 Adding New Student to CRM o If students are currently on an IEP or IFSP or 504 Plan with/without service time and you need to add them to the database provide Yvette Baker at, [email protected] , with: . First and last name of the student . Date of birth . Grade . School Building . School District . Name of Regional Director, AEA vision liaison, TVI and OMs . Parent’s name and address

 Procedures for 504 Students-When students on 504 plans do not require services from IESBVI, staff will inform Yvette that student is to be deactivated on CRM.

 Exiting Students from Services-When a student is being exited from IEP/IFSP services or graduating, notify Yvette Baker at [email protected] to let her know the student’s name and why they are being exited.

 TVI/OMS add Services or send this information to Yvette to be entered for a new student.

CRM-Instructions Rev 7/31/17 o Service time is determined by the IFSP/IEP team so the service time on the IFSP/IEP and CRM will be the same. . IFSP students will always be considered direct service o Travel time-Enter the number and select from the dropdown boxes for Units of Measure and Frequency. When calculating your travel time use a typical day to represent that student and use your professional judgment. The calculation will reflect your mileage log. For example: . If you typically go from your office to a student and back again, calculate from office to student and back to the office. . If you typically go from a starting point and then serve student to student, calculate from the time from the starting point to the first student (that time is given to the first student) and then calculate from student to student. o Material preparation- Enter the number and select from the dropdown boxes for Units of Measure and Frequency. Preparation related to a specific student will include the following: adapting materials (not otherwise adapted by the paraeducator), AIM/NIMAS, Bookshare applications, Borrowing/trialing/returning resources for specific student, Cleaning toys and manipulatives, Collaboration sessions with schools (time not included on the IEP Activities tab), Creating new materials, Extended Learning Programs/ESYS, FIE consent, Lesson Planning, Obtaining and reviewing doctor reports, Ordering materials and equipment for specific student, Providing professional development and training to LEA/AEA staff, Providing technology support to student and IEP tram, Store trips to purchase materials, Transition activities, Review of core curriculum materials
