National Quality Center is pleased to provide this guidance and support around the upload of CAREWare performance measures for the end+disparities Learning Exchange Disparities Calculator.
Instructions 1. Save each of the NQC Disparities XML files to a location that’s easy to find such as your desktop 2. Log into CAREWare, and go into the Performance Measures Module 3. Import the NQC Disparities XML files by selecting F10 (Load From file) as in this screen shot
4. Now select F3 Multiple Performance Measure Report. Select the NQC Disparities Measures only and run the report. Below is a sample report from a fake database.

***Be aware of how the numerator and denominator is expressed in each!!
If you show these measures on the client’s Performance measures tab, there may be a green check () for an item that really should have a red X!!!
NQC Disparities - All Patients: Clients in the denominator are all patients who have been touched by your care system. This is to allow case management agencies to participate in measurement. Clients in the numerator are those from the denominator whose most recent viral load is less than 200 copies/mL (viral suppression) and those who had a visit in every six month period for the last 24 months (medical visit frequency). For medical agencies, it will be imperative that the data are cleaned to remove patients who are not official patients of the hospital or clinic.
NQC Disparities - AA-Latina Women: Clients in the denominator are African-American and/or Latina women patients who have been touched by your care system. This is to allow case management agencies to participate in measurement. Clients in the numerator are those from the denominator whose most recent viral load is less than 200 copies/mL (viral suppression) and those who had a visit in every six month period for the last 24 months (medical visit frequency). For medical agencies, it will be imperative that the data are cleaned to remove patients who are not official patients of the hospital or clinic.
NQC Disparities - Youth: Clients in the denominator are patients aged 13-24 who have been touched by your care system. This is to allow case management agencies to participate in measurement. Clients in the numerator are those from the denominator whose most recent viral load is less than 200 copies/mL (viral suppression) and those who had a visit in every six month period for the last 24 months (medical visit frequency). For medical agencies, it will be imperative that the data are cleaned to remove patients who are not official patients of the hospital or clinic.
NQC Disparities - MSM of Color: Clients in the denominator are MSM of color patients who have been touched by your care system. This is to allow case management agencies to participate in measurement. Clients in the numerator are those from the denominator whose most recent viral load is less than 200 copies/mL (viral suppression) and those who had a visit in every six month period for the last 24 months (medical visit frequency). For medical agencies, it will be imperative that the data are cleaned to remove patients who are not official patients of the hospital or clinic.
NQC Disparities - Transgender: Clients in the denominator are transgender patients who have been touched by your care system. This is to allow case management agencies to participate in measurement. Clients in the numerator are those from the denominator whose most recent viral load is less than 200 copies/mL (viral suppression) and those who had a visit in every six month period for the last 24 months (medical visit frequency). For medical agencies, it will be imperative that the data are cleaned to remove patients who are not official patients of the hospital or clinic.
