FEDERAL CODES AND REGULATIONS IN THE FIELD OF ATOMIC ENERGY USE "STANDARD CONTENT OF ACTION PLAN FOR THE PROTECTION OF PERSONNEL IN THE EVENT OF ACCIDENT AT NUCLEAR POWER PLANT" (NP-015-12)

I. Purpose and scope

1. The Standard Content sets forth the main requirements for the content of action plans for the protection of personnel in the event of an accident at a nuclear power plant (hereinafter "Protective Action Plan"). 3. The Protective Action Plan provides for scope, timing and order of measures to protect personnel at the nuclear power plant in the event of a threat and occurrence of natural and man-made emergencies, organization and conduct of rescue and other emergency operations, and defines manpower and resources to be involved. 4. The Protective Action Plan is developed by the administration of the nuclear power plant in conformity with Provisions on the unified state system for prevention and elimination of emergency situations approved by Decree of the Government of the Russian Federation No.794 of December 30, 2003.(Collection of Legislative Acts of the Russian Federation, 2004, No.2, art. 121; 2005, No.23, art.. 2269; 2006, No.41, art.. 4256; 2008, No.47, art. 5481; 2009, No.12, art. 1429; No.29, art. 3688; 2010, No.37, art. 4675; No.38, art. 4825; 2011, No.7, art. 979, 981; No.14, art. 1950; 2012, No.1, art. 145; No.17, art. 2009; No.37, art. 5002; No.44, art. 6026; No.46, art. 6339), and this Standard Content based on radiological consequences of a most severe beyond design basis accident considered by the designers of the nuclear power plant taking into account the "early, middle and late" phases of the accident, as well as other factors of natural and anthropogenic origin that fall under the criteria of emergency. 5. The Protective Action Plan developed at the nuclear power plant consists of the main body text and appendices. The text of the main body is composed of eleven sections: 1) General provisions; 2) Brief geographical and social-economic characteristics of the nuclear plant location area; 3) Basic data for planning personnel protection measures; 4) Brief assessment of possible situation at the nuclear plant in the event of an emergency; 5) The main criteria for announcing "Emergency Preparedness" or "Emergency Environment" state (bringing in force the modes of functioning of the system for prevention and elimination of emergency, such as "Increased Preparedness" or "Emergency Situation"); 6) organization of warning, communication and information exchange; 7) the procedure for alerting and deployment of control bodies of the unified state system for prevention and elimination of emergency situations (hereinafter the "Control Bodies"), control and monitoring manpower and resources, manpower and resources for elimination of emergency situation; 8) measures to protect personnel in case of announcement of a state "Emergency Preparedness" ("Increased Preparedness" functioning mode of the system for prevention and elimination of emergency situations); 9) measures to protect personnel at the announced state "Emergency Environment" ("Emergency Situation" functioning mode of the system for prevention and elimination of emergency situations); 10) elimination of emergency situations at the nuclear power plant, involved manpower and resources; 11) support of actions of manpower and resources involved for elimination of emergency situations at the nuclear power plant. 6. The Protective Action Plan should be signed by Director of the nuclear power plant, coordinated with the heads of territorial bodies of the Ministry of Emergency Situations of Russia, the Federal Security Service of Russia, Russian Ministry of Internal Affairs, Federal Bio-Medical Agency of Russia, Hydrometeorology and Environmental Monitoring Agency, organization - the developer of the nuclear power plant design, and submitted for approval to the operating organization. 7. The Protective Action Plan should be developed before delivery of nuclear fuel to the first unit of the nuclear power plant prior to its physical start-up, harmonized, approved and provided with material and financial resources. It should reflect the specific features of the reactor facility and the location of the nuclear power plant. By that time the design solutions for engineering and technical activities of civil defense should be completed at the nuclear power plant. 8. Adjustment of the Protective Action Plan should be carried out through amendments and supplements on the basis of notices approved by the operating organization. The order of harmonization and approval of amendments and supplements is similar to that of the original harmonization and approval of the Protective Action Plan. 9. The need to adjust the Protective Action Plan is understood by the results of reconstruction of the existing power units, based on the orders of the state safety regulatory authorities, with the introduction of a new regulatory documentation and by the results of training (complex exercises). 10. If there is no threat of emergency situation at a nuclear power plant, the control bodies, manpower and resources for monitoring and control, manpower and resources for elimination of emergency situations of the unified state system for prevention and elimination of emergency situations are functioning in the mode of daily activities. With the threat or occurrence of an emergency situation at the nuclear power plant site and (or) within the buffer area, following a decision of Director of the nuclear power plant there can be set one of the following modes of functioning of the emergency situations prevention and elimination system: "Increased Preparedness" mode ("Emergency Preparedness" state) - with the threat of an emergency situation; "Emergency" mode ("Emergency Environment" state) - in the event of occurrence and elimination of emergency situation. 11. Steps for the prevention and elimination of emergency situation of natural and man-made origin at the nuclear power plant should be taken in accordance with the Protective Action Plan. 12. Typical contents of the Protective Action Plan (structure, arrangement and designing of elements of the cover sheet, approvals record sheets, sections) is provided in the Appendix.

General Provisions

1.1. The action plan for personnel protection is developed taking into account the recommendations of the Ministry of Emergency Situations of Russia in planning actions for the prevention and elimination of emergency situations on site. 1.2. The requirements of the Protective Action Plan should be applicable to the NPP staff, personnel of military division (unit) of IMF and the internal affairs body of the Ministry of Internal Affairs of the Russian Federation safeguarding and servicing the NPP and on-site FFPS divisions protecting the NPP, the staff of organizations (enterprises) supporting the operation of NPP with any type of RP (during their commissioning, operation and decommissioning), the requirements should be observed at the NPP site and in BA. 1.3. Decision on the enforcement of the Protective Action Plan should be taken by Director of NPP (or a person acting for Director of NPP), and in their absence at the plant, by the SSP. 1.4. The Protective Action Plan is put into effect if a decision is made to announce the "Emergency Preparedness" state and (or) "Emergency Environment" state as well as under the influence of other natural and man-made factors covered by the emergency criteria, and under the threat or at the occurrence of terrorist acts.

