SOIL AND WATER CONSERVATION DISTRICT 2016 SALARY & BENEFITS CONTRACT
This Salary and Benefits contract by and between the ______County Soil and Water Conservation District, herein referred to as the EMPLOYER, and ______herein referred to as the EMPLOYEE, sets forth certain conditions and agreements as follows:
Employment policies and minimum standardized benefits provided to the EMPLOYEE, are contained in the Soil and Water Conservation District Operational Handbook, Personnel Section, and upon execution of this contract are agreed to by the EMPLOYER and EMPLOYEE.
A. EMPLOYMENT POLICIES (Each blank must contain N/A or a number.)

FOR INTERMITTENT/PART-TIME EMPLOYEES ONLY: Employee is retained on an intermittent/part- time or as needed basis and shall work no more than hours per pay period. The Employee is to be paid $ ______per hour.
B. PAYMENT OF SERVICES (Each blank must contain N/A or a number.)
1. Mileage shall be reimbursed at _____¢ per mile for private automobile use when conducting EMPLOYER business. EMPLOYEES using personal vehicles for business purposes are required to carry insurance at least equal to the amount required by state law for personal property damage and bodily injury. EMPLOYEE shall provide EMPLOYER proof of auto insurance.
C. BENEFITS (Each blank must contain N/A or a number.)
1. Health Insurance: Individual group health insurance coverage is provided for eligible EMPLOYEES. This coverage will become effective on the first day of the month following sixty days of employment provided the EMPLOYEE has completed and mailed the proper application. EMPLOYEES may apply for additional, optional group health insurance coverage beyond the basic single coverage provided for by the EMPLOYER. All premiums for optional coverage that are the responsibility of the EMPLOYEE will be paid through payroll deductions.
2. Life Insurance: All eligible employee(s) participating in the group health insurance plan are provided $15,000 life insurance and long-term disability insurance. The EMPLOYER will provide $______to purchase additional life insurance for the EMPLOYEE.
3. Additional benefits in the manner of health insurance and/or short-term disability insurance (circle all that applies) will be provided to the employee with the district paying $______and the employee paying $______.
4. Social Security: The EMPLOYEE and EMPLOYER will each pay one-half of Social Security taxes. The EMPLOYEE’S share will be deducted from his/her gross salary through payroll deduction.

All full-time employees and regular part-time employees shall earn vacation at a rate commensurate with their years of service. Full-time and regular part-time employees serving a probationary period earn vacation, but may not use it during their probationary period.
D. FUNDING EFFECTS UPON CONTRACT
The provisions of this agreement are contingent upon the financial support the EMPLOYER receives from State, local or other sources. The EMPLOYER reserves the right to renegotiate or cancel the contract if adequate State or local funding is not received.
E. PERSONNEL POLICIES
Upon signing this contract, the EMPLOYER certifies that the EMPLOYEE has been provided a copy of the EMPLOYERS current personnel policies and the EMPLOYEE certifies that the EMPLOYERS current Personnel Policies have been received, read and understood by the EMPLOYEE.

No amendments to this agreement shall take affect until approved by the EMPLOYER and the EMPLOYEE in writing.
