Student Activities- List all school related activities in which you have participated in since the beginning of 9th grade, activity, role or position, and name of coach/adviser with school name. These can be individual or non-school related group service hours/projects done outside of school. Generally, service activities are those done for or on behalf of others (not immediate family members) for which no compensation (monetary, requirements, or otherwise) has been given or group you are a member. For each “group”, attach a service hour form or letter of proof of hours from the same supervising adult; which includes dates, times duties, location, and supervisor’s signature and contact number (letter head). Ask for clarification of any hours before submitting the application. NO E-mail letters of proof will be accepted.
