The Marine & Oakridge Partnership is a general practice offering medical care and services under the General Medical Services contract.

Arrangements for disabled patients The entrance and internal doors at both surgeries have been designed to allow access by wheelchairs. There are also toilet facilities for the use of disabled patients. If you think you may experience difficulties or feel you may have particular needs please contact the receptionist before arrival and we will attempt to make special arrangements. 2 Practice Team

The Practice Management Team, consisting of the Practice Development Manager, Mrs Victoria Whitham, Practice Business Manager, Miss Gillian Sanders and Patient & Personnel Manager, Mrs Michelle Deary manage the day to day running of the two surgeries. They are supported by the reception, administration and secretarial teams.

Practice Nurses and Primary Health Care Assistants Our practice nurses and health care assistants offer a full range of treatment room services including injections, dressings, ear irrigation, suture removal, smears, blood taking, monitoring of Chronic Obstructive Disease (COPD), Spirometry and baby immunisations. They also monitor conditions such as diabetes, asthma, and blood pressure and may advise on a variety of aspects of health and diet including family planning, well women, and travel. If you are aged between 16 and 74 and have not seen a GP or nurse for the last three years please feel free to request a health check which will be made initially with the Health Care Assistant.

District/Community Nurses The district/community nursing team attached to our practice provides nursing care for patients in the community. Their role includes assessing patients’ needs in their homes, checking patients after hospital discharge, and giving professional nursing care and advice. Student nurses employed by the Primary Care Trust are often on placement with the district/community team. To contact the district/community nurses please telephone: - 01202 428025.

Health Visitors and Nursery Nurse A team of part-time health visitors and a nursery nurse provide special support, supervision, and advice for example, for child care and safety and prevention of accidents in the home. Midwife The midwife runs regular ante-natal clinics at Marine and Oakridge Surgeries.

3 Clinical Psychologist A clinical psychologist is based at Marine Surgery. Patients may be referred to the Primary Care Mental Health Team by their own doctor if appropriate.

Counsellors A team of counsellors from the Gateway Team is based at Marine Surgery and are specially trained to help with a wide range of social, personal, family, and emotional problems, for example, bereavement or redundancy. Please ask your doctor or nurse for details.

Dietician A dietician is based at Marine Surgery and offers help and advice on all dietary problems. To see the dietician you will need a referral from your doctor.

7 Out of Hours Medical Care and Emergencies

For a medical emergency please call 111

When the practice is closed the number to call for medical help that is not life- threatening is - 111.

When you call 111 a trained adviser will ask you questions to find out what’s wrong, give you medical advice and direct you to someone who can help you, like an out-of-hours doctor or a community nurse. If they think your condition is more serious they will direct you to a hospital or send an ambulance.

Examples of when to call 111 include:

 if you don’t know who to call for medical help  if you don’t have a GP to call or your GP surgery is closed  if you need medical advice or reassurance about what to do next

999 should still be used in an emergency, for example when someone is seriously injured or there is a risk to life.

Calls to 111 are processed by South Western Ambulance Services NHS Foundation Trust who also provides emergency ambulances, out of hours and patient transport services.

The local treatment centre for the urgent care service is currently based at The Royal Bournemouth Hospital however from time to time the service might be provided at alternative locations, this is determined by the service provider and not by the practice.

Calls to 111 are free from landlines or mobile phones (although the mobile will require a minimum of 1p credit to be in place)

Please remember an appointment is for one person only and of 10 minutes duration. If you no longer require your appointment please ensure you contact the surgery as soon as possible in order that we can offer the appointment to other patients.

We are a group practice and offer appointments with a choice of doctors. We endeavour to work with our patients to provide continuity of care so that you can book an appointment with your usual doctor, however this may not be possible for emergency appointments. We will try to accommodate your wishes wherever possible.

All consultations with a doctor or practice nurse are by appointment only. We offer appointments which you may book up to six weeks in advance. If you feel you need to be seen quickly we will provide you with an appointment with a health care professional within less than 24 hours.

We will make every effort to ensure that you are seen at the earliest possible opportunity by the doctor of your choice, however if you need an urgent appointment this may not be possible and you will be seen by one of the emergency doctors.

Telephone Appointments – a telephone consultation with the doctor or nurse can be booked by calling the usual appointments number as above. The doctor or nurse will call you back later during the surgery. We are unable to provide you with a specific time for the doctor/nurse to call you. Please be aware that the doctor or nurse will only call you the once. If there is no reply to their call they will not call again, you will need to ring the surgery again to request a further telephone appointment. The surgery will not leave a message on an answer phone unless you have previously provided written consent for us to do so.

