Directions for both New and Renewal Applications are easy:

1. State of Wisconsin Four-Year Commission Notary Public Application Form a. Complete sections 1 through 13 carefully, making sure everything is correct. b. Take and pass the Notary Exam at www.wdfi.org. Print the certificate to send with the forms and checks.

2. Bond Instructions: Notary Public Bond/Oath of Office Form a. Complete sections 1 through 6 on the Bond form. DO NOT COMPLETE SECTIONS 7 – 13, (Notary Bond Renewal Service will complete 7 – 13) b. Turn form over to Oath of Office form. 1. Have another Notary witness your signature (#1-2) and then they complete sections 3 through 9.

3. Enclose 2 Check(s) a. One made out to: Notary Bond Renewal Service for $25.00 for the four-year, $500.00 bond. b. The second check made out to: Wisconsin Department of Financial Institutions for $20.00 for the filing fee. (unless fee was prepaid online).

4. Mail the following to the Notary Bond Renewal Service: a. Completed Application Form b. Completed Notary Public Bond/Oath of Office Form c. Certification for completed exam d. Check(s) payment

We will sign the bond on behalf of the Surety Company and file your application with the Wisconsin Department of Financial Institutions. You will get an e-mail from [email protected] notifying you of your Notary commission appointment. If you are renewing your notary, Madison will hold your paperwork until your expiration date to give you your entire four years’ worth. You can also look up commission dates at www.wdfi.org.
