Regional Associations need to complete this form on behalf of their clubs attending. Sheets must be completed and accompany entry documents. Event passes will be allocated to Coaches and Managers based on this information. If names are not provided for club coaches and managers, a pass will not be created and general admission fees will be required to be paid.

Entries and Reports are to be emailed to [email protected] by midnight Monday 18 August, 2014. Late entries will not be accepted. CLUBS check with your REGIONAL ASSOCIATION for their entry closing date.

Ratio of Coaches/Managers to Swimmers Clubs are entitled to nominate coaches and team managers based on the number of swimmers they have competing, as follows: 1-10 swimmers 2 coaches/managers 11-20 swimmers 3 coaches/managers 21-30 swimmers 4 coaches/managers 31+ swimmers 5 coaches/managers Teams may travel with any combination of coaches and managers.
Regions will be sent an invoice for entry fees once entries have closed. Post a cheque or make a bank payment for this invoice using the invoice # as a reference. Cheques can be sent to Swimming New Zealand, P.O. Box 302145 North Harbour 0751.