For patients who are too ill to come to the surgery, please ask the receptionist for a home visit. Visits usually take place after morning surgery so please telephone before 10.00am. We are unable to guarantee that a specific doctor will be able to make a home visit. 9 If you need an urgent visit, please give the receptionist as much detail as possible to help us assess the urgency. In some circumstances you may be advised to go to the Emergency Department at the hospital or to telephone for an ambulance.

Registering with this practice To register with this practice please visit Marine or Oakridge surgery and speak to the receptionist about registering. You will be given a GMS1 patient registration form to complete for each person wishing to register. Alternatively log on to our website at www.marineandoakridge.nhs.uk and click on the link New Patients – Registering with us. You will also be required to provide the practice with proof of your address in order to complete your application, for example a utility bill, council tax bill, paper copy of a bank statement (not an on- line copy from the internet), driving licence etc. Bills must be dated within three months of the date of your application to register.

Repeat Prescriptions All prescriptions that your doctor has approved for repeat medication will have a computerised repeat slip. These can be delivered or posted to the surgery or taken to a chemist and will be ready two working days later. If you do not use the pre-printed slip or if an item is not listed on the slip your request will take longer to process. If you would like us to post the prescription to you please securely attach a stamped addressed envelope to the request. Please note: -

Prescriptions can be ordered via the internet using the SystmOnLine service – for further information and how to register for this service please see the Website section of this leaflet or visit our website at www.marineandoakridge.nhs.uk ; . Prescriptions can be requested via your chosen local chemist and once the prescription is ready it will be collected by the chemist and dispensed for you to collect from them. Please contact the chemist of your choice for information about their service.

In addition  Registered patients aged 16-75 who have not been seen for three years may request a consultation;  Registered patients aged over 75 years who have not been seen in the previous 12 months may request a consultation. Courses Information and referral for Stop Smoking courses is also provided by the practice. Immunisations and Vaccinations A full programme is available to patients organised by the practice nurses. Flu vaccinations are available each autumn for patients aged 65 and over and for patients in the ‘at risk’ group, for example, asthmatics, diabetics. 11 Medical Students Medical students are occasionally attached to the practice. Patients will be advised by the receptionist when there is a student sitting in with their doctor. If this is unacceptable please inform the receptionist before you see the doctor.

Non NHS Services Occasionally you may require a service that is not covered by the NHS. Services such as pre-employment medicals, insurance claims and reports, travel claim forms and private certificates are not covered by the NHS and therefore this work will attract a fee. The practice scale of fees is in accordance with fees suggested by the British Medical Association (BMA) and the current list of charges is available at reception or by visiting our website at www.marineandoakridge.nhs.uk and clicking on the link to Our Documents.

Non NHS work of this nature is carried out by the doctors in their own time once all NHS work (appointments, visits and paperwork) has been completed.

Carers If you have a carer or are a carer, please let us know. We may be able to offer some useful information.

Travel Clinic We offer a comprehensive service for patients travelling abroad who may need to review vaccination requirement. The advice provided may be complex and will require an appointment for each traveller with our practice nurse. Some travel vaccinations do incur a charge. Please check with reception.

13 TPP SystmOne – is the name of the computer clinical software that this practice uses for your GP records and SystmOnline is a feature of the software.

You do still have a Lloyd George folder of older paper records however nowadays all records relating to your medical care provided by your GP, for example consultations, prescriptions, referrals to hospital and letters etc are stored on this computer system.

When you visit the surgery the doctor or nurse will access your patient record on the computer and will enter details of the consultation including any medication prescribed for you. If you are referred for a hospital appointment the referral letter is processed through the clinical software via an NHS secure internet service to the local hospitals.

The receptionists book all appointments through this software and these appointments are then added to your record when you book to see the doctor or nurse.

A feature of this software called SystmOnline allows patients* to book and cancel appointments, view their past and future appointment dates and times, request repeat medication, up-date your contact details and to submit questions to the practice.

If you would like to be able to access this service you will need to print off and complete the SystmOne on-line registration form a copy of which is shown below.

Once you have completed the form please bring it in to either Marine or Oakridge surgery together with photographic id. We are unable to issue user access codes to anyone other than to you.

* This service is currently only available to patients aged 16+.

