Enter • your personal information. DO NOT enter a business name or a fictitious business name, LEAVE IT BLANK unless you are an LLC and want your checks to be made out to your business name. If you are a business owner with a license & DBA/fictious business name then Sign • up under your EIN. You will need to email [email protected] to submit your business documents before you will be able to order without sales tax. REMEMBER- you will be legally responsible for collecting and submitting sales tax on a quarterly basis. Select new distributor kit (NBK), which is the $55 ANNUAL enrollment fee. NDK in- • cludes Distributor Handbook, Career Opportunity DVD, Business Forms, Order Forms and Beauty Book. You will see options to purchase the lips kit or the glamour kit. It is highly recom- • mend to SKIP both these options, finish signing up and THEN work closely with your sponsor to place your first 300pv order. If you have not talked to your sponsor about skipping these two options and placing your first 300pv order, stop what you are doing and contact them before proceeding any further. Enter your shipping, billing info and payment information. BE SURE that your infor- • mation matches your billing information for your card EXACTLY, even down to the CVV & middle initials. With the updated security from chip cards your payment will be declined by the card company if it is not perfect. Read over terms and agree- ments and submit.