2. Brief description of NPP and NPP location area

2.1. NPP brief description

To be given power unit type, number and electrical power, the main and backup power supply of NPP systems and equipment, a brief description of the main production buildings and structures indicating the highest and lowest number of employees in them and operating conditions, including personnel of the military division (unit) of IMF forces of the Ministry of Internal Affairs of Russia guarding the NPP, personnel of FFPS unit, transport network, hydraulic engineering structures, characteristics of vent pipes for the removal of gaseous and aerosol media. NPP site layout should be in accordance with Appendix No.1 to the Protective Action Plan.

2.2. Brief description of NPP location area

2.2.1 Brief description of the NPP satellite town (area, population, list of main industrial enterprises, distance from the NPP site, potential for warning, sheltering and evacuation of NPP personnel, road and railroad network, water supply, transportation communications between the power plant and the NPP satellite town). NPP satellite town layout should be in accordance with Appendix No.2 to the Protective Action Plan. 2.2.2 Brief description of the features of the environment and climate at NPP location: characteristics of the landscape, surface and groundwater, meteorological conditions (wind speed and direction, their frequency of occurrence, the air temperature seasonal), seismic and geodynamic characteristics of the site, the phenomena and factors of natural origin found in the NPP location area that may cause undesired operational occurrences at NPP and addressed in the NPP design by RP (NPP) designers. 2.2.3 Brief description of BA and RCA (the boundaries and dimensions of the areas, their structural elements, isolation spaces, organizations engaged in production and business activities, and for BA, also the strength of their staff and modes of operation). BA and RCA layout should be in accordance with Appendix No.3 to the Protective Action Plan. 2.2.4 Radiation and chemically hazardous, as well as explosive and flammable facilities and enterprises, hydraulic engineering strictures in possible radioactive contamination area. NPP possible radioactive contamination area layout should be in accordance with Appendix No.4 to the Protective Action Plan. 2.2.5 List of settlements in RCA with the numbers of their populations, the distance from the NPP site, the possibility of warning. 3. The basic data for planning personnel protection measures

3.1. The sizes of the areas, the forecast of external and internal exposure of personnel with severe beyond design basis accidents (beyond design basis accident is determined by the NPP designers) in the worst weather conditions. The initial events, the time sequence and the possible propagation ways of the accident, the assessment of the radiation situation in the NPP compartments, at the NPP site, in BA and RCA, the assessment of activity and radionuclide composition of emergency release and discharge. Calculation of the size of the contaminated areas, of external and internal exposure at a beyond design basis accident should be in accordance with Appendix No.5 to the Protective Action Plan. 3.2. Location of the protected emergency control centers on the NPP site, in the NPP satellite town and in the NPP evacuation area. Fitting of emergency centers with soft and hardware complexes, data communication system, communication means and documentation. 3.3. Location of NPP protective structures. 3.4. Data on the number of the plant personnel to be evacuated and the staff of organizations supporting the NPP operation (hereinafter the "personnel"). 3.5. NPP evacuation areas and the personnel evacuation routes. 3.6. Data on the state of the road network, the state of transport. 3.7. Brief description of the organization of radiation monitoring at NPP site, in BA and RCA, the location of monitoring posts. ARMS availability and characteristics. 3.8. LWA presence and characteristics (key features in accordance with the data sheet and the actual coverage). 3.9. CHS name and quantity at the plant, its location, the volume, the main effects. Selection of CHS that determine the chemical environment in the accident area, its effects, features and other necessary parameters, the calculation of the size of the chemical contamination area due to CHS release (spill). Calculation and dimensioning of the expected chemical contamination area due to CHS release (spill) at NPP should be in accordance with Appendices No.6 and N 7 to the Protective Action Plan. 3.10. Adverse external impact factors of natural origin in the NPP area subject to the criteria of information on emergency situations and requiring measures to protect personnel (in accordance with the ES information criteria). 3.11. Provide personnel with individual and collective protection means (list, quantity and storage place of individual protection means, radiation monitoring instruments and dosage meters, pharmaceuticals and decontamination and sanitation means). 3.12. Financial and material resources for elimination of emergency situations.

4. A brief assessment of possible situation at NPP in case of emergency situation.

4.1. A brief assessment of possible situation at NPP site and in BA during radiation accidents: 1) a brief description of zones (protective measures planning zone, mandatory evacuation measures planning zone, controlled access zone, unrestricted access zone) allowing for NPP location features for the considered beyond design basis accident; 2) determine the number of personnel who may be exposed to radiation factors (in the territory of the NPP, BA and RCA); 3) key indicators of personnel evacuation on the basis of forecast data on radiation accident (number of evacuees, the planned evacuation areas, number of vehicles, the planned removal and evacuation time). 4.2. Estimates of the potential environment in chemical accident: 1) characteristics of the contamination zone; 2) the expected number of victims; 3) the need for protective measures. 4.3. Estimates of the potential environment at external effects taking into account the characteristics of structures of the buildings and constructions: 1) the level and nature of possible destruction, flooding, damage; 2) possible fires; 3) the need for protective measures. 4.4. Estimates of the potential environment in case of fire. 4.5. General conclusions of the assessment of possible environment at NPP in an emergency situation. General conclusions of the assessment of possible environment at NPP in an emergency situation. Projected environment as a result of emergency situation at the NPP should be entered on the Site layout, BA and RCA layout, NPP satellite town layout, the layout of possible hazardous radioactive contamination area depending on the scale of the accident, and the environment developing in the accident should be entered on the earlier prepared topographic map of the NPP location area (the diagram) or electronic versions of layouts and diagrams.

5. The main criteria for the announcement of a state "Emergency Preparedness" or "Emergency Environment" (to put in effect an EPES functional mode "Increased Preparedness" or "Emergency Situation")

The criteria for announcing a state of "Emergency Preparedness" or "Emergency Environment" (putting in effect an EPES functional mode "Increased Preparedness" or "Emergency Situation") should be established in accordance with the federal atomic codes and regulations which define the procedure of announcement of emergency environment.