16 Information about You We treat all information about you in accordance with legislation regarding Medical Records and with Data Protection. We hold information about you on the computer and in written files. Our computer system complies with NHS standards regarding data security.

Patient Confidentiality We are bound by legislation relating to the confidentiality of medical records. The current APMS legislation is “The Confidentiality and Disclosure of Information: General Medical Services, Personal Medical Services and Alternative Provider Medical Services Directions 2005”

These rules are designed to protect all patients and to prevent the release of your personal information to anyone other than yourself. These rules mean that we cannot divulge any information about you to anyone other than yourself.

If you do wish for the practice to divulge information about you to a third party then you must provide the practice with your written consent and authorisation to do so in advance and identifying which information might be released. If you are using an insurance or financial services broker to apply for loans or insurance cover please provide the practice with written consent to speak to the adviser as well as the mortgage/insurance loan company.

Please do not ask us to give you information about anyone else; we are not permitted to do so.

Access to Patient Information Confidential patient data will be shared within the practice health care team; it will also be shared with community health care professionals attached to the practice such as health visitors and district/community nurses involved in your care and with other health care professionals to whom you are referred for care. Your data may be used by those clinical teams providing your care for the essential purpose of clinical audit.

When we refer patients to other services the referral is administered by the local NHS referral management centre (RMC). The referral management centre is responsible for allocating your referral to other services and normally sees full information about the referral, including any confidential information.

17 If your GP is referring you to other services and you feel there is any information you wish to withhold from the referral management centre please inform your GP as soon as possible during normal working hours.

Confidential patient data may also be required for the broader purposes of public health, audit, inspection, and research, the provision of health care services, teaching, and training. Data disclosed will be kept to the minimum required to serve the purpose and if possible will be anonymised before disclosure.

Confidential and identifiable patient information will not be disclosed otherwise without explicit consent, unless;

1. it is a matter of life and death or serious harm to you or to another individual; 2. it is overwhelmingly in the public interest to do so; 3. there is a legal obligation to do so.

In all of these circumstances the minimum identifiable information that is essential to serve the purpose may be revealed to someone with a legal entitlement to access the data for that purpose. All individuals with access to your data have a professional and/or contractual duty of confidentiality. If you are concerned about any of the ways in which your confidential data further may be used further information is available from the practice manager. You are entitled to register an objection, which will be respected if this is possible.

Patient rights and responsibilities You have a right to expect a high standard of medical care from the practice and we will try at all times to provide the very best care possible within the resources available. In order to assist us in this we require that you take full responsibility for ensuring that you do not abuse the service. For example, it is your responsibility to ensure that you keep medical appointments and follow the medical advice given.

You have a right to express a preference to see a particular doctor or nurse and you can express this preference when telephoning for an appointment and/or by putting your preference in writing to us. 18 emergency in which case the appointment will be booked with the next available clinician.

Very occasionally a practice/patient relationship breaks down completely. In this situation the patient may choose to register with a different practice. The practice also has the right to remove the patient from their list. This would generally only follow a warning that had failed to remedy the situation and we would normally give the patient a specific reason for the removal.

Patient Participation We would like to know how we can improve our service to you and how you perceive our surgery and staff.

To help us with this, we have set up a virtual patient representation group so that you can have your say. Each year we ask the members of this representative group some questions such as what you think about our services and the quality of care or service that you receive. This usually takes place in the autumn. We will contact you via email and keep our surveys as brief as possible so it shouldn’t take too much of your time. If you would like to take part please ask a receptionist at either site how you can join. If you do not have access to email but would like to be a part of this group please let the receptionist know and we will arrange to contact you by an alternative method. Violent patients – Zero tolerance The NHS operates a Zero Tolerance Policy with regard to violence and abuse and the practice has the right to remove violent patients from the list with immediate effect in order to safeguard practice staff, patients, and other persons. Violence in this context includes actual or threatened physical violence or verbal abuse which leads to fear for a person’s safety.

In this situation we are obliged to notify the patient in writing of their removal from the list and record in the patient’s medical records the fact of the removal and circumstances leading to it. The Dorset Clinical Commissioning Group is then responsible for providing further medical care for such patients.

The practice will inform patients if their behaviour is perceived to be aggressive or offensive. Should the inappropriate behaviour continue, the practice will consider removal of the patient from the practice register.

We don’t get it right 100% of the time. If you are not happy with the care you or a loved one receives at this practice please tell us so that we can do something about it.
