This is your community service log. To maintain membership in NHS you need to complete 15 service hours per semester. Five of these hours must be with NHS sponsored activities, and ten must be independent activities done by you (can also be NHS sponsored). You must turn in the service log on the first and second semester due dates to maintain standing in NHS.

Your Service Log must be signed by a Supervisor or by an NHS officer who witnessed the service to be valid. Please DO NOT have parents sign off your volunteer hours, even if your parent was present at the event. Instead, please have another adult supervisor sign off your hours. Please make sure it is filled out thoroughly and correctly- see reverse for an example 
