Student Name______Campus: SJMS

Our CATE high school level classes are designed for success and development of student’s critical thinking skills. We utilize a “Project Based Learning” management approach. It requires consistent personal interaction, documentation and time-frame completed products. For a student whose grade average is less than “70” on any official progress reporting period, a student-parent-teacher conference will be held to develop strategies for success using an “Individual Academic Intervention Plan (IAIP)” standards reflecting SJMS 11-12 Students’ Handbook: 1. Raise and maintain an average of at least 70. “X’s are entered until projects are completed, but are not reflected in the average until actual grade is given. 2. Attend tutorials promptly to alleviate future learning gaps. 3. Turn in all assignments/projects. MISD Policy: 70%-Summative; 30%-Formative. 4. 3R’s – Review, Re-do and Re-take any test or assignment’s scores 70 or below once. 5. Make-up work promptly following any absence within “handbook” timelines/guidelines. 6. Actively participate in class discussions, asking clarifying questions for understanding. 7. Actively participate in class activities and individual and team projects. 8. Demonstrate responsibility, self-control, and appropriate decision-making skills. 9. Follow class specific standards (e.g., maintain virtual notes and/or notebook, planner). The Individual Academic Intervention Plan (IAIP) will be: Reviewed three (3) weeks into IAIP which will use an “Assignment Completion” documentation form, daily review by parent, daily accountability by student. Progress will be documented at the next “Progress Report” period. If, the grade remains below “70” for two (2) consecutive 3-week grading periods: A conference will be held with an administrator, the student’s teacher grade level team, the student, and parent(s) to determine if any additional interventions might change the student’s work habits into a positive successful state of learning and achievement.