6. Organization of warning, communication and information exchange

6.1. The availability of means of warning, communication and information exchange used and engaged in the threat and occurrence of emergency situations. 6.2. The functioning of communication centers and real time information transmission time. How to use the hardware, software and information tools of ES/S and ES/T, MCC, EOM with the threat and occurrence of emergency situations. 6.3. Organization of warning at NPP in case of threat and occurrence of emergency situations. Automatic warning system for members of the commission for emergency prevention and response in the operating organization (hereinafter "CEPR") and NPP management. List of users of warning system to be notified on the status of "Emergency Preparedness" and (or) "Emergency Situation" at NPP. The scheme of warning under the threat and occurrence of an accident at NPP is developed in accordance with Appendix No.8 to the Protective Action Plan. 6.4. Organization of communication with NPP manpower and facilities. The order of warning and transmission of information about the situation, the actions taken, communication with the NPP divisions during reconnaissance, bringing manpower and resources in the emergency situation area, and when conducting RUO. 6.5. Establishing communication and information exchange with the manpower and resources engaged in RUO. 6.6. Organization of informational communication with the public and mass media. The procedure for preparing messages (reports), and informing the public in case of threat and occurrence of emergency situations. 6.7. Organization of information exchange and transfer of messages between ES/S and ES/T (internal and external), PECC/EA, CC, MCC, EOM and other subscribers of emergency prevention and response system and EPES including CDCS. 6.8. The organization of information exchange between the NPP management, the operating organization, the State Corporation "Rosatom", the heads of the municipality (NPP satellite town) and the member of the Russian Federation (district), where the NPP is sited. The scheme of communication arrangement under the threat and occurrence of an accident at NPP is developed in accordance with Appendix No.9 to the Protective Action Plan. 7. The procedure of bringing into preparedness and deployment of control bodies, control and monitoring manpower and facilities, emergency situation elimination manpower and resources of the Unified State System for Prevention and Elimination of Emergency Situations

7.1. In developing the measures take into account that command of monitoring and control manpower and facilities, and emergency situation elimination forces and supervision of mitigation (elimination of emergency situations) is realized by SSP from MCR or SCP (depending on the prevailing conditions) of the emergency power unit, EOM from PECC/S or EOM MCC. In the event of adverse radiation environment in the NPP territory, control over manpower and facilities is exercised from PECC/T. 7.2. Organizational measures depending on procedure of bringing into preparedness and deployment of control bodies, monitoring and control manpower and resources, emergency situation elimination forces are the following: 7.2.1. Bringing control bodies in preparedness state. CEPR assembly place and time. SSP actions for bringing control bodies in preparedness before NPP Director arriving at the NPP. 7.2.2. Bringing PECC/S, PECC/T, PECC/EA, ES/S and ES/T into preparedness for work state. 7.2.3. The order of transfer of control between control centers, the order of setting targets to manpower. 7.2.4. Bringing into preparedness and deployment of monitoring and control manpower and resources, NPP ES elimination forces, their structure, terms of readiness, deployment places. The structure and equipment of the monitoring and control manpower and resources, NPP emergency situation elimination forces are defined by Appendix No.10 to the Protective Action Plan. The schedule of bringing into preparedness the control bodies, monitoring and control forces, NPP ES elimination forces should be developed in accordance with Appendix No.11 to the Protective Action Plan. 7.3. Management of manpower and facilities for mitigation (elimination) of emergency situation at NPP. The scheme of command of NPP emergency situation elimination manpower and facilities should be prepared in accordance with Appendix No.12 to the Protective Action Plan.

8. Personnel protection measures at the announcement of "Emergency Preparedness" state ("Increased Preparedness" EPES functioning mode)

8.1. SSP, operating personnel actions. The SSP actions schedule in case of the threat and occurrence of emergency situations at NPP is developed in accordance with Appendix No.13 to the Protective Action Plan. 8.2. Actions of NPP management. The NPP Director actions schedule is developed in conformity with Appendix No.14 to the Protective Action Plan. 8.3. The order of implementation of personnel protection measures: bring NPP manpower and resources to readiness state; bring vehicles intended for the evacuation of personnel to readiness state; provide personnel with individual protection means and radiation monitoring devices; ensure preparedness of protective structures to receive personnel; preventive fire protection measures; bring the medical protection manpower and resources in readiness for the provision of first aid to personnel (if necessary); prepare the list of officers (position, quantity, work place) who stay in the work places from the moment of accident origination till the receipt of EOM order. 8.4. The order, volume and timing of transmission of real and ongoing (more) information about the environment at the plant. Key actions schedule in case of the emergency situations at NPP is developed in accordance with Appendix No.15 to the Protective Action Plan. 9. Personnel protection measures at the announcement of "Emergency Environment" state ("Emergency Situation" EPES functioning mode)

9.1. NPP management, SPP and operating personnel actions in case of announced "Emergency Environment" state

9.1.1. SSP actions (warning, decision on the use of individual and collective protection means, iodine prophylaxis, first aid to victims, emergency evacuation). SSP actions should be developed in conformity with Appendix No.13 to the Protective Action Plan. 9.1.2. Actions of operating staff (warning about the situation of officials in the chain of command up to the SSP, rendering first aid to victims, and taking the necessary measures available to eliminate the accident). 9.1.3. The actions of NPP management in the early phase of the accident (the choice of the main measures for the protection, urgent assessment of emergency radiation doses, the use of individual protection means, provision of radiation monitoring instruments and dosimeters, determination of the order of taking shelter in the protective shelter facilities, realization of iodine prophylaxis, medical aid and other safety measures, determination of the amount, timing, forces and means to be engaged to carry the measures out.) The list of documents developed by NPP management to regulate actions of NPP management and personnel in case of a radiation accident, in accordance with Appendix No.16 to the Protective Action Plan. 9.1.4. The actions of NPP management in the middle phase of the accident (prevention of further development of the accident and accident management, localization (elimination) and mitigation of the radiological consequences of the accident, organization of medical aid, individual protection of personnel, an expanded radiation monitoring and other necessary actions). 9.2. Radiation protection

