THE RESOURCE CENTER’S SCHOOL-BASED HEALTH CENTER Jamestown High School FREQUENTLY ASKED QUESTIONS

1. What services are provided at the School-Based Health Center? First Aid, Immunizations Health education, Referral services Health Screenings (vision, hearing, scoliosis) Oral screenings and dental referrals Mental Health screening and referrals Diagnosis and treatment of acute and chronic illnesses (e.g. diabetes, asthma)

2. When is the School-Based Health Center open? During school hours and other designated times. Please call 716.483.4373 for specific times.

3. How do students enroll in the SBHC? In order for a student to receive services, a parent or legal guardian must complete Enrollment and Consent forms. There are no eligibility requirements except to be a student attending Jamestown High School. Parents are welcome to accompany their child to the SBHC. A parent may call their child’s school nurse or SBHC staff or send a note to arrange for the child to be seen at the SBHC. Every effort will be made to contact parents before a child receives any services, except if treatment is a matter of life or death. If a child is too sick to attend school and the parent would like the child to be seen by SBHC staff, parents are encouraged to call the school nurse or SBHC staff to arrange a same day appointment.

4. What is involved in the student enrollment process? A physical history is taken and the parents provide consent to contact the child’s health insurance company, if one exists. The SBHC will notify the child’s doctor at the time the student enrolls in the SBHC and every time the student receives treatment.

5. Will students need to be enrolled in the SBHC to utilize the school nurse services? No, the school nurse will continue to provide all students wit the same services they have in the past regardless of their enrollment in the SBHC. Enrollment in the SBHC is voluntary and can be completed at any time.

6. What is a Nurse Practitioner? A Nurse Practitioner at a SBHC is a Registered Nurse who successfully completed an accredited Nurse Practitioner program and has national certification. Nurse Practitioners may diagnose and treat illnesses and prescribe medications within their specialty area of practice. This is done in collaboration with Dr. Khan in accordance with an approved written practice agreement and protocols. 7. Who is the SBHC staff? The school nurse in each school will coordinate health services with the nurse practitioner and physician. The nurse practitioner will be able to assess the student’s health status and treat illnesses. A physician from The Resource Center will be the nurse practitioner’s collaborating physician. The physician will be available for consultation as needed and will be on site at the school regularly to provide direct patient services and review. A medical assistant will provide clerical support to the program along with assisting families with Child Health Plus enrollment.

8. What is Child Health Plus? New York State has a health insurance plan for children, called Child Health Plus. Depending on your family’s income, your child may be eligible to join either Child Health Plus A or Child Health Plus B. Enrolling in Child Health Plus is easy and available through the SBHC.

9. What does it cost to use the SBHC? The program was given a start-up grant by The Community Healthcare Foundation of Central and Western New York, in addition, Jamestown Public Schools provided in-kind contributions to get the SBHC up and running. If a student is covered under a health insurance plan, the health insurance company is billed directly. All the money collected will be applied toward SBHC expenses. Only the money paid by the insurance company is accepted. There can never be any out-of-pocket expenses to any family member for the SBHC services. Billing services are provided through The Resource Center. (It is important to note that the cost of services not provided on site at the SBHC, such as some laboratory tests, all x-rays, specialty consultations, and prescriptions are the responsibility of the parent(s).)

10. Who supervises the staff at the SBHC? The Resource Center operates the SBHC. The SBHC operates as a satellite clinic. All staff are employees, consultants, or contractors and report directly to The Resource Center Article 28 supervisor structure.

11. What happens when my child is ill and the school is closed or during summer vacation? The SBHC is open whenever the school is in session. Services will not be provided during the summer, during school vacations or days off. When the SBHC is not open, The Resource Center’s Primary Care Clinic is the back up facility (716.661.1447). You will be able to access care 24 hours per day, if necessary. If it is not a medical emergency, call your health care provider (doctor, nurse practitioner, physician assistant) or The Resource Center. For all urgent and emergency calls, please call 911.

12. Who pays for the SBHC services? The SBHC relies on state and private insurance reimbursement, grants and donations. The SBHC does not take money from the school. The program is administered with staff and funds that are dependent of the school.
