6. BRIEF RESUME OF THE INTENDED WORK:

6.1 NEED FOR THE STUDY :

Ayurveda is the ancient medical wisdom of India. Among the various concepts that Ayurveda put forward, a very important concept is Srotas which are the structural and functional unit of the shareera. These Srotas plays an important role in maintaining the health and in producing the disease when they are in normal and abnormal condition respectively.

The Mutravaha Srotas is one among the abhyantara Srotas. The group of organs concerned with Utpatti and

Visarjana of Mutra is called Mutravaha Srotas. Basti and Vankshana are said to be the mula of this Srotas. It plays very important role in the excretion of waste products of the shareera and thus maintains the health. When

Mutravaha Srotas get vitiated it produces many Mutravaha Srotogata vikaras.

Mutrashmari is one of the major disease which is having significant world wide health problem due to the debilitating and progressive condition of the nature. In this Jet era every person is in hurry and on account this busy life style person is suppressing the natural urges and less fluid intake.

They were prevalent during ancient times and continues to be seen even today also. It is estimated that approximately 2% of the population have Mutrashmari in their life with male female ratio being 2:1

To get rid of this terrible problem people surrender to the modern treatment which is quiet expensive and out of reach of common people along with that having their own demerits. So the safe remedy is the need of the day.

In Ayurveda, Acharyas have mentioned different types of treatments in the management of Vataja

Mutrashmari. Among them Tilanalaka Kshara Prayoga is mentioned as the best in the management of Vataja

Mutrashmari. Since the Kshara is having the properties of Chedana, Bhedana, Lekhana, due to these properties the

Kshara may disintegrate the stone into small pieces. Because of its Tridoshaghnata properties it corrects the derranged vata, thus vatanulomana occurs, which may help to pass the disintegrated Ashmari through the urethra.

Hence, the present study will be undertaken to study the concept of Mutravaha Srotas and to assess the effect of Tilanalaka Kshara in the management of Vataja Mutrashmari. 6.2 REVIEW OF LITERATURE :

Srotas nirukti, paribhasha, paryaya, bheda, swaroopa, srotodushti hetu, and its

karmas are mentioned in classics. 1.

The description of Mutravaha Srotas with its moola, samanya hetus, dushti laxanas and samanya chikitsa is mentioned. 1.

The Nidana, samprapti, laxanas and chikitsa of Vataja Mutrashmari is explained 2.

Tilanalaka Kshara is explained as the best in the management of Ashmari. 2.

Tiladi Kshara is mentioned as remedy for Vataja Mutrashmari. 2.

Paneeya Kshara is helpful in the management of Gulma, Udara roga, Sharkara, Ashmari, Abhyantara vidradhi etc. 3.

6.3 OBJECTIVES :

1. The detail study of Mutravaha Srotas.

2. In detail study of Vataja Mutrashmari.

3. To asses the the effect of Tilanalaka Kshara in Vataja Mutrashmari.

7. MATERIALS AND METHODS:

MATERIALS : 1. Daignosed patients of Vataja Mutrashmari.

2. Tilanalaka Kshara with Madhu and Dugdha as Anupana.

3. X-ray and USG of KUB.

METHOD : Tilanalaka Kshara orally in dose 2 Ratti (250mg) of twice in a

day with Madhu and Dugdha as Anupana.

7.2 METHOD OF COLLECTION OF DATA :

Classicaly diagnosed 30 patients of Vataja Mutrashmari will be selected by Random Sampling procedure with respective of age 16 – 60 years, irrespective of sex, occupation socio-economic status from OPD, IPD and

Camps conducted by Dr. B.N.M.E.Trust’s Shri Mallikarjun Swamiji Post-Graduate and Research centre, Bijapur.

STUDY DESIGN :

The mode of study will be efficacy analysis of 30 patients. The Tilanalaka Kshara will be given orally in a dose of

2 Ratti (250mg) with Madhu and Dugdha as anupana for 30 days. Mean while the patients will be asked to come for followup after 15 days . The patients will be observed both Subjectively and Objectively before treatment, after treatment by grading the symptoms. The post treatment follow up will be done after 15 days.

Total study duration will be 45 days. SAMPLING PROCEDURE :

The 30 patients of daignosed Vataja Mutrashmari will be selected by Random Sampling procedure excluding dropouts. The mode of study will be efficacy analysis of 30 patients.

The sample size will be 30 (minimum) excluding dropouts.

STATISTICAL ANALYSIS :

Statistical Analysis will be done by student ‘ t ‘ test. The patients will be observed both Subjectively and

Objectively before treatment, after treatment and follow up by grading the symptoms.The data will be collected. The intervention will be compared and analyzed by using student ‘t’ test consulting the Biostatistician.

PARAMETERS:

1. Subjective : 1. Shoola.

2. Chardi.

3. Jwara.

4. Sarudhira mutrata.

5. Vishirnadhara.

6. Mutradhara sanga.

7. Avilata mutrata.

8. Gomedakopana.

2. Objective : 1. X-ray of KUB.

2. USG of KUB

The patients will be observed both Subjectively and Objectively before and after treatment.

INCLUSION CRITERIA :

1. Diagnosed patients of Vatatja mutra ashmari according to classical signs and symptoms.

2. Patients between the age group of 16 - 60 years.

3. Patients of both the sex.

4. Size of the Ashmari ranging from 3 to 7 mm.

EXCLUSION CRITERIA:

1. Patients other than the disease of Vataja mutrashmai .

2. Patients between age group of below 16 and above 60 years.

3. Pregnancy.

4. Patients who are unfit for Kshara Karma.

5. Size of the Ashmari below 3 mm and above 7 mm.
