Notes of the meeting held Wednesday 22nd July, Ludgvan Community Centre

1. Generic referral form
Scott Bennett is sorting out the drop down options for the form
Lucy and Ally seeking authorisation from the Professional Practice Forum on the 10th September
Form will be sent by email, not by fax
Has gone through the PCH information governance route
Will need some communication around this before it is used – for those who aren’t aware of this group
Current processes will still work – e.g. when a GP telephones a DN for an urgent visit
Going to the Penwith Locality Group on the 29th July
Aware that it might lead to delays in responses being identified
Agreed that service would respond to referrer if response time can’t be met, but will pass on the form to the appropriate service directly, without it having to go back to the referrer
Agreed to trial the form and keep it on the agenda
Add ‘please reply to…’ box
Add guidance notes to the back of the form, e.g. define ‘urgent’
Add Living Well logo

2. East locality practitioner workshops
Workshops were held across the East locality recently in each of the cluster areas Issues/barriers and solutions were shared with the Penwith Practitioners for information
This was particularly useful for RCHT staff who are looking to develop a similar process internally
The aim is to apply the core principles of Living Well in the wider roll-out, facilitating a local approach, rather than being prescriptive about the model.
3. Community Line
A concept developed by this group, originally intended as a single contact point for the Living Well voluntary sector team and local community activities
Running since January 2015, staffed by an administrator funded by Age UK, staffed 9-1, Monday to Friday
30 calls received to date, mostly from community therapy teams
Number has been changed since January
Not widely promoted initially – only to GP practices
Chimney sweep example: a person rings for a chimney sweep because she can’t get down to sweep out her fireplace any more. Guided conversation approach would be to find out why she can’t get down and why she doesn’t have anyone to ask for help
Group discussion on how it’s going and next steps:
o Should have a guided conversation approach – training?
o Helpful to link to other voluntary sector services
o MDT meetings are also helping to share information
o For people and practitioners
o Co-ordination gap
o Need for central repository
o Make every contact count – e.g. chimney sweep
o GP practices were sent details but no real awareness or sense of how it is being used
o Be clear it is connecting people and/or practitioners to their local community

4. Update from practitioners
Jane Royle is now the West team leader for the Cornwall Carers Service – there are 7 support workers in the West and can help with carers assessments, carers grant applications, respite care and social support - http://www.cornwallcarers.org.uk/
Alex runs a falls clinic that has moved to Penzance leisure centre – she is looking for a base one day a month in Penwith and offered to talk to practitioner groups/MDT meetings about what the service offers. She can be contacted on [email protected]
Carolyn read out two case studies, the first about a person’s experience of joining an art group; the second about a gentleman who had repeated falls – to be forwarded to the group and put on the Knowledge Bucket
Tina gave an update on the Welcome Home project at West Cornwall Hospital, which continues to go from strength to strength, demonstrating significant reductions in length of stay and repeated admissions. The Welcome Home/Living Well team are involved on a daily basis. Staff morale is excellent on the wards – there is even a waiting list for bank staff shifts
Jane Ansell reported that there have been nearly 400 referrals to Welcome Home from RCHT and WCH so far and a groundswell of interest in Living Well from ward staff Matthew reported that there is a project underway to put care plans online so that they can be updated by the person and practitioners and kept as a live document. Matthew also reported that a local anonymous benefactor has funded an additional Living Well Co- ordinator post to be based mainly at WCH.
