Drop Off Information: All Entries should be dropped off at the Presentation room in Plano West on Monday November 21st 2005 from 10 a.m to 12 noon.

Color Coded Dot System:

The dot system helps us to sort the artwork quickly. Please apply sticky dots to the TOP RIGHT-HAND CORNER OF THE FRONT OF THE ARTWORK. Apply stickers according to the following table: Grade Dot Division Color PreK-K White 1-2 Red 3-5 Blue 6-8 Yellow 9-12 Green Special Ed. Orange

Bring the following items to drop off along with the artwork:

 Form A: 2 copies, indicating the total # of entries received and total # of entries forwarded.  Inventory sheet: master list of all work being submitted -- includes last name, first name, grade, age, category code, title, school, chairperson’s name, chairperson’s phone number. Category Use the table above, replacing the Category Code samples with your own data. Shrink wrap or acetate wrap is recommended for protection. The Official Entry Form is to be placed in a sheet protector and taped with masking tape to the back of the entry.  The Local 8 digit National PTA ID Number and Local Unit Number are found in your PTA Presidents Handbook. I have a list of these numbers if you cannot locate yours.
