JOB TITLE: LICENSED NURSING ASSISTANT

MISSION: To promote the optimum level of well-being, independence and dignity of those living in the community by providing trusted, compassionate and expert health care.

Job Description: Licensed Nursing Assistant provides personal care assistance and other designated duties as a part of a continuing nursing care plan originated and supervised by a Registered Nurse.

Accountability: Team Manager Equipment Used: Various medical equipment and access to an insured, reliable vehicle.

Environment: Inside: 75 % Outside: 25%

Duties and/or Responsibilities:

Except as specifically noted, the following functions are considered essential to this position.

STANDARD I: Theory The Licensed Nursing Assistant applies theoretical concepts as a basis for decisions in practice. The Licensed Nursing Assistant will observe and maintain the following basic procedures to include the following: 1. Observe, report and document patient status and the care of services provided. 2. Read and record temperature, pulse and respiration. 3. Basic infection control procedures. 4. Basic elements of body functioning and changes in body function that must be reported to the LNA’s supervisor. 5. A clean, safe and healthy work environment. 6. Recognize emergencies and knowledge of emergency procedures. 7. Respect for the patient, his/her privacy and his/her property. 8. Understands the physical, emotional and developmental needs of the population served by LNA’s. 9. Appropriate and safe techniques in personal hygiene and grooming that include: bed bath, sponge, tub or shower bath, shampoo (sink or bed), nail and skin care, oral hygiene, toileting and elimination. 10. Safe transfer techniques and ambulation. 11. Normal ROM and correct body positioning. 12. Adequate nutrition and fluid intake. 13. LNA will :  Know how to apply/use certain equipment.  Identify educational needs re: equipment.  Participates in training/inservices as needed. 14. Performs tasks as delegated by the RN to include but not limited to stable wound care.

STANDARD II: Planning The LNA is able to read a care plan developed by RN and carry out its contents. 1. Plans assignments by priority and geographic area. 2. Obtains and follows updated plan of care. 3. Works collaboratively with peers and other health professionals as a contributing member of the health team. 4. Attends and participates in patient care conferences, contributes information to primary nurse for development of LNA plan of care

D:\Docs\2017-12-29\0340b75e0e86b77be624fc18dc461adb.doc STANDARD III: Intervention The Licensed Nursing Assistant, guided by the care plan, intervenes and observes to promote comfort, to restore, improve and prevent complications of illness, and to effect rehabilitation. 1. Communicates changes in patient’s condition to primary nurse/Rehab and intervenes appropriately as directed by RN/Rehab staff. 2. If primary nurse is not available, the concern would go to the Team Manager or Intake. 3. Understands and adheres to proper lines of communication via chain of command

STANDARD IV: Documentation The Licensed Nursing Assistant will adhere to agency policy and procedure concerning the documentation of individual care plans and records. 1 1. Care provided is accurately reflected through the CareWatch Documentation System. 2 2. CareWatch Documentation System is utilized appropriately. 3 3. Communication notes are utilized as appropriate

STANDARD V: Quality Assurance and Personal Development 1. The Licensed Nursing Assistant will contribute to their own learning needs and those of others. Licensed Nursing Assistant is able to identify their own learning needs and seek ways to meet those needs. 2. Licensed Nursing Assistant seeks constructive suggestions to improve one’s own practice and understands the importance of principles of improving organizational performance. 3. The Licensed Nursing Assistant will satisfactorily meet competency requirements annually. 4. LNA will accept responsibility and accountability for one’s own actions. 5. LNA may participate in agency committees. 6. LNA attends agency inservice programs and meetings.

STANDARD VI: Performance Appraisal LNA can perform his/her job role according to agency policy and procedure.  LNA will maintain LNA license.  LNA will maintain 12 contact hours annually – (contact hours accrue from date of hire). 1. Licensed Nursing Assistant meets physical requirements and considerations as stated in job description (page 4). 2. Licensed Nursing Assistants can perform his/her job in all kinds of weather with priority patients. 3. Demonstrates cooperation and collaborative work efforts to benefit coworkers. 4. The LNA engages in performance self-appraisal on a regular basis, identifying areas of strength as well as areas for professional development.

STANDARD VII Ethics The Licensed Nursing Assistant adheres to Cornerstone VNA‘S code of ethics: 1. Understands Cornerstone VNA ethics process. 2. Protects and honors patient and coworker confidentiality.

STANDARD VIII: Other factors influencing performance. The LNA is responsible for adhering to agency policy and guidelines concerning the following: 1. Productivity – LNAs are expected to see 6 clients in 8 hrs. 2. Addresses patients and coworkers in respectful and professional manner. 3. Works towards resolution of interpersonal conflicts as they arise. 4. Adheres to agency dress code. 5. Punctuality 6. Attendance 7. Dependability

Licensure/Certification Requirements: CNA license

Other Training, Skills and Experience Requirements: 1 year CNA experience and Homecare experience.

Judgment: Highly complex work performed under general direction requiring a substantial amount of original thinking.
