NATIONAL FOOTBALL FOUNDATION ALL LONG ISLAND HIGH SCHOOL FOOTBALL TEAM NOMINATION FORM
The National Football Foundation & College Hall of Fame, Inc. (“NFFCHF”) has been in existence since 1947 and is dedicated to promoting the sport of amateur football and honoring those individuals who serve the game. On a local level there are 119 chapters with 12,000 members. Since 1995 our chapters in Nassau and Suffolk counties have provided over $160,000.00 to deserving high school students who have achieved academic and athletic excellence.
The NFFCHF has decided to select our first ever All Long Island High School Football Team. It will be a first of its kind, a by position team selected solely by the coaches. The Section Chairmen from each county will designate five (5) coaches from each county to select the team. The offensive and defensive team will consist of twelve (12) positions each (1 quarterback, 1 center, 1 tight end, 4 linemen, 2 running backs, 2 wide receivers and 1 all-purpose player on offense and 4 defensive linemen, 3 linebackers, 4 defensive backs and 1 all-purpose player on defense). All honorees will be honored by the NFFCHF in April along with our inaugural class of inductees into the Long Island High School Football Hall of Fame.
Directions:
1. Nomination Form should be completed by the Head Coach and mailed to the NFFCHF at 97 Joludow Drive, Massapequa Park, New York 11762 or sent via fax at (516) 541-4710.
2. The deadline for applications to be received is Friday, January 14, 2005.
3. It is strongly recommended that videotape or your candidate be submitted with the Nomination Form. No highlight tapes will be viewed, rather one (1) full game tape or ½ of one (1) full game should be provided.
4. Students selected to the team will be notified under separate cover and their Head Coaches will be copied on all correspondence.

Forms can either be mailed to The National Football Foundation & College Hall of Fame at 97 Joludow Drive, Massapequa Park, NY, 11762 or sent via facsimile at (516) 541-4710.
It is strongly recommended that videotape is submitted for each candidate. NO highlight tapes will be accepted. A full game or one complete half of footage should be forwarded for review.