9.2.1. Measures to protect the personnel in the early phase of accident development: a) basic measures for personnel protection: sheltering of personnel; urgent evacuation; use of respiratory and skin individual protection means; iodine prophylaxis; b) emergency assessment of emergency exposure doses (who to perform, the expected doses); c) provision of first aid to the victims; d) determination (calculation) of time the staff stay on NPP site (in the area of radiation accident); e) control of radioactive contamination of clothing and skin when leaving the protective measures planning zone and evacuating personnel from NPP site; f) sanitization of personnel; g) exclusion or restriction of water and food consumption (if necessary); h) briefing the personnel on measures for radiation protection; i) distribution and use of radiation monitoring instruments and individual dosimeters; j) assessment of radiation environment, monitoring and accounting of individual exposure levels of personnel. 9.2.2. Measures to protect the personnel in the middle phase of accident development: a) justification of the expected radiation dose to exceed the specified dose limits (if necessary); b) mitigation (elimination) of radioactive contamination; c) individual personnel protection (use of individual protection means, anti-radiation medication); d) use of advanced radiation monitoring in view of the features and conditions of work performed; e) organization of control of personnel access in radioactively contaminated compartments for work; f) organization of real-time radiation monitoring and individual accounting of exposure doses of personnel engaged in RUO; g) the procedure for distributing individual protection means and dosimeters to personnel involved in conducting RUO (the inventory of individual protection means and dosimeters should be at least two sets for each participant in RUO); h) the combined use of anti-radiation medical means (if necessary); i) before work in radiation hazardous conditions, briefing of personnel on protective measures and possible individual exposure doses. Procedure for decontamination of clothing and individual protection means in case of radioactive contamination (in accordance with civil construction code SanPiN 2.2.8.46-03 "Sanitary rules for decontamination of individual protection means" approved by the Resolution of the Chief State Sanitary Doctor of the Russian Federation No.157 dated October 28, 2003 (registered with the Ministry of Justice of Russia of December 5, 2003, registration No.5298).. 9.2.3. Decontamination procedure (location of decontamination stations, clothing and footwear allowance, radiation monitoring, the number of personnel who may be subjected to decontamination, decontamination agents reserve). 9.2.4. Measures to protect personnel in the late phase of the accident (planned measures, protective measures in the course of their implementation). Measures for radiation protection of the staff are determined on the basis of the tasks accomplished in this phase of the accident, the implementation of which may harm the health of the staff: execution of emergency recovery work; decontamination of contaminated areas, buildings in the NPP territory; collection and transportation of radioactive waste; rehabilitation (if necessary) of contaminated areas. Decisions on the protective measures is taken in process of determination of tasks to be implemented in the late phase of the accident, taking into account the current radiation environment and the specific socio-economic conditions.

9.3. Engineering protection

9.3.1. Procedures for maintaining protective structures in availability for immediate reception of the staff. 9.3.2 The order of sheltering the staff (including personnel of IMF division (unit), the internal affairs body and FFPS units) in the protective shelter facilities in radiation accidents and other emergency situations, and also sheltering staff to be sheltered in other compartments. Calculation of on-site sheltering of personnel and staff of other organizations should be in accordance with Appendix No.17 to the Protective Action Plan. 9.3.3 The order of submission of reports on engineering protection activities.

9.4. Physical protection

9.4.1. Planning of organizational and engineering activities and also operations of the NPP security forces including their partial reorganization and the order of commitment of reserve forces in IMF divisions of the RF Ministry of Internal Affairs guarding the NPP and departmental security divisions which include: 1) providing for immediacy of controlled access of emergency rescue units to the guarded territory of the NPP; 2) assistance to unimpeded evacuation of personnel from the NPP territory and structures through all check points and posts; 3) protection of personnel of external security posts; 4) establishment of mobile reserve force for reinforcing or relieving the guard; 5) assurance of reliable control of NPP security from protected control post; 6) prevention of unauthorized access; 7) timely detection of unauthorized activities; 8) impeding the entry of the offender; 9) suppressing unauthorized actions; 10) detention of persons involved in the preparation or execution of diversion or theft of nuclear materials and logistical assets; 11) prevention of terrorist attacks. 2.4.9 Manpower and facilities, timing, and persons responsible for the conduct of measures referred to in paragraph 9.4.1. 9.4.3. The real time information on the NPP special safety is transferred by a security service shift supervisor to the IRD duty officer. In the development of subsection 9.4 you should take into account the requirements of the Rules of physical protection of nuclear materials, nuclear facilities and nuclear materials storage points (paragraph 23), approved by Decree of the Government of the Russian Federation No.456 of July 19, 2007 (Collection of Legislative Acts of the Russian Federation, 2007, No.31, art. 4081; 2009, No.18, art. 2248; 2010, No.38, art. 4825; 2011, No.7, art. 979; No.21, art. 2961), which obliges the NPP management to ensure the development of: NPP security plan; Action Plan for the physical protection personnel and NPP personnel; Plan of Interface of the NPP Management, IMF divisions (units) of the Ministry of Internal Affairs of the Russian Federation with the law enforcement agencies of the Russian Federation and the bodies of the Federal Security Service of Russia in routine and emergency situations. In the development of the Protective Action Plan, one can use data from the above- mentioned plans, with the exception of information constituting a state secret.

9.5. Evacuation measures

9.5.1. Procedure for emergency evacuation of personnel from the NPP site and BA. 9.5.2. The order of pre-evacuation: a) evacuation authorities and procedures to bring them in readiness; b) supervision of the evacuation of personnel from NPP site and BA, establishing control of the evacuation of personnel and members of their families from the NPP satellite town and the level of involvement of the NPP management; c) the decision to conduct evacuation measures and the order of their implementation (the right to make a decision, criteria); d) timing of evacuation; d) personnel embussing points (in a normal working day and at night (weekends) separately); e) organizing the evacuation of personnel (notification on the start of evacuation and briefing, communication facilities and organization, embussing procedure, IEP and EAP locations, evacuation routes, disembarkation points, organization and procedure of sanitization of evacuated personnel at decontamination points); f) basic data on the types of support of evacuation, warning and communications (medical, transportation, logistics, public safety, engineering, reconnaissance, utilities); g) acquisition and collation of evacuation progress data and transmission of the reports to the control bodies. NPP personnel evacuation measures schedule is shown in Appendix No.18.

