These guidance notes are to help you complete the Denbighshire County Council Health & Safety Pre Qualification Questionnaire. Please read these in conjunction with the questionnaire. Any YES answers in the questionnaire must be supported by evidence. Answering YES and not supplying evidence will not get you the points allocated.

1.5.1 Please confirm level of company turnover for the last two financial years. Please provide relevant extracts of accounts for the most recently completed two financial years to substantiate levels of company turnover.

1.6 To apply for work in the relevant categories you must be registered with the appropriate body. If you tick Yes you must provide evidence of current registration with your application.

1.7 You are required to hold a current permanent Employers Liability Insurance Policy to a minimum cover of £10,000.000 (Ten Million Pounds) in respect of any one occurrence. Please give details below of your Employers Liability Insurance and provide proof of cover. Copy of Current Certificate to be enclosed.

1.7.1 You are required to hold a current permanent Public Liability Insurance Policy to a minimum cover of £5,000.000 (Five Million Pounds) in respect of any one occurrence. Please give details below of your Public Liability Insurance and provide proof of cover. Copy of Current Certificate to be enclosed.

1.8 Provide details of any incidents within the last 5 years, which have resulted in either the issue of an improvement notice, prohibition notice or prosecution under The Health and Safety at Work etc Act 1974 of any other health & safety legislation. (If you have you will also need to say yes to 6.1 and/or 6.2 )

1.9 Please state the number of directly employed staff, names and job designations and Health & Safety Qualifications. This can be completed manually or if you have a training matrix that can be sent. • General Statement of Safety Policy • Organisation details • Arrangements

INDG 259 (rev.1) - an Introduction to Health and Safety, pages 20-30 of this publication, covering policy statement, responsibilities and arrangements will, if properly completed, satisfy our requirements. Please remember to sign and date the Policy Statement.

2. Health and Safety Organisation and Arrangements

2.2 You policy must show the responsibilities of the different groups or people within your organisation. See INDG 259 for more details

2.4 The law says you must appoint someone competent to help you meet your health and safety duties. Please supply evidence of competence, CV, training, etc.

2.5 This can be as 2.4 for a small company where the competent person named above will be on site. For larger companies who will be in charge on a day to day basis and what evidence can you provide? CITB site management safety training scheme certificate or an equivalent, SMSTS trained managers.

2.6 Please provide evidence of how you ensure any sub contractors used are competent, a contractors questionnaire for example. You need to satisfy yourself that your subcontractors are competent (ie they have sufficient skills and knowledge) to do the job safely and without risks to health and safety. How do you do this?

If you are a decorator, working at height, painting & sanding, preparation possibly, a groundworks company, excavation, concreting & traffic control for example.

2.8 Please provide at least 3 site specific risk assessments / 3 site specific method statements that are relevant to your activities. There needs to be references to risks that were specific to the site and not just any site. Examples might be proximity to water, overhead cables, school, sloping ground.

2.9 Please explain how you ensure your employees are made aware of any site specific risks on sites they visit.

2.10 If there is an emergency such as a fire or an accident, what are your employees expected to do and how would they know what to do?

2.11 Over 33% of accidents are attributable to slips, trips and falls. 95% of major slips result in broken bones. How do you ensure your activities do not put your employees or others at additional risk?

2.12 Please supply COSHH risk assessment examples. (Not MSDS sheets)

2.13 Please provide evidence of machinery certificates, PAT testing, vehicle checks.

2.14 If an employee has an accident, what are they expected to do? How do you communicate this to them?

2.15 By law, you must consult your workforce about: any change that may substantially affect your workforces’ health and safety. Such changes may include: new or different procedures, types of work, equipment, premises, ways of working (for example, new shift patterns). Please explain and provide evidence of how you do this.

2.16 Monitoring and reporting are vital parts of a health and safety culture. Please provide evidence of how you achieve this, for example, site audits, safety tours etc.

2.17 Please provide details of effective systems you may have developed to ensure the protection of children or vulnerable persons who may be present in or around the work area. The names of employees who have had enhanced CRB (Criminal Records Bureau) checks may be supplied where CRB legislation permits.

2.18 Workplace transport incidents are the second most common cause of fatalities after falls from height. When people and vehicles collide, people come off worse. What controls do you have to prevent this

3.1 Please provide evidence of membership to any trade associations you belong to.

3.2 Please provide evidence of the training your employees undertake (example training certificates, training plan, training matrix, CSCS, CCNSG cards, IIP membership etc)

3.3 Please provide evidence of First Aid provisions. If you do not provide first aiders, how do you ensure such provision is available for your employees?

3.4 Please provide details of the PPE identified in some of your risk assessments. How do your employees obtain the PPE?

3.5 In 2008/2009 over 4000 major injuries were caused by falls from height at work. All work at height must be properly planned and organised; those involved in work at height must be competent. Please provide evidence of any training you have provided for working at height. Training certificates, tool box talks etc.

3.6 Please provide evidence of manual handling training done, certificates, toolbox talks etc.

3.7 Mechanical handling techniques have improved efficiency and safety, but have introduced other sources of potential injury into the workplace. Cranes or hoists and fork lift trucks are the primary means for mechanical handling. Please provide evidence of training for your employees to use these aids.

3.11 The Considerate Constructors Scheme is the national initiative, set up by the construction industry, to improve its image. http://www.ccscheme.org.uk/ . Sites and companies that register with the Scheme sign up and are monitored against a Code of Considerate Practice, designed to encourage best practice beyond statutory requirements. If you are member, please provide evidence.

Please note: Evidence of this approved training will be required of every operative engaged by yourselves in construction related activities on our premises.

5.0 Environmental Management

5.1 If you answer Yes, please supply a signed and dated copy of your Environmental policy statement.

5.2 Do you take waste away once you have completed your work?
