BackPack Program Agreement 2017-2018

The purpose of the BackPack Program is to help solve child hunger by providing nutritious and easy-to-prepare food to children to take home on weekends and school vacations when other resources are not available.

This document is an agreement made between the Northeast Iowa Food Bank (hereinafter referred to as “NEIFB” and the above mentioned locations (hereinafter referred to as “Program”). By signing this agreement, both parties acknowledge their respective duties and responsibilities related to the administration of the BackPack Program.

A. Program Partner AGREES TO: • The safe and proper handling of food, which conforms to all local, state and federal regulations. • Willingness to abide by the policies, procedures, and record keeping requirements of the NEIFB. • Staff or volunteers of the program will not engage in discrimination, in the provision of service, against any person because of race, color, citizenship, religion, gender, national origin, ancestry, age, marital status, disability, sexual orientation including gender identity or expression, unfavorable discharge from the military or status as a protected veteran. • Distribute healthy, nutritious food to children free of charge. • Provide backpacks a minimum of once a month while the program is in operation. • Ensure the school and/or program complies with all applicable federal and local statutes, ordinances and regulations. • Ensure staff and volunteers with direct repetitive contact with children pass a national background check. • At least one representative must receive some form of food safety training, such as ServSafe Food Handler for Food Banking developed by Feeding America and the National Restaurant Association, or an equivalent training. • Be available for an on-site monitoring visit at least once every two years from the NEIFB or more as needed. • Store BackPack Program food in a secure, sanitary and temperature controlled place away from cleaning materials and toxic chemicals. All food must be stored 6 inches off the floor, away from the wall and 6 inches below the ceiling. • Distribute the BackPacks to program participants in accordance with the pre-determined schedule. • Keep accurate records, submit reports and provide necessary information as requested. Keep registration forms on hand for all students participating in the program. • Identify a BackPack coordinator to be the primary contact for the BackPack Program. • Receive deliveries on the designated day and time, and provide help in unloading the deliveries. • Inform the NEIFB in writing of any changes in BackPack Program personnel, days/hours of operation and/or number of children being served. • Encourage families, children and staff to complete evaluations of the program as requested by the NEIFB. • Identify children that are chronically hungry to participate in the BackPack Program. • The Northeast Iowa Food Bank is the fiscal agent for the program. Any fundraising and promotion of the program needs to be done in partnership with both entities – this includes any use of our logo, program logos or any information about the Food Bank and its programs. Please call the Food Bank prior to promoting the program. • Must attend the annual BackPack Program Coordinator Meeting. • Use school records to identify food allergies children participating in the BackPack Program may have (peanuts, milk, etc.) and flag their name so those items are not distributed to them. • Communicate problems and requests to the NEIFB in a timely manner. • Failure to comply with any part of this agreement will result in a warning. Notification of a warning will be provided in writing. After three warnings are received, the site will closed and must wait six months to reapply to become a BackPack Program site.

The Program understands that they will receive food and backpacks from the NEIFB at no cost to their school/site/agency to help them provide food to children in need. They also understand that the food and backpacks cannot be sold, used for other school programs, fundraisers, given to staff, or used for any other purpose other than to provide food to hungry children.

This agreement may be terminated at will by either party with written notice delivered to either party not less than 30 days prior to the desired date. Upon termination of this agreement, the Site Coordinator will return any equipment and/or materials provided by the NEIFB for the BackPack Program to the NEIFB within 30 days of the program termination date. This agreement will be effective for one year from the date signed and renewed for each year of participation in the program.

The NEIFB strives to include healthy and safe food. While some products might be past the marked “best by” date, we have worked with the manufacturer to ensure it is still safe to consume. If you open the package and notice a problem, please contact the Northeast Iowa Food Bank immediately.