9.6. Medical protection

9.6.1. Medical protection manpower and facilities and the order to bring them in readiness. 9.6.2. The order of medical protection depending on the phase of the accident. In an early phase of an accident: a) provision of first aid, first physician and specialist care; b) provision of iodine prophylaxis, the use of anti-radiation medication in a radiation accident; c) for the affected by life-saving indications, time and place of evacuation (vehicles used for evacuation of injured personnel should be equipped with radiation control instruments); d) delivery to hospitals of persons with traumatic injuries, burns, and those received the dose above 200 mSv, and their examination; e) urgent hospitalization of the injured exposed to radiation dose above 1 Sv; f) medical support for personnel in the shelters or in other protective structures; g) appointment of assembly places for rendering the first medical aid to the injured personnel; h) medical support of evacuated personnel. In a middle phase of an accident: a) The combined use of medical anti-radiation drug for work in hazardous radiation conditions (if necessary); b) medical support of RUO; c) the provision of expert assistance to victims; d) medical support for evacuated personnel; e) medical examination of persons who have been exposed to irradiation; f) monitoring and accounting of individual exposure doses of persons involved in mitigation (elimination) of the accident and its consequences. In the later phase of the accident the medical protection measures should be taken on the basis of the features listed in paragraph 9.2.5. Measures for medical protection of personnel during conducting RUO. Calculation of manpower and resources for medical protection and support in case of an accident on NPP site should be in accordance with Appendix No.19 to the Protective Action Plan. 9.6.3. The order of the sanitary-hygienic and anti-epidemic measures. 9.6.4. The availability of medications, drugs, medical facilities and their provision. 9.6.5. The order of submission of reports on the status of medical protection measures. 10. Elimination of emergency situations at NPP. Involved manpower and facilities

10.1. Management of the accident elimination activities at NPP

10.1.1. RUO control organization. 10.1.2. Decision of the head of emergency operations at NPP on elimination of emergency situation (always taken in case of emergency at NPP). Decision of the head of emergency operations at NPP on RUO to be carried out should be prepared and issued in accordance with Appendix No.20 to the Protective Action Plan. 10.1.3. Organization of support of the actions of control bodies, emergency situation elimination manpower and resources. Structure and equipment of manpower and resources raised for elimination of emergency situation at NPP should be in accordance with Appendix No.21 to the Protective Action Plan. 10.1.4. The order of bringing tasks to the notice of control bodies and involved emergency elimination manpower and resources (conducting RUO).

10.2. Organizing and conducting RUO to eliminate the radiation accident.

In case of fire the staff and personnel of FFPS division should act in accordance with the NPP Fire Fighting Plan which is developed by the FFPS division in conjunction with the NPP management in accordance with the established procedure. 10.3.1. Organization of command of manpower and facilities in the event of a fire. The order of submission of reports on the status of fire elimination measures.

The structure of the main forces and means involved in firefighting

Structure of the main and additional forces and means involved to extinguish the fire is given in Appendix No.22 to the Protective Action Plan. 10.3.2. SSP and operating personnel actions in case of fire. 10.3.3. The organization of interface between the FFPS division, FEM and NPP management. 10.3.4. Measures to protect the NPP personnel in case of fire in the possible adverse radiation environment or in electrical installations. 10.4. 11. Support of forces and means raised for the elimination of emergency situations at NPP

11.1. Radiation and chemical reconnaissance

11.1.1. The number and size of the units of radiation and chemical reconnaissance (hereinafter "reconnaissance"), its equipment (the equipment should include individual protection means, communication means, first aid means, radiation and dose monitoring instruments allowing to estimate the gamma-radiation dose rate in the range from 0.05 µSv / h to 10 Sv / h and the range of recorded radiation energy from 0.015 to 3.000 MeV). The structure of forces and means to carry out reconnaissance including those from the forces aimed at the NPP for elimination of emergency situation at the emergency power unit, on the NPP site, in BA and RCA, in the NPP satellite town, in the area of possible hazardous radioactive contamination is determined in accordance with Appendix No.23 to the Protective Action Plan. 11.1.2. Establishment of communication and transmitting messages on the reconnaissance results. 11.1.3. Organization of reconnaissance. Schemes of organization of reconnaissance by NPP forces in accordance with Appendices No.23.1, N 23.2, N 23.3, N 23.4. 11.1.4. Organization of interface with the involved forces to conduct reconnaissance. 11.1.5. Measures to protect personnel of reconnaissance units, organization of active treatment.

11.2. Engineering support

Engineering support includes engineering reconnaissance of facilities and terrain in the ES elimination forces work area, arrangement and maintenance of transportation and evacuation traffic routes, putting ES elimination forces in the areas where RUO is being carried out, engineering measures to overcome the devastation, flooding, arrangement of water supply points. 11.2.1. The number and size of engineering reconnaissance divisions, equipment. 11.2.2. Establishment of communication and transmitting messages on the results of engineering reconnaissance and engineering support in conducting RUO. 11.2.3. Organization of engineering support during RUO. 11.2.4. Involved forces (from those targeting at the NPP) and the organization of their interaction during RUO. 11.2.5. Preparing routes for moving-out and maneuver of engineer reconnaissance divisions to RUO areas, supply routes, arrangement of vehicle passages in the debris and passages through the obstacles. 11.2.6. Measures to protect personnel of engineering support divisions, organization of active treatment. 11.3. Medical support

Medical support is organized and implemented in order to maintain health and efficiency of personnel involved in the elimination of ES at the NPP, the timely provision of medical assistance to the injured and ill persons, their evacuation, treatment, and prevention of occurrence and spread of infectious diseases. 11.3.1. The order of medical support under the threat and occurrence of emergency situation at the NPP. The list of resources and organizational decisions necessary to provide medical assistance to the victims. 11.3.2. The number and strength of medical and sanitary-epidemiological reconnaissance divisions, their equipment, organization of communication and presentation of reports on the progress of medical support activities. 11.3.3. The organization and procedure of medical and sanitary-epidemiological reconnaissance. 11.3.4. Forces engaged in medical support and the organization of their interaction. 11.3.5. Measures to protect personnel of medical support divisions, organization of active treatment (if necessary).

11.4. Logistics and material support

11.4.1. LMS structure and procedure in case of emergency situation at NPP. a) NPP LMS support manpower and resources, structure, equipment, bringing into preparedness procedure, arrangement of communication and submission of reports on the progress of logistics support activities. Calculations of logistics support of manpower and resources raised for elimination of emergency situation at NPP should be in accordance with Appendix No.24 to the Protective Action Plan; b) organization of distribution of individual protection means and equipment to the NPP rescue divisions; c) LMS of NPP manpower and resources for conducting RUO (spare parts, water, food, clothing, footwear, fuel, transportation, communication, personal protective equipment, dosimetry and radiation monitoring instruments); d) support of personnel life activity in their movement to the evacuation area; e) involvement of forces and means for logistics support in case of emergency situations and organization of interaction. 11.4.2. LMS of NPP IAG (in accordance with the applicable federal atomic codes and regulations regulating the procedure for announcing emergency situation, rapid transmission of information and organization of emergency assistance to the NPP in the event of radiation hazards). 11.4.3. Reserves of material resources. Structure and placement of emergency kits (storage places, responsible persons): - radiation monitoring devices and personal dosimeters; - individual protection means, special clothing and medicines; - external and internal communication means and warning systems; - equipment, tools and accessories for RUO and emergency rehabilitation activities; - means for decontamination, outgassing of facilities, equipment, structures and territories.

