Waubonsee Community College Human Services Department Seminar and Field Experience Case Presentation-Human Services Student Case presentations are a professional responsibility of all counselors. There can serve several purposes for case presentations. Often the counselor will present a client who is not progressing to peers and/or supervisors as a method of obtaining help with that client. It can be a required part of team treatment planning. It can be used as part of clinical supervision. Case presentations can also be used as part of a staff-training program.

 In this semester, each intern will be required to make a case presentation in class.  Directions for preparing the case presentation are:

1. Use an actual/typical client from your internship. Use FICTIONAL NAME AND DELETE OR CHANGE ALL IDENTIFYING INFORMATION FROM YOUR PRESENTATION. 2. Complete the demographic information on the client. 3. Provide the information for items A. through J. Section K. should be completed on client’s who have been discharged. If the client is still active in treatment, complete based upon current status, and current aftercare planning. Use the attached format. (note: this form and process is very similar to the process used by IAODAPCA for case presentations) 4. Prepare 2 typed copies of the report, one for you, and one for me. 5. Be prepared to answer questions from fellow students. 6. You will have 20 minutes for the presentation and 10 minutes for questions and answers. Be prepared, as you will be expected to stay within time frames. 7. Use your typed report to stay focused 8. Note to class and presenter—questions should be asked about the treatment provided by the counselor/program, not about the client’s history, unless you need additional historical clarification to ask a question. This assignment is about clinical services provided, or needed, not about the client. 9. Be working on the presentation during the course of the semester. 10. You can make your presentation any evening you would like. However, it must be done by end of the semester. 11. Please see me if you have questions or wish to schedule your presentation. Attached is a list of sample questions based upon the 12 core functions that your peers may ask you. When preparing for your presentation, review the questions. This is a very serious assignment, do not take it lightly, it reflects your professional development to date. We are all friends in this room and it is my job to make it a safe place, but I will be asking hard questions and will expect fair, hard questions to be asked by your fellow students. H. Assessment: Identify assessment tools used. Identify and evaluate the individual’s strengths and weaknesses. Identify problems and needs identified during the assessment process.

J. Course of Treatment: Described counseling you used. Your rationale for its use and any revisions you made to the treatment plan based upon the client’s unique needs and responses to treatment.

K. Discharge Summary—Concise description of the client’s overall response to treatment, including status at the end of treatment and aftercare plan. Core Functions of the Addictions Counselor Sample Questions for Case Presentation

1. Screening a. What is the purpose and process of screening, as it relates to this case? b. Describe the methods you use to screen your client? c. What factors are appropriate to consider in screening, and how did you apply them to your client?

2. Intake a. Describe how the intake procedure was completed in this case. b. What were the procedures for admitting your client to your program? c. What is important to be considered in doing an intake? Give examples from your case.

3. Orientation a. Explain how you oriented this client to your program. b. What is the purpose and process of orienting the client in your case?

4. Assessment a. Explain the methods and procedures used to assess this client, and explain the results of those assessments. b. How did you assess your client? c. What are the specific factors, which must be taken into account in assessing your clients?

5. Treatment Planning a. Who was involved in establishing the goals and objectives of the treatment plan? b. How was this treatment plan individualized for your client? c. What are the components of treatment planning, and how do they apply to your case?

6. Counseling a. What treatment approach or approaches did you use in this particular case? b. In your case, explain the counseling processes/methods/modalities used and why they were used. c. Explain your theoretical counseling orientation, and how you applied it to the client in your case. 7. Case Management a. What factors are important to be considered in case management? Relate them to your case. b. How did you manage your client’s case? c. Describe the core function of case management, and how it applied to your client.

8. Crisis Intervention a. Explain what steps you would take if your client were to experience a psychotic episode. b. Give an example of a crisis and how you used it to facilitate treatment. c. What steps would you take if a client threatens or attempts suicide? d. What steps would you take if a client received HIV positive test results while in treatment?

9. Client Education a. What is meant by client education, and how did you implement it in your case. b. How did you individualize your client education in this case? c. What are the important considerations in client education? Relate these to your case

10. Referral a. How and why do you refer a client, and what are the important factors? b. Describe your agency’s referral process, and specify how and by whom this process was completed in this case. c. What are the purposes of referral? Relate this to your case.

11. Reports and Record Keeping a. What is the purpose of reports and record keeping? Provide one example of good record keeping that assisted you as a counselor. Provide one example of poor record keeping that hindered quality care. b. Why and how do you keep records and make reports? c. Discribe your agency’s policy regarding treatment notes.

12. Consultation a. What is the purpose of consultation? b. Describe the rationale for, and results of any consultations necessary in this case. c. Describe how you used this consultation to modify your treatment approach.
