NATIONAL HONOR SOCIETY APPLICATION
In order to apply follow these steps:
o Save the application onto your computer or flash-drive. o Fill out the following application on the word document. o Print a hard copy of the application (already filled out) and the rest of the packet (do not print this page). o Obtain the necessary documents such as recommendation letters and attendance record. o Bring a hard copy of the document ON May 16, 2013 to room 4306 between 4:00 pm and 5:00 pm. (If you will not be in school that day you must provide proof and bring your packet earlier). o If your packet is incomplete on the day of submission, it will not be accepted. o Sign up for an after-school interview for the week of September4, 2012 at the time of submission. Please return this sheet as the cover sheet for your packet. Please place the forms in the following order:
______1. A copy of your transcript (The minimum GPA for membership is a 90.)
______2. A copy of your current attendance record (This should be obtained from one of the clerks in the attendance office.)
______3. A signed form (part of the packet) from a vice-principal stating that you have no disciplinary problems
______4. Two letters of recommendation from teachers, counselors, or administrators who are NOT sponsors of any organizations to which you belong
______5. A form (part of a packet) listing your extracurricular, co-curricular and community activities/contributions (Mandatory community service does not count. Neither does any school activity or certification that is a COURSE REQUIREMENT, such as hospital rotations for HPA students)
______6. A proof-of-participation form (part of the packet – make copies as necessary) signed by applicable coaches, sponsors, mentors, and/or anyone else in charge of the activities in which you are involved
______7. A typewritten essay addressing the following: How I Have Been a Positive Influence in My Community (community includes school, neighborhood, city, church etc.) Disciplinary Record Verification

To be considered for membership in the National Honor Society, a student must complete and return all required forms. Please provide an honest and comprehensive picture of your activities and achievements. This information will be used only by the Faculty Council to evaluate prospective members. Completion of this application does not guarantee selection. Requirements necessary for consideration include the following:
1) 90 or higher cumulative GPA 2) Classification as a junior or a senior 3) Attendance at Socorro High School for at least one complete semester 4) Sponsor verification forms 5) Vice-principal disciplinary verification form 6) Attendance verification form 7) Current transcript 8) Letters of recommendation 9) Personal essay
In addition to scholarship requirements, the Faculty Council will evaluate service, leadership, and character. Please do not list as service activities or community activities, anything that is required for participation in your academy. PLEASE TYPE YOUR RESPONSES. If you need additional space, please attach only one piece of paper with additional listings.
Co-curricular Activities – Please list all activities in which you have participated during high school. Include clubs, teams, musical groups, etc., and major accomplishments in each.
Activity Year Sponsor Accomplishments Leadership Positions – List elected or appointed leadership positions held in school, community or work activities. Only those positions in which you were directly responsible for directing or motivating others should be included. For example, you may list elected student body, class, or club officer, committee chairperson, newspaper editor, work area manager, community leader, church leader.

Service Activities – List services activities in which you have participated. These can be service projects done with a group, either in or out of school,(or done as individual service performing projects.) Generally speaking, service activities are those which are done for or on behalf of others, not including immediate family members, and for which no compensation has been given.

These should be activities outside of school in which you participated for the betterment of the community. For example, you may list clubs such as Boy or Girl Scouts, community fine arts endeavors, or any proactive group with which you have worked.

Coursework – List Advanced Placement, Pre-Advanced Placement, Dual-Credit and weighted courses for which you have received credit.

Let’s get started, first off you MUST have obtained a GPA of a 90. If you fail to meet this requirement, do not apply.
Thing you need:
1. A copy of your transcript. A transcript can be obtained in the counselor’s office. 2. An attendance record. An attendance record may be found in the attendance office. 3. A filled out verification of disciplinary record form. Go to the front office with your verification form in hand and ask the front office secretary to deliver your request to the vice-principal in order to be cleared. Please keep in mind that it may take 3 to 4 days in order for your verification to be completed. 4. Two letters of recommendation. Make sure to select a teacher who can substantiate your character, academic and leadership qualities. Give your teacher a week to write the recommendation. To help teachers, it would be wise to provided them with a resume. 5. The proof of participation forms. In order to increase your chances of becoming a member of the NHS, be sure to get as many participation forms filled out by your coaches and/or sponsors. Make several copies of this form which is included in the packet. 6. A type written essay on how you have been a positive influence in your community.