11.5. Transport support

11.5.1. The order of transfer of personnel to provide NPP operation in emergency situations. 11.5.2. The order of transfer of personnel involved in conducting RUO to the site. 11.5.3. Points of technical assistance, gas stations. 11.5.4. Involved forces and means, organization of interaction. 11.5.5. Calculation of transport support for evacuation of personnel form NPP site and BA is shown in Appendix No.25. 11.6. Hydrometeorological support

11.6.1. Forces and means of hydrometeorological support, locations, organization of communication. 11.6.2. Procedure of hydrometeorological situation data submission to the control bodies in charge for elimination of emergency situation at NPP (means of communication, the form and content of reports, deadlines for submission). 11.6.3. The order and the extent of data transmission to the emergency rescue divisions and services on the weather in the RUO area and urgent information on dangerous meteorological and hydrological phenomena and their development trends.

11.7. Protection of public order

Protection of public order in the emergency area should be understood as the actions of the public order maintenance forces targeted to the organization and regulation of traffic of all types of transport, security of material values of any form of property and personal property of the victims, as well as maintenance of the established emergency situation mode, the order of entry and exit of people and vehicles, and assigned to the NPP structural units in accordance with their organizational and functional structure and divisions of the RF Ministry of Internal Affairs assigned to the territory in question. 11.7.1. Forces and means for maintenance of public order. Establishment of communication and submission of reports on the progress of activities to ensure the public order. 11.7.2. Procedure for termination of movement of all vehicles and people on the NPP site and (or) in the BA (depending on the scale of the accident), with the exception of those participating in elimination of the accident consequences. 11.7.3. The regulation of movement of the personnel who had taken shelter at the NPP site. 11.7.4. Radiation accident (emergency situation) area blocking procedure. 11.7.5. Procedure for patrolling the established routes. 11.7.6. Procedure for regulation of traffic on the personnel evacuation routes. 11.7.7. Protection of public order and safety assurance at the evacuation facilities (EAP, IEP, ERC, ECP, in the embarkation and disembarkation points, on evacuation routes and in evacuation areas). 11.7.8. On the NPP site layout there should be shown buildings and structures, major utilities, PECC and shelters (without specifying their characteristics); locations of stationary communication means; medical center to provide first aid to victims; perimeter of NPP site and the location of entry control points, posts of departmental security divisions and IMF of Ministry of Internal Affairs of Russia; personnel assembly points; vehicle bringing points and evacuation routes from the NPP site; routes and lines for input of forces involved for the response to the accident; location of radiation monitoring stations (including ARMS); points of distribution of radiation monitoring devices; locations of containers with explosive, flammable and combustible substances, water sources; symbols. Layouts and schemes in the related appendices in electronic and paper form should be provided with an indication of scale. The format of the layouts and schemes used in the Protective Action Plan should be A2 or A3, and allow to provide the necessary information in accordance with the purpose of the appendix.

Appendix No.2

NPP SATELLITE TOWN LAYOUT

On the layout of the NPP satellite town there should be shown the boundaries of the design development of the city, PECC/T, shelters and fallout shelters (without specifying their characteristics), medical facilities, a communication center, broadcasting center, TV studio engaged in LWA, SWS (staff warning system), SPP; ARMS sensors, ARMS central control post, ERML; the town road network, evacuation routes that provide for motor transport getting in and out from the town while all the evacuated population being simultaneously taken out from the town; control points to regulate the vehicles getting in and out of the town, ECP, evacuation assembly points, embussing points, KI tablets reserve storage facilities, individual protection means for the public and their distribution points; warehouses and storage areas for mandatory in accordance with the equipment lists hardware, vehicles, instruments, tools and equipment of SDF, NPERT of the NPP and IAG equipment, railway stations, platforms, river piers and docks, bus stations, airfields and landings; places of permanent deployment of divisions and territorial bodies of the Ministry of Internal Affairs of Russia, the Ministry of Emergency Situations of Russia, the Ministry of Defense of Russia, the Federal Security Service of Russia, Federal Bio- Medical Agency of Russia, Environmental Monitoring Agency and the sources of water supply of the town, characteristics of transport routes between the town and NPP, industrial facilities, symbols. NPP POTENTIAL HAZARDOUS RADIOACTIVE CONTAMINATION AREAS LAYOUT

On the layout there should be shown the administrative and territorial boundaries of the NPP satellite town, and municipalities located within NPP potential hazardous radioactive contamination area, BA and RCA boundaries, settlements in the BA, protective measures planning zone, PECC/S, PECC/T and PECC/EA and also location points of territorial control bodies for Civil Defense and Emergency Situations of the city, region (county), the routes and locations of the forces involved in mitigation (elimination) of ES: divisions of Russian Ministry of Defense, Ministry of Internal Affairs of Russia, Russian Ministry of Emergency Situations (hereinafter “EMERCOM”), Federal Bio- Medical Agency of Russia (areas of concentration, the starting areas, the input lines on BA and RCA boundaries), the location of the meteorological stations in the town (district, region), the NPP evacuation areas (main and reserve), personnel evacuation routes, communication and medical facilities, location of radiation monitoring stations, paved roads and exit from them to the NPP from all possible directions, location of IEP, EAP, DS, SPP, TDS and CDS, ECP and regulation posts on the routes of evacuation and bringing emergency elimination forces into the NPP, the legend.

Appendix No.5

CALCULATION OF CONTAMINATED AREAS, EXTERNAL AND INTERNAL DOSE RATES IN BEYOND DESIGN BASIS ACCIDENT

1. Justification of the choice of beyond design basis accident. 2. Scenario of the selected beyond design basis accident. Radionuclide composition and activity of the release of radionuclides from the start of the accident at its early phase (up to mitigation (elimination) of the emission source). 3. Analysis of radiological consequences of the selected beyond design basis accident and the results of calculations of projected dose of external and internal exposure. To analyze the radiological consequences calculation of projected doses of external and internal exposure is performed using a conservative approach. 4. Possible number of injured individuals of the NPP personnel, which may require medical assistance taking into account the severity and type of injury. 5. Evaluation of radiation conditions in the NPP premises, MCR, SCP (ECP) on NPP site and in BA. 6. Basic data of calculations of zones, their boundaries and characteristics (radioactive contamination zone, protective measures planning zone, zones of planning measures for personnel mandatory evacuation, contamination levels at the boundaries of zones) for the given weather conditions. 7. Key actions to protect personnel on the basis of calculations of the projected effects of the chosen beyond design basis accident. 8. Conclusions: The results of calculations of the size of a radiation accident zone are applied to the NPP POTENTIAL HAZARDOUS RADIOACTIVE CONTAMINATION AREAS LAYOUT indicating the wind rose, the average surface wind speed, the average air temperature and the legend.

Appendix No.6

SITUATION PLAN NPP CHEMICAL ACCIDENT LAYOUT (REQUIREMENTS FOR THE LAYOUT)

1. The layout should be issued on A4 (A3) sheets. 2. The graphics of the layout should indicate: scale of the figure; the NPP site boundaries and components; location of the nearby organizations and facilities; location of nearby communities; contamination zone in conformity with calculations shown in Appendix No.7 to the Protective Action Plan. 3. The caption to the layout contains an interpretation of symbols and a brief description of the scenario relevant to the graphics with indication of the following: the main calculation input data (major damaging factors, weather conditions, time of exposure); CHS names and amounts in case of their release (spill); the size of the chemical contamination zone; possible number of victims and other information necessary to assess the situation and take measures to protect personnel. Appendix No.7

CALCULATION OF THE SIZE OF THE ANTICIPATED CHEMICAL CONTAMINATION ZONE FOR CHS RELEASE (SPILL)

1. In the early anticipation of the chemical environment the following assumptions to be made: a) CHS release (spill) value is assumed as the maximum volume of the unit storage capacity (for seismic areas - the on-site total CHS stock), if there is no CHS at the NPP, data on CHS volume on neighboring sites should be used for calculations; b) the thickness of the fluid layer h for CHS freely spilled onto the underlying surface is assumed to be 0.05 m; for CHS spilled into the sump or border, it is determined by the relationship: h = H - 0,2 where H is the height of the sump (border), m; c) the extent of contamination is calculated by primary and secondary clouds; d) weather conditions: the degree of vertical stability of the air - inversion, wind speed - 1 m / s, and air temperatures of - 20 ° C, wind direction - equiprobable from 0 to 360 °. 2. The calculation involves determining: equivalent amount of the substance in the initial cloud Q1; time of evaporation of the substance T; equivalent amount of the substance in the secondary cloud Q2; depth of contamination by primary and secondary clouds G1, G2; the full depth of contamination zone G and the possible values of the depth of air mass transport Gt; duration of effect of the harmful substances. 3. The results of calculation of the size of the anticipated chemical contamination area and its configuration formed as a result of the accident, should be marked on the NPP potential hazardous contamination layout (layouts of NPP site, BA , NPP satellite town depending on the size of the area) and shown on the layout in accordance with Appendix No.6 to the Protective Action Plan. 4. The main activities for the protection of personnel in chemical accident on the basis of forecast data (calculations). 5. Conclusions: After the accident there should be used concrete data on the quantity of spilled material and real weather conditions for clarification of the preliminary forecast.

Decision of the Head of emergency activities at NPP on RUO to be carried out includes: а) brief findings of the assessment of the situation; b) plan of action; c) task for the forces and means to eliminate the emergency situation, safety and protection measures; d) organization of interaction; e) the fundamentals of establishing control; f) support of the actions of emergency situation elimination manpower and resources; g) other relevant data. a) brief findings of the assessment of the situation should contain basic information on the nature and extent of emergency situation, the degree of hazard to personnel, zone boundaries, the types, quantities of upcoming RUO and conditions of RUO implementation, available forces and means and their capabilities, need in manpower and equipment to carry out RUO , adequate supply and preparedness to act of manpower and resources, meteorological conditions, specific features; b) plan of action includes; - the objectives, major tasks and the sequence of operations; - sites (districts, areas) of main efforts focus; - stages and ways of doing things; - allocation of power and resources, the sequence of their entry into the emergency situation area for operations; - permissible radiation doses and the order of registration of the planned increased exposure of personnel (if necessary); c) tasks for emergency situation elimination forces. The following is indicated when assigning the tasks for the subordinates: - sites (districts, areas) to carry out works; - work forces and means, sequence and timing; - sites (districts, areas) of main efforts focus, the order of using hardware; - safety measures and measures to ensure the continuity of operations; - specified radiation dose; - the order of interaction, control signals and protection measures (individual protection means, dose control, sanitation, decontamination); d) interaction of the NPP divisions with the divisions of other ministries and agencies, territorial forces (district, municipality) should be organized upon the decision is taken (with the participation of representatives of the interacting forces) and carried out during RUO. Arrangement of interactions includes: - specifying the work site for each division; - establishing the sequence of actions on the adjacent sites (especially when carrying out activities which may endanger or affect their functioning); - coordination of timing and point of focus of efforts for the joint implementation of most critical and complicated works; - coordination of timing to start and complete the works (roughly); - defining the order of data exchange concerning environment, changes in the environment and the results of operations on the adjacent sites; - establishing procedures for emergency actions and mutual assistance in the event of unforeseen circumstances; - establishing the locations of control points, the order of information on the work progress and the methods of communication; e) the organization of RUO control, place and time of deployment of RUO control points, control and warning signals, organization of communication, the order of submission of reports on the measures taken; f) supporting operations of the forces and means of ES elimination (clarification of the task, the assessment of the situation to the extent of his/her responsibility (competence), the readiness of supporting forces and means and their timely entry into the emergency situation area, setting tasks to subordinates and their clarification in the course of work, monitoring how the set tasks are being solved); g) other relevant data. 2. The decision to hold RUO should be documented with the schemes and calculations attached (if necessary). The tasks should be set to the involved in RUO divisions in the form of EOM order in accordance with the decision taken. During RUO, the task to ES elimination forces reporting to EOM (Director of NPP) should be specified (if necessary) by orders (instructions) to the Leaders of the divisions which are given by EOM verbally, with the subsequent registration of the orders in documents. In preparing a decision you start planning RUO that should be completed after taking the decision and setting tasks to the ES elimination forces. The RUO plan should be issued in the form of text with maps, charts, diagrams and calculations attached. The plan should be signed by the head of the NPP structural unit authorized for the solution of civil defense issues, and approved by EOM. Extracts from the plan should be communicated to all the leaders of ES elimination divisions at NPP to the extent they are concerned. The plan is subject to adjustments during the entire RUO period in the emergency situation area. Formed as a result of the accident and ES elimination environment should be applied on the map (plan, diagram) of the appropriate scale. KEY ACTIONS OF THE PROTECTIVE ACTION PLAN

1. The decision to announce for the NPP the state "Emergency Preparedness" or "Emergency Situation" should be taken by the Director of NPP (or a person acting for the Director of NPP). 2. If you are unable to communicate information on the evolving situation at the NPP to the Director (a person acting for the Director), decision to announce "Emergency Preparedness" or "Emergency Situation" state at the NPP may be taken by the NPP Chief Engineer (a person acting for the NPP Chief Engineer). 3. Extracts from the Protective Action Plan should be available with the key structural units of the NPP and the chiefs of the rescue divisions and communicated to the staff to the extent the staff is concerned. 5. In the corridors of administrative building and the main shops, and the NPP personnel gathering places there should be placed boards displaying information on the rules and sequence of the staff actions in emergency situations. 6. Familiarize personnel with the "Memo on the actions of the staff in an accident or radiation hazardous situation" and pocket books for member of the special departmental forces (SDF) on actions in emergency situations and in other situations. 7. EOM (NPP Director or Chief Engineer) is in charge for the command of forces and facilities raised for the elimination of emergency situation at the NPP, organization of their interaction on NPP site and in BA. EOM decisions on ES elimination are mandatory for everyone on the NPP site and in BA, unless otherwise is provided for by the legislation of the Russian Federation. 15. KEY ACTIONS OF NPP OPERATING PERSONNEL, HEADS OF NPP STRUCTURAL UNITS IN ACCIDENT AT NPP

1. These actions of the NPP officers should be understood as recommendations. 2. Developed at each NPP actions of officers should be included in the job descriptions of the heads of the NPP structural units.

1. Key actions of operating personnel of the shops (departments, laboratories)

1.1. Upon receipt of the information on violation of NPP Unit safe operation conditions (or in case of detecting the indications of such a violation in person) the operating personnel of the shops (departments, laboratories) of the plant, in accordance with job descriptions and operating instructions, immediately reports violation of NPP Unit safe operation conditions to SSP (SSU) and provides for: establishing the emergency situation causes based on instrument readings, functioning of alarms, and reports from the work places; inspection of the location wherein the violation occurred and assessment of the emergency situation scale; take necessary and available measures to provide assistance in case of injures, threat to human life and personnel exposure in excess of allowable limits; mitigation (elimination) of emergency situation and its consequences; participation of operating personnel (if necessary and reasonable) in the reactor plant (RP) shutdown and cooldown; monitoring of limits and conditions of safe operation of equipment and units that do not experience the emergency; making records in operative logs with chronological reflection of the incidents of actuation of emergency alarms and protections, equipment failures, received commands and instructions of the officers, implemented operative actions and results thereof; reporting the execution of commands and instructions to the persons from whom those were received. 1.2 With the actuation of radiation hazard alarm in the controlled access area compartments (including visual and audible alarm), all personnel should immediately stop work, leave the room and shut the door after disconnecting electrical appliances, cutting off running water, lowering the load, etc. 1.3. The immediate supervisor, operating personnel of the corresponding structural unit and radiation safety service should be informed of the alarm actuation. Permit to proceed with the work might be received from the operating personnel of the radiation safety support service after establishing the causes of alarm actuation.

2. Key actions of shift supervisor of NPP radiation safety department

NPP radiation safety department shift supervisor shall: upon actuation of alarms of radiation safety monitoring system, immediately inform the SSP of the instrumentation readings and the list of emergency compartments (with the indication of elevations); in the absence of SSP at the workplace, immediately inform by telephone the shop shift supervisors of emergency alarm actuation in the compartments, for which they are responsible, and of the current situation, and take measures for conveying the information to SSP; arrange and provide for urgent assessment of radiation situation in the accident zone (first of all, in places where personnel is present), more exact definition of radioactive contamination boundaries; select safe routes for escape of the personnel not participating in elimination of the emergency from the accident zone; provide for monitoring and registration of working clothes and skin of evacuated personnel in sanitary inspection rooms; provide for continuous monitoring and registration of radioactive release value from ventilation systems and activity of discharge water; based on meteorological data for the moment of the accident (wind velocity and direction, outside air temperature and humidity, condition of the atmosphere bottom layer, an availability of precipitation) provide SSP with a forecast (for 4, 8, and 24 hours after origination of the emergency situation) of accelerated development on the site, in the BA and beyond its limits.

3. Key actions of the leader of the NPP structural unit

The head of NPP structural unit after receiving information of the announcement of "Emergency Environment" state (emergency situation mode) arrives at PECC/S (NPP EC) and shall: verify that the personnel of the unit received the annunciation signals, is using IPM and antirad medication, call the roll and find out the location of the unit personnel at the moment of the accident; arrange the initial medical aid to the injured and their delivery to the first aid post, and assign attendants for this purpose (if necessary); make sure that iodine prophylaxis of the subordinate staff has been performed; in consultation with SSP arrange putting out of operation of the equipment that does not affect NPP safety condition; provide for withdrawal of personnel not participating in routine switching and main equipment maintenance from the accident zone and check their sheltering (if necessary) according to the scheme; make up a roll of subordinate personnel and persons temporarily assigned to the unit with the indication of their location at the time of the accident, duration of stay in the accident zone and escape route to the shelter; arrange timely replacement of personnel who were left in the work places for putting the equipment out of operation and provide them with IPM, radiation and medical protection means; replace (if necessary) the staff who are unable to perform their duties as agreed with the SSP; upon receipt of CEPRFS/S Chairman's order for assigning the unit personnel to NPERT divisions, arrange assembly of NPERT divisions personnel in the appointed places; check the team assembly and equipment and report to the head of the NPP department authorized for resolving the issues in the field of the protection of personnel and territories from emergency on the readiness of his team to carry out tasks in elimination of consequences of the accident; act in accordance with the instructions of NPP Director (CEPRFS/S Chairman) and the Head of the NPP department authorized to solving issues in the field of the protection of personnel and territories from emergency situations; carry out RUO in the area assigned to the structural unit in compliance with the safety measures.
